(Critical) Observed the following food contact surfaces not clean to sight and touch: 1. Ice machine-ice chute, deflector, and interior of ice maker with a heavy accumulation of black, red, and orange residue 2. Clean utensil storage- several utensils with dried food debris 3. Pasta basket- in use pasta baskets in the rethermalizer with a heavy accumulation of historic pasta buildup correct by wash, rinsing, and sanitizing each item so it is clean to sight and touch

At the ventilation hood for the dishwashing machine observed the filter to be missing. Also, a thick accumulation of grease has formed on the vent piping. Chunks of grease were observed on top of the dish washing machine. It appears the chunks have fallen from the vent. Correct by cleaning the ventilation hood now and on a more frequent basis. Also, replace and maintain the ventilation hood filter
Observed the floors at the following areas with a heavy accumulation of food debris and grime: 1. Inside the wic 2. In the dish area, especially under the dish machine and sink 3. Around the water heater 4. Several area throughout the store where floor grout is missing. Correct by cleaning now and on a more frequent basis to prevent the accumulation of debris. Also, as noted under 6-501. 11°Floor grout replacement is needed in some areas. Missing floor grout provides a location for food debris to accumulate
Throughout the kitchen and prep areas observed several patches of tile with missing grout. Correct by replacing the tile grout
Observed the following items at the water heater in need of repair: 1. Missing front cover exposing friable fiberglass insulation to the dishwashing area. 2. Leaking condensate line on the vent pipe correct each by repairing of replacing.
At the dishwashing area clean dishware storage rack observed several stacks of clean pans wet stacked. Correct by completely air drying the pans before stacking for storage. This violation has been observed on several previous inspections. Please work on a solution for this violation
On the main cook line observed two separate single door reach in coolers with open door seals. Correct by replacing the door seals. The pic indicates the seals are on order and will be replaced soon

(Critical) On two separate occasions observed an employee use the handwashing sink, next to the pasta cooling sink, to rinse off and scrub two utensils after use. Also, observed two green scrubby pads on the handwashing sink. One of these utensils was a knife which was returned to the clean knife storage area without first being sanitized. Item corrected by the employee taking the knife to the dishwasher for proper cleaning. Also, the employee was educated about the need to properly sanitize utensils before use/storage after cleaning. Observed a mop stored wet sitting in the mop sink. No hook is available to hang the mop on. Correct by installing a hook to hang the wet mop on so it can thoroughly air dry
Observed the following pieces of equipment with dirty non food contact surfaces: 1. At the prep top unit used for salad preparation observed the inside bottom of the cabinet with an accumulation of decaying food debris and water. 2. At the prep top unit used for saut� item storage observed an accumulation of grime on the inside back blower cover. Also, on the inside top observed grime trapped between a trim piece and the cooler cabinet. Correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris and grime

(Critical) During the begining of this routine food service inspection hot food was found at 108 degrees f. Recently placed in an insert pan of the hot holding unit located at the kitchen prep area. Pic corrected this item by properly reheating the foods in a rethermalizer to above 165 degrees f. As required prior to the hot food being returned to the hot holding unit.

Observed the interior of the rethermalizer lower non-food contact surfaces of the equipment with debris in need of routine cleaning. To correct schedule more frequent cleaning of the cabinet of the rethermalizer located at the preparation area of the kitchen

Observed the dishwashing machine with a loose hose line at the top of the machine. During use of the dishmachine, water discharges from the loose hose line. The spray water from the loose hose connection splashing onto the walls of the dishwashing area of the kitchen. To correct the pic person in charge contacted ecolab to service the dishwashing machine at this facility. Observed the vacuum breaker for the garbage disposal missing a cap cover at the top of the vacuum breaker. To correct schedule service to replace the missing cap for the vacuum breaker located at the garbage disposal of the dishwashing area

Note this unit is designed to hold already properly cooled foods cold at 41 degrees f. And not to cool foods as was being observed with the nested insert pan of tomato. Note this refrigeration unit is operating properly at below 41 degrees f. As required. Pic corrected by relocating prepared tomato to proper refrigeration, and exchanging with properly cooled cut tomato. This item was corrected during this inspection. Pic corrected this item by providing tomato at a temperature of 39 degrees f. Acceptable temperature below 41 degrees f.

Obseved top section of refrigerated make unit at food cooking and preparation area of the kitchen to need mechanical repairs to function properly, pic has instructed employees to use ice as a method to hold potentially hazardous foods time/temperature control for safety foods at below 41 degrees f. As required. This is acceptable as a temporary correction as all foods in this unit were found to be cold holding at below 41 degrees f. As required. Repairs are required to be made to this unit to provide proper operation according to manufacturer specifications for cold holding of foods to 41 degrees f. Or below temperatures.
Observed walk-in refrigerator with door gasket missing sections. To correct schedule service to the walk-in refrigerator door to replace worn door gasket seal. To provide proper seal when door is in the closed position
Observed paper towel dispenser located at the back area hand sink with paper towel dispenser at this sink to not be operating properly, note around the other side of this hand sink is another paper towel dispenser that does operate properly and is equipped with paper towels, to correct this item you should schedule repairs to the paper towel dispenser to encourage proper employee hand washing at this hand sink
