Health Report - 04/27/2015 (3 critical incident(s))

(Critical) Observed the following: 1. Au graten potato's that were cooked the night before at 53f and still in the cooling process, located in the drawer cooler on the cook line, in a tightly sealed container. 2. Lobster bisque soup that was cooked the night before at 52f and still in the cooling process, located in the top loading cooler on the cook line closest to the dish line, in a tightly sealed container. Cooling shall be accomplished in accordance with the time and temperature by using one or more of the following methods based on the type of food being cooled: (1) placing the food in shallow pans; pf (2) separating the food into smaller or thinner portions; pf (3) using rapid cooling equipment ; pf (4) stirring the food in a container placed in an ice water bath; pf (5) using containers that facilitate heat transfer; pf (6) adding ice as an ingredient; pf or (7) other effective methods. Pf to correct: discard the noted food and start cooling the noted food with one of the above, approved methods. Corrected: person in charge discarded the noted food into the garbage and stated that more ice wands would be purchased and they would be used in combanation with an ice bath to cool the food in the proper time frames.

(Critical) Observed the following: 1. (Critical) Observed the following potential cross contamination: 1. Butter used for the bread was stored with and below raw sea food, located in basement walk-in cooler. 2. French onion soup stored below raw steaks, located in basement walk-in cooler. 3. Uncooked french fry's and a block of parmesan cheese stored under raw steaks located in the lockable steak cooler on the end of the cook line. Food shall be protected from cross contamination by: (1) except as specified in (1)(c) below, separating raw animal foods during storage, preparation, holding, and display from: (a) raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables,(p) and (b) cooked ready-to-eat food; (p) to correct: move the noted ready to eat foods up and away from all raw food product. Review with all staff on raw food storage. Corrected: person in charge moved all the ready to eat foods up and away from all raw food product

Observed the wall under the clean side of the dish line to be in poor repair. Physical facilities shall be maintained in good repair. To correct repair the noted wall

Observed the following plumbing in need of repair: 1. Mop sink in basement leaks from several locations on the faucet. 2. Two compartment prep sink in basement leaks from the middle when turned on. 3. Three compartment sink on the upstairs dish line leaks from the middle when turned on. 4. Grease trap is showing signs that it may be leaking located on the dish line. 5. Hand sink petal in basement is in poor repair and its very difficult to turn hot water on. A plumbing system shall be: (a) repaired according to law;(p) and (b) maintained in good repair. To correct have all the noted items repaired to eliminate the leaks and repair the noted hand sink.

Observed the following: 1. Ceiling vents over the dish area soiled with a dust build up. 2. Floor under equipment in the server dish area located across from the bar is soiled with a debris and standing water build up. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct have the noted areas cleaned and maintained clean

(Critical) Observed french onion soup, bisque soup and aug gratin potato's that were cooked the night before and still in the cooling process, located in the walk-in cooler, in tightly sealed containers. Cooked potentially hazardous food shall be cooled: (1) within 2 hours from 135f to 70f; p and (2) within a total of 6 hours from 135f to 41f or less. P discard the noted food and discontinue cooling food in sealed containers. Person in charge discarded the noted food and stated that they would cool the food in containers that are not sealed

(Critical) Observed both mop sink equipment with an atmospheric vacuum breaker with the chemical tower attached to the mop sink with it not venting water. A plumbing system shall be installed to preclude back flow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and back flow prevention is required by law, by: (a) providing an air gap as specified under # 5-202. 13 (p); or (b) installing an approved back flow prevention device to correct provide a vented t (also known as a side kick) to the noted sinks. ,

(Critical) Observed the following food holding above 41f for an unknown amount of time: a. Saut� cooler on cook line not holding cold food between 48-50f: 1. Cooked carrots 49f. 2. Whipped butter 48f. 3. Veal glaze 50f. 4. Siracha sour cream 50f. 5. Cooked baby bock 48f. B. Bar pasty cooler 50f for an unknown amount of time: note: cooler was working. 1. Peanut butter sauce 50f. 2. Kalua sauce 50f. Potentially hazardous food that is to be held cold shall be held at 41f or below. To correct discard all the noted food. Corrected: person in charge discarded all the noted food into the garbage

Observed the hand sink in the downstairs pastry/prep room with a broken foot petal. Note: sink turns on and works, but not easily. A plumbing system shall be: (a) repaired according to law;(p) and (b) maintained in good repair. To correct repair the noted foot petal.

