Health Report - 03/24/2015 (3 critical incident(s))

(Critical) On follow up inspection found no food prepared and packaged by a food processing plant passed it's use by date. Pic states they have changed their process of tossing food passed it's use by date, by immediately discarding food, instead of leaving food in cooler to record as waste later. This violation was corrected on routine inspection, and reverified on follow up.

(Critical) On follow up inspection used a quat test kit and found quat sanitizer flowing from quat dispenser in the 3 compartment sink at about 150 to 200 ppm, within manufacturer's recommended range of 150 to 400 ppm. Pic states quat dispenser was adjusted to dispense within manufacturer's recommended range. Violation corrected.

(Critical) On follow up inspection found the water spout of the ice cream dipper well at least 1 inch above the rim of the ice cream dipper well. Pic states water line was secured to holding bracket to provide air gap. Violation corrected.

(Critical) Found the water spout of the ice cream dipper well with out air gap, that is below the rim of the ice cream dipper well. To correct properly repair spout holder to securely hold spout at least 1 inch above the flood rim of the ice cream dipper well.

(Critical) Found food prepared and packaged by a food processing plant inspected by a regulatory authority, not honestly presented and passed it's use by date, pasta salad 2/6/15, coleslaw 2/6/15, and potato salad 2/13/15 in the walk in cooler. Pic corrected by discarding commercially prepared food not honestly presented and passed it's use by date. Pic has been instructed to monitor and ensure commercially prepared food is not passed it use by date.

(Critical) Used a quat test kit and found the sanitzing solution in the sanitizing buckets and 3 compartment sink below manufacturer's recommended range of 150 to 400 ppm. Pic states quat dispenser broke and staff were manually mixing solutions. Pic corrected by adding more quat sanitizer to solutions and used test kit to verify proper concentration.

Found shake toppings (cookie crumbs) not covered in the shake prep area. To correct cover cookie crumbs.

Found the pop nozzles in the self service pop machine soiled with build up and accumulation. Pic states pop nozzles are cleaned daily. To correct follow manufacturer's directions and clean pop nozzles.

Found the floor under equipment, edges and corners in the self service pop machine, front counter prep area, and back kitchen area soiled with accumulation and build up. To correct thoroughly clean floor and clean on a more frequent schedule.

Found heavy dust build up in the ducts of the oven hoods. To correct clean ducts.

Observed water dripping from drain line of what appears to be the drip tray of the hot water heater above the mop sink. To correct verify source of leak, and repair if necessary.

Found the quat dispenser in the 3 compartment sink with a broken hose connection (which attaches to quat supply solution). To correct repair quat dispenser hose connection, so quat dispenser functions properly.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, opened, unsliced pieces of salami, ham, mortadella, roast beef, and capicolla without 7 day (day opened + 6 days) datemark on the shelf in the walk in cooler. Pic corrected by datemarking noted items, and has been instructed to monitor and ensure employees are datemarking rte, tcs food. ,

(Critical) Found the drain line of the ice machine above the pop machine draining into receptacle without air gap. To correct provide an airgap of at least 1 inch from the drain line and rim of the receptacle. ,

(Critical) Found the edges and surfaces of the slicer surrounding the slicer blade soiled with food debris. Found the can opener blade soiled with accumulation. To correct follow manufacturers directions and clean slicer and can opener. ,

Found no label on the salad oil dispenser in the back kitchen shelf. To correct label salad oil dispenser.

Found light accumulation in the pop nozzles of the self service pop machine. To correct follow manufacturer's directions and clean pop nozzles.

Found the floor under equipment, edges and corners in the front counter area and storage area soiled with accumulation of debris. To correct thoroughly clean floor and clean on a more frequent schedule.

Found the exhaust duct in the cooking/prep area soiled with heavy dust build up. To correct properly clean ducts.

Found lid of the dumpster broken and laying on the floor next to dumpster. To correct call for repair so dumpster can be closed.

Found several faucets in the front counter and kitchen leaking water. To correct properly repair faucets to be free of leaks.

At the dishwashing area observed the grease trap to leak water on the floor when the wash compartment of the three compartment sink is drained. Staff was observed using a squeegee to direct the water to a floor sink. The water was not observed to affect any food storage areas. Correct by repairing or replacing the grease trap so it does not leak on the floor. Pic indicates this has been an ongoing repair issue. A maintenance contractor is scheduled to attempt another repair on the drain at the end of this week.

(Critical) Observed no paper towel in the dispenser at the handwashing sink adjacent to the dishwashing area. Pic corrected during the inspection by installing paper towel in the dispenser. Item corrected.
