Observed ceiling in cooking area (near dry storage room entrance) in poor repair (plaster/paint is chipping and hole forming in ceiling). To correct, please resurface ceiling and repaint as soon as possible so all surfaces are smooth and cleanable

(Critical) Observed insert pans of rte chicken patties, chicken tenders, sliced ham and sliced turkey in 2 door upright cooler (cold cut prep cooler) with internal food temperatures of 53-74f. All other food items in same cooler cold holding at 40f. Staff stated all above mentioned food items were prepped less than 30 minutes ago. Upon further investigation, discovered that all of these food items are brought out and prepped/sliced at the same time. Please remember that all phf must be cold held at 41f or below at all times. Staff need to prep/slice/handle one food item at a time and leave other phf in cooler until ready to prep another item. Corrected by rapidly cooling all above mentioned food items in wif to 41f at time of inspection. Violation corrected.

(Critical) Observed several back up pizzas on speed rack in wic with internal food temperatures of 63-69f. All other food in wic was cold holding at 41f or below. When i first arrived for inspection, these same pizzas were being stored on speed rack with other hot cooked pizzas in middle of kitchen. Within 3 minutes after i arrived and identified myself, the hot pizzas were placed in hot holding table on serve line and cold back up pizzas on speed rack were wheeled into wic. This procedure needs to be changed immediately. All phf must be cold held at 41f or below at all times. Do not store cold pizzas at room temperature and place hot pizzas on same rack for unknown lengths of time before lunch starts. All back up pizzas must be kept in wic until they are ready to be cooked. Since pizzas have been out of refrigeration for less than an hour, corrected by rapidly chilling all pizzas to 41f at time of inspection. Pic will educate staff on proper cold holding requirements as needed. Violation corrected

Observed clear water leak coming from ceiling in employee restroom. To correct, please repair as soon as possible

Automatic faucet at handsink in employee restroom only runs for 3-4 seconds before shutting off. All automatic faucets must run for at least 15 seconds without the need for reactivation. To correct, please repair faucet as soon as possible

(Critical) Observed pan of sliced rte melons on table at end of serve line with an internal food temperature of 53-55f. Melons are not under any kind of temperature control. Sliced melons are considered potentially hazardous and must be kept at 41f or below at all times. Pic indicated that lunch lasts from 11am-1pm so they would like to use time control with the sliced melons. If wanting to utilize time instead of temperature control, will need to develop a time control policy to indicate how phf will be monitored and discarded as required. Please develop plan and submit to our office for review as soon as possible. Pic corrected violation by removing melons from table and placing in cold table at time of inspection. Critical corrected.

Observed boxes of rte fruits and vegetables stored on floor in wic. All food must be protected from sources of contamination and must be stored a minimum of 6 inches off of floor at all times