Observed the wall behind the dish machine to have missing tiles, cove base and loose stainless steel wall covering. Physical facilities shall be maintained in good repair. To correct repair the noted items. ,

Observed the following equipment in poor repair: 1. Sautee cooler not holding cold food at 41f or below. Note: food was 45-50f. 2. Gaskets on the saut� cooler torn. 3. Drawer front plate falling off located on the bottom left. 4. Gasket on the downstairs pasty cooler torn. 5. Gasket on the lockable meat cooler, upstairs torn. Equipment shall be maintained in a state of repair and good condition. To correct: 1. Repair the noted cool to hold food at 41f or below. 2,3,4,5: repair or replace the noted items.

Observed the following not sealed to the wall: 1. Three compartment sink at the bar. 2. One compartment prep sink on the cook line nearest to the dish washing machine. 3. One compartment sink across from the lockable meat cooler on the cook line. 4. Wait staff station as you enter the kitchen from the dinning room. 5. Two compartment prep sink in basement prep room. 6. Hand sink is basement prep room. 7. Seal all parts of the hood to the wall. Note: use heat sensitive silicon. Equipment that is fixed because it is not easily movable shall be installed so that it is: (1) spaced to allow access for cleaning along the sides, behind, and above the equipment; (2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch; or (3) sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage. To correct: seal the noted items to the wall. ,

Observed the following areas in need of cleaning: 1. Floor wall juncture along the dish washing machine area, especially in the corner. 2. Floor under the unmovable cook line has a black build up. 3. Floor wall juncture along the bar, including around the bar dish machine. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct clean the noted area and clean at a frequency that maintains them free of build up

Observed flies in the basement kitchen area. To correct keep areas clean and dry, in good repair, and continue fly control measures.

Found several spray nozzles in the dishwashing machine clogged with debris. To correct clean spray nozzles and clean when needed.

Found the drain in the basement employee restroom handsink draining slowly. To correct clean/repair drain to drain properly.

Found no handwashing sign in the bar handsink area. To correct provide sign reminding employees to wash hands.

Found several bags of folded towels used for drying hands stored on the floor of the basement hallway. To correct store towels on shelf/table.

Found a utensil container full of silverware stored next to the handsink in the kitchen waitstation. To correct store utensils in a more protected location or provide a shield on side of handsink.

Found the spray nozzles of the dishwashing machine clogged with debris. To correct follow manufacturer's directions and clean spray nozzles.

Found a back pack stored on top of unfolded dessert boxes. To correct store employee personal items in designated areas.

Found a few cutting board with deep grooves and scratches. To correct either resurface cutting boards or replace.

Found the ceiling in the entire kitchen soiled with dusty build up. To correct carefully clean ceiling and keep clean.

Pic corrected by providing proper datemark, and has been reminded to monitor and ensure staff are datemarking appropriate items. ,

(Critical) Found ready to eat, time/temperature control for safety (tcs) food, cream garlic dressing and whipped herb butter passed it's 7 day discard datemark in the basement walk in cooler. Pic corrected by discarding dressing and butter and has been instructed to monitor and ensure items passed it's discard datemark are discarded. This violation has repeated 3 of 5 inspections, a risk control plan and administrative conference notice has been issued. ,

Found cases of single use utensils stored on the floor of the basement storage room. To correct store cases of utensils on shelves at least 6 inches off the floor.

Found employees clothing stored on top of utensils in the storage room. To correct store employee clothing in designated areas only.

