(Critical) A non-food grade black rubber hose was used to supply water from the overhead spray nozzle supply pipe to a newly installed pre-soak chemical dispenser by gfs. Remove and replace this hose with one that meets food-grade standards

Bar: observed the ice transfer pail to be stored upright on a shelf. Corrected by inverting this pail on a clean serving tray. Also, observed a small stack of foam to-go food containers on a bar shelf. Corrected by inverting these containers to protect the food contact surfaces

Observed the floor sink under the bar counter to have debris build-up. Corrected by cleaning this drain

Observed the dispensing tube for the bulk milk dispenser to be +/- 3" long. Corrected by cutting off this tube so it only extends 1" out of the refrigerated compartment

Observed four light fixtures in the back kitchen storage rooms without light covers. Provide light covers to protect these light bulbs from breakage

(Critical) Observed a spray bottle with a blue liquid in it at the dish washing station to not be labeled. Also, at the bar storage room two unlabeled spray bottles were also found. Corrected by properly labeling these bottles as to their contents

(Critical) Large walk in cooler: a case of raw bacon was stored above a case of apples, and a container of raw marinating chicken was stored above green peppers. Corrected by relocating the apples to the smaller breakfast walk in cooler and moving the chicken to the bottom shelf below the vegetables

Observed three baskets of plastic ware with the utensils stored jumbled. Have an employee with clean hands organize these utensils so the handles are all oriented in the same direction toward the customer
