The following non food contact surfaces need cleaning: the outside of the blender containers, the interior of the upright self serve freezer, the interior bottoms and shelvings and doors of all upright coolers and freezers, the shelving in the walk in cooler and freezer, the outside and base and backsplash control panel of the waffle maker. Clean now and place on a more frequent cleaning schedule

The light bulb is burnt out in the walk in freezer. Replace bulb so that there is adequate lighting in the walk in freezer

The paper towel dispenser cover is broken and not staying closed. It is held closed by a heavy container sitting on top of it. The handle on the walk in freezer door is broken and loose. The gasket on the self serve freezer in the lobby is torn. Once torn it is no longer cleanable. Replace split gaskets. Repair or replace the paper towel dispenser and handle for the walk in freezer door

The floors in both walk in units need cleaning. Clean now and place on a more frequent cleaning schedule

Covered waste receptacle is missing for the unisex restroom. Provide a waste receptacle with cover for proper disposal of tampax. Correct as indicated above

(Critical) Assorted food contact surfaces, including knives, spatulas, cutting boards, blenders not being cleaned at required frequency of cleaning. Clean now and place on a more frequent cleaning schedule as indicated above.

(Critical) Bulk soap with dispenser is sitting on ledge of handsink. The dispenser is missing and the soap does not come throught the tube. There is a hole in the bag that may or may not dispense soap when squeezed. Provide approved soap with dispenser or pump soap so that the soap may be easily obtained for proper handwashing.

(Critical) The containers for the blender are broken/cracked and no longer cleanable. Replace with new containers that are smooth and cleanable. These are ordered out of service and may no longer be used as they cannot be properly cleaned. Correct by discarding and purchasing new containers that are smooth and cleanable

(Critical) The spray rinse arm and faucet is in complete disrepair. When turned on water flows out from multiple locations rendering this faucet/spray rinse arm inoperable. The 3c sink is being filled from the mop sink via attached hose. Repair the faucet/spray rinse arm for the 3c sink so that it is usable. Correct immediately so that the sink is usable. It is prohibited to fill the 3c sink to wash dishes with the hose from the mop sink.

Lights in the walk in cooler are burned out. Replace bulbs so that there is light in the cooler so employees can see properly. A minimum light intensity is required as indicated above for proper lighting.

Thermometers missing from the reach in cooler and reach in freezer at the front counter. Provide thermometers for these units and elsewhere if missing in order to properly monitor the temperatures of the units

Assorted containers of foods found stored on the floor in the walk in unit. Keep all containers a minimum of 6" above the floor to allow access for routine cleaning.

Cleaned utensils are being stored in dirty containers and are filled with water from not properly drying utensils. Keep clean utensils in clean containers.

The facility needs a thorough cleaning top to bottom. The floors inbetween the display cases and under equipment are coated with food debris. Associated electical boxes on floor at front counter are coated. Drains are plugged with food debris or coated with dirt. Clean now and place on a routine cleaning schedule to eliminate dirt and harborage conditions.

The handle for the stopper for the drain in the left side comparment of the 3c sink will not stay up so that the stopper stays closed. The facility is using a waste basket that has a hole in it at the right level to keep handle up to keep water in the sink compartment. Repair handle for stopper and discontinue use of the wastebasket for this purpose. Wastebasket had water and unknown substances in i evident of being there for a while repair and then remove wastebasket and clean it out.

The cover for the paper towel dispenser is broken and will not stay closed. Replace paper towel dispenser. Shelves are hanging off the hinges in the reach in freezer at front counter. Repair shelves. Door is missing from the upright freezer in the rear causing excess ice build up. Gaskets on cooler/freezer doors are torn. Once torn they are no longer cleanable. Replace split gaskets.

(Critical) Hot water is not supplied to the front handsink due to a broken knob which is laying next to the sink for hot water. Repair so that hot water may be provided to this handsink for proper handwashing.

(Critical) The handsink was blocked for access for routine handwashing by large wastebasket parked directly in front of the handsink. Do not place anything in front of handsink or in sink basin in order to allow access for routine handwashing. Corrected by rolling handsink to opposite side of room at time of inspection

(Critical) Spray bottle with windex found hanging on the shelf on the rear storage rack with spray nozzle directly over single service plates. Store chemicals and cleaners so that they are in a separate designated location away from foods and food contact surfaces and single service articles to minimize potential for contamination of these items. Corrected by relocating windex to a bottom shelf away from single service plates

The spray rinse arm as a severe leak when faucet is turned on to fill up the 3c sink. Employee indicates plumber is due to come in this week to repair. Repair leak immediately.

Excess frost build up in walk in freezer at the floor door juncture most likely due to torn gasket. Severe ice build up inside the two door upright freezer. Unclear why ice build up is so severe. If leaking, repair. Also remove frost so that these areas are cleanable

The interiors of all coolers and freezers need cleaning, including the shelves in the freezers and especially the walk in freezer. Clean now and place on a more frequent cleaning schedule.

The entire facility needs a thorough cleaning from top to bottom. The floor wall junctures have excess build up, the cabinets under the front counters have excess build up, the counter backsplash/counter top juncture has debris accumulation. Clean now and place on a more frequent cleaning schedule

(Critical) Hot water is not being provided to the facility. The facility makes their own ice cream. It is essential that hot water be resupplied to the facility asap in order to allow for proper cleaning of utenisls and equipment and handwashing. Repair or replace hot water heater as it is not working.

(Critical) Paper towels are not provided for the rear prep area handsink. Provide paper towels for proper hand drying.

(Critical) The rear prep/dishwash area handsink was blocked for access for routine handwashing by large trash can in front of sink and yellow rubber gloves in the sink basin. The handsink must be accessible for routine handwashing at all times. Corrected by removing gloves and relocating the trash can

The base of the blender container which secures to the blender motor is cracked. Once cracked this is no longer smooth and cleanable. Replace blender containers with a minimum of two that are intact and smooth and cleanable. Two is recommended so that the blender container can be cleaned at required frequency with a clean replacement available for back up during the cleaning process of the one that has been in use.

The floors throughout the facility need cleaning especially at the floor wall junctures. Includes the floors in the walk in boxes too. Clean now and place on a more frequent cleaning schedule.

Thermometers found to be missing from both retail freezers up front in the lobby. Provide thermometers for these units in order to be able to monitor the temperatures of the units.

(Critical) The 3c sink is improperly set up with dirty dishes in the sanitize compartment, soap and water in the middle sink and sanitizer and water in the 1st compartment on the right which is plumbed to a grease trap. Set up sink with wash, rinse and sanitize from right to left as labelled. Corrected by discussion and proper sink set up at time of inspection

(Critical) The drainline to the handsink in the rear is broken making sink unavailable for routine handwashing. Repair drainline so that the sink is available for routine handwashing.

Called owner during inspection, he indicated that the glass has been broken for over a month. Cardboard. Used as a liner in the bottom of ice cream cakes were taped together and placed up against broken glass to prevent any glass from falling into display case. Told owner he should had contacted the health dept. When glass broke. The glass needs to be repaired asap. Owner said he would complete repaired with in two weeks. Inspector contact supervisor. Call health dept. At 734-222-3972 when glass is replaced