Found water dripping from the sewer lines and ducts in the ceiling of the storage room. To correct locate source of leak and repair as necessary to eliminate leaks/drips.

Found the door gaskets of coolers on the cooking line cracked. Found the guages of the dishwashing machine not functioning properly. To correct repair/replace door gaskets and guages of the dishwashing machine.

Found stacked containers stored wet on the shelf under the prep table. To correct allow containers to air dry prior to stacking.

Found the ice cream dipper well faucet not turned on with in use utensils stored in ice cream dipper well. Pic corrected by turning on ice cream dipper well. Found ice scoop stored on top of the ice machine. To correct store ice scoop in a holder or clean container.

Found employee personal clothing stored in an unorganized manner, on shelf post, and boxes in basement storage room. To correct store employee personal clothing in a more orderly manner.

Found the left fan in the 1 door reach in cooler on the cooking taped and held with duct tape. To correct repair/replace fan.

Found the wall under the dishwashing machine counter with missing tile and uneven surfaces. To correct repair/replace wall to be durable, smooth, and cleanable.

Found no handwashing sign in the kitchen waitstation handsink. To correct provide a sign reminding employees to wash their hands

Found a utensil bin of clean utensils stored next to the handsink in the kitchen waitstation. To correct either install a splash guard or relocate utensils to a more protected location.

Found the tiled floor with worn grout in basement prep area by the mop sink. Found the tiled wall under the dishwashing counter damaged with missing tile. To correct properly repair/replace noted areas and keep facility in good repair.

Found rte, tcs food, cheese cake passed it's 7 day discard datemark in the bar 2 door dessert prep top cooler. Pic corrected by discarding salad dressing and cheesecake, and has been instructed to monitor that employees are checking dates and discarding items passed it's 7 day discard datemark. This is a repeated critical violation, and a risk control plan form for discarding rte food passed it's 7 day discard datemark has been issued. ,

Found employee personal food and drink in various coolers on the cooking line. To correct designate an area in cooler where employee may store their personal food, and drink and ensure employees store their personal food in designated area only.

Note: this is not a repeated cold violation, and no rcp or ac issued at this time, as cold hold violations were in different situations at this time. ,

(Critical) Found the water supply line of the espresso machine without backflow prevention device. To correct either show that water supply line of espresso machine has backflow prevention or provide a back flow preventer with intermediate atmospheric vent that meets asse 1012 or 1022. ,

Found the toilet tank cover in the basement employee restroom missing. To correct replace toilet tank cover.

Found the shared dumpster/compactor area soiled with garbage and debris from this facility. To correct clean area of garbage and debris, and work with neighbors to keep areas clean.

Found the handle of the ice scoop laying on top of ice in the bar ice bin. Pic corrected by having employee store ice scoop on counter top.

Found no handwashing sign in the basement employee restroom. To correct provide sign reminding employees to wash their hands at all locations where employees may wash their hands.

Found no paper towels in the basement employee restroom paper towel dispenser. Pic corrected by providing paper towels.

Found the top portion of the dessert 2 door prep top cooler not holding tcs food at 41°F. To correct monitor and verify that tcs food is held at 41°F, and repair/replace cooler if necessary. Found hood filters above the grill broken and missing parts. To correct repair/replace hood filters. Found some food containers in the kitchen prep area warped and cracked. To correct replace damaged food containers.

Found 2 bags of rolled cotton hand towels (used to dry hands) stored on the basement hallway floor. To correct store towels on shelf at least 6 inches above the floor. Found an opened roll of plastic wrap stored on the floor under the bar counter top by the dessert cooler. To correct store plastic wrap in a more protected manner on shelf above the floor.

Found employee personal items (shoes and clothing) stored on top of cases of food utensils. To correct designate where employees may store their personal items, and remind employees to store their personal items in designated areas only.

Found the pop nozzle of the pop gun in the center of the bar soiled with light accumulation. To correct follow manufacturers directions and clean pop nozzle.
