Found plastic squeeze bottles of sauces in the hot holding well in the back kitchen table. To correct either provide written documentation that plastic squeeze bottles can be placed in a hot hold well or use commercial food grade utensils to hot hold sauces that are designed to be used in a hot hold well.

Found facility without a manager certified in food safety. To correct provide a certified manager.

Found employee personal food in the 2 door prep top cooler. To correct designate employee storage area in cooler and place employee food in designated area only.

Found mop left in the bottom of the mop sink. To correct hang mop after cleaning, allowing it to properly air dry.

Observed employee working in the cooking and prep area not wearing hat. To correct provide hat/hair restraint and remind employees to wear them.

Found case of lettuce stored on the floor of the walk in cooler. To correct store lettuce on shelf at least 6 inches off the floor.

Found the hood filters in the pizza hood soiled with dusty build up. To correct clean hood filters and keep clean.

Found a paint tray with roller stored on the bottom shelf of the clean utensil storage shelf in the dishwashing area. To correct store maintenance tools away from food or food utensils.

Found the floor under equipment, edges and corners in the kitchen soiled with accumulation of debris. To correct thoroughly clean floor and clean on a more frequent schedule.

Found an opened bottle of water on the prep table in the back kitchen area. To correct discontinue drinking from water bottles in the food prep area, and use cups with lids and straws or beverage containers that prevent contamination of hands.

Found whole fruits and vegetables stored in milk crates in the walk in cooler. To correct replace milk crates with storage boxes that are food grade, durable, and cleanable. Found orange "home depot" 5 gallon plastic containers used to transport ice. To correct either provide written documents that buckets are food grade or replace with food grade buckets.

Found single use utensils, portion cups lids stored on a tray under and around the soap dispenser on the wall. To correct relocate lids to a more protected location.

Found 1 of the lights in the pizza hood not working. To correct repair/replace light.

Found stacks of "clean" wiping towels (which are used to clean and maintain equipment) and a "clean" dust mop placed on top of pizza oven to dry. To correct discontinue using top of pizza oven to dry towels and duster, as top of pizza oven should not be used to dry towels and duster and could create situations of contamination.

Please get hose reel for hose laying on floor rear kitchen near food prep sink and attach drain line for water softener to the wall with brackets; currently drain line on floor running along wall. Floors are not getting cleaned in these areas

Some grout missing between floor tiles especially in dishwash area. Please re-grout to prevent standing water between the floor tiles and to provide smooth, easily cleanable surface. Until grout work can be completed please use a floor fan to dry the floors to prevent any standing water

Once opened, prepared or removed from the freezer these food items shall be clearly marked. Add 6 days to open date, prep date or date removed from feezer. Corrected at time of inspection by discarding some food items and properly labeling other food items if date opened or date removed from freezer known by pic. Some food items that were not date marked included cottage cheese, macaroni salad, chicken wings, deli meats and meat pizza toppings. We worked on a list of food items requiring date marking at time of inspection. Please check daily to assure these food items are clearly marked and discard as needed. Add 6 days to open date, prep date or date removed from freezer. There were two different systems being used; please come up with one date marking system so all staff are clear

Observed bucket for ice placed onto the floor, should store the bucket atleast 6 inches above the floor for proper storage. Proper storage of utensils/ice transfer bucket will help prevent potential contamination as the bottom of the bucket may transfer soil from the floor onto other surfaces it is placed onto. ,

Observed the dishwashing area of the kitchen with an empty dispenser for sanitizer concentration test strips for both quat and choline based sanitizers used at this facility. To correct supply sanitizer concentration test strips for ready access at the dishwashing area of the kitchen to encourage use by employees. ,
