(Critical) Observed all the potentially hazardous foods (meats, chicken salad, sliced tomatoes, etc. ) in the prep refrigerator to be at 49 to 57f. Corrected by discarding these foods

(Critical) Observed the floor sink basin to only be resting on the drain pipe that goes below the floor. As water drains into this wastewater pipe some leaks on to the floor. Maintain this drain pipe in good repair to prevent wastewater from leaking onto the floor

(Critical) Observed the following foods to have a last day of use of 6-12: chicken salad, tuna salad and deli turkey. Corrected by discarding

(Critical) Observed small flies in the back of the facility (dish washing area, office, and bathroom areas). Provide vinegar traps, fly paper, etc. To help trap adult flies.

1) the door gaskets on the two door low-boy salad cooler to be torn and collecting food crumbs. Replace these door gaskets to provide a smooth and cleanable surface. 2) the pizza oven exhaust hood filter sections were not installed in the exhaust hood. They were in storage above the walk in cooler. Corrected by installing the four filter sections in the hood. 3) the sub and salad coolers were not operating at 41f or below. Corrected by adjusting the sub cooler to a colder setting and by defrosting the salad cooler

A wet mop head was observed resting on the bare concrete floor in the back storage room. Hang up wet mops to facilitate faster drying

Dish washing area: the floor drain, the three compartment sink drain basin and pipes underground are in need of cleaning at this time to eliminate the old food build-up. Maintain these areas clean to prevent fly breeding areas

Observed two wooden utensils made from wide grained, soft wood. Corrected by discarding these utensils

The corners of the dish washing sink basins have old food debris accumulated in them. Thoroughly scrub these surfaces on a daily basis

Observed parchment paper still lining the baking trays on the speed rack in the storage area. This paper is only to be used one time. Corrected by discarding this used parchment paper

1) observed several packages of bulk pizza boxes (un folded) being stored on the floor of the kitchen. Provide a shelf/rack with at least 6" legs to store these items off of the floor to protect them from contamination. 2) two stacks of aluminum foil catering pans (some full and some 1/2 pans) stored upright in the equipment storage area. Either store these items inverted or in a sealed bag/container for protection of the food contact surfaces

The drip pan used for catching water draining from the wet mop head is overdue for cleaning. Clean this pan regularly to prevent this kind of build-up

Observed food containers (tubs and lids, etc. ) stored in seven cardboard boxes in the equipment storage area and one cardboard box being used to store spice containers in the kitchen. Discontinue using these boxes as they are not cleanable, can absorb moisture and can harbor pests

Observed an employee's coat, scarf, etc. Stored on a back stainless steel table. Ensure staff are using designated locations such as the office (or coat hooks/hangars in areas away from food or clean equipment)

No mop sink is provided at this facility. Install a mop sink for the disposal of mop water

Observed an unused microwave oven and a tv stored on the bottom of a wire rack shelving unit used for dry food storage. Remove these unnecessary items from the facility

(Critical) Observed chlorox clean-up disinfectant being used (undiluted) in a spray bottle to sanitize the electric food slicer. This solution, when tested bleached out the chlorine test papers. This product does not have label directions to sanitize food contact surfaces. Corrected by dumping out this solution and rinsing out the sprayer. Chlorine sanitizer at 75 ppm was mixed and provided in the sprayer. Ensure staff read and use chemicals per label directions

(Critical) Observed quartered tomatoes in the top of the two door reach-in salad cooler to be at 45 to 47f. Corrected by discarding the tomatoes

(Critical) Observed three plastic dough proofing trays, a 1/3 pan, 4-5 plastic food tubs for steak, and the vegetable dicer to be unclean. Corrected by re-washing these items to remove food debris and oil residues

(Critical) Observed two containers of raw beef (philly steak) stored in the top of the sandwich prep cooler. Corrected by relocating these containers to the bottom of the cooler.

Install a mop/service sink for the disposal of mop water

Observed a couple employee sweaters stored hung on the corner post and lying on a shelf of a shelving unit used to store foods and clean equipment. Ensure staff only hang their garments in a designated location away from food and clean equipment/single service items

Two door salad cooler: ice was observed on the interior back wall of the cooler. Defrost and adjust this unit as needed to maintain phfs at 41f and below. The owner noticed that employees had placed pizza delivery bags against the air vents on the back side of this unit--limiting air flow to the compressor

Observed several cardboard boxes being used to store clean equipment in on the storage shelves. Eliminate these boxes and replace them with a multi-use, smooth, easily cleanable container that doesn't absorb moisture

Observed a couple open employee beverage containers stored on the food/equipment storage shelf, and inside the one door coke cooler. Additionally, employee foods were observed on the top and middle shelves of the same coke cooler above customer food. Re-arrange these foods so all employee foods are stored on the bottom of the cooler

1) again observed mold growth inside the old two door upright cooler that is being used for dough proofing. Thoroughly clean and sanitize the interior surfaces of this unit and maintain clean in the future. 2) observed some dry crumbs that have accumulated on the bottom of the containers for the plastic eating utensils. Regularly clean out these containers. Consider keeping a cover on these containers to prevent debris accumulation

Observed only a damaged air thermometer in the two door salad cooler. Corrected by providing a replacement in this unit

Speed rack in the storage room: observed previously used parchment paper still on the sheet trays for baking the bread. This paper is to be discarded after each use.

Observed some plastic spoons and forks stored jumbled in their storage containers. Please organize these utensils so only their handles are extended to prevent the contamination of the eating ends when dispensing them

Observed the chlorine test papers to be discolored. These test papers have a color change when run under tap water. Provide a replacement test paper kit to obtain accurate sanitizer readings

Observed excessive mold growth on the interior surfaces of the traulsen two door upright unit that is being used as a dough proofer. Thoroughly clean and sanitize all interior surfaces of this unit

Observed a damaged food thermometer in the low-boy two door salad cooler. Provide an accurate replacement thermometer so temperatures can be monitored

Observed a few metal pans in storage that were stacked while still wet. Allow sufficient time for equipment to fully air dry prior to stacking them in storage

Observed several single service and multi-use trays, pans, containers in the storage room in an upright position. Ensure these items are stored inverted to prevent debris accumulation

Observed the wiping cloth sanitizer pail to be lacking any residual chlorine. Corrected by providing a replacement solution at 150 ppm chlorine

Observed large food bags (used for sub buns) being saved for re-use. Discard these bags after each use

Only observed a quaternary ammonia sanitizer test kit available in the facility. Chlorine specific test papers are needed for your facility since you use a chlorine sanitizer

Observed the plastic storage tubs for the portion cups of salad dressing in the two door low-boy cooler to be in need of cleaning at this time

Observed the coil on the three door low-boy freezer to be iced up. Many foods in the freezer were unfrozen, but were below 41f. Remove these foods, unplug the freezer and leave the doors open until the unit can completely thaw prior to plugging it back in to chill

There is still an uncapped inlet port on the back area hand washing sink drain pipe above the trap. Please plug this inlet port to prevent waste water from leaking onto the floor

(Critical) Observed multiple raw animal foods (bacon, beef pork sausage samples) stored above and with rte foods. Corrected by relocating these raw animal foods to the bottom of the cooler

Observed an uncapped port on the back area hand washing sink drain line. Please cap this line to prevent waste water from leaking on to the floor

Observed several white plastic food tubs in storage with a sticky, oily residue on the out sides of them. Please thoroughly clean these containers

Observed a wet wiping cloth stored in a weak solution of sanitizer. Corrected by providing a fresh solution at 100 ppm
