Time and temperature requirements were not met, based on staff information. Pic corrected by discarding soups and have been instructed to monitor that soup is not covered, divided into smaller portions and cooled from 135 to 70°F in 2 hours and from 70 to 41°F within 4 hours. This violation has repeated and a risk control plan is required. ,

Found tcs food, liquid eggs 50°F, rice 47°F, potatoes 47°F, and corned beef hash 51 in the 2 door worktop cooler. Found rte, tcs food, portion cups of ranch dressing without 7 day datemark in the 4 door sectional reach in cooler. Pic corrected by providing 7 day datemark on noted items, and has been instructed to monitor and ensure staff are datemarking appropriate items. ,

Found the kitchen and waitstation floor under equipment, edges and corners soiled with accumulation of debris. To correct thoroughly clean floor and clean on a more frequent schedule. Found the ceiling air vents in the kitchen soiled with dusty build up. To correct clean ceiling vents and keep clean.

Found several containers of ice cream without covers in waitstation ice cream freezer. To correct cover ice cream when not in use.

Found the drawer/door prep top cooler and the 2 door worktop cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower.

Found light in the back kitchen cooking hood not working. To correct repair/replace light.

Observed water dripping from the back flow prevention device on the coffee marker water supply line. To correct repair/replace back flow prevention device.

(Critical) On follow up inspection found no soup in the process of cooling, and soup made previous days at 38°F in the walk in cooler. Staff states they are using ice baths and ice wands to cool soup. Reviewed with staff proper cooling of tcs food, such as soup and reminded staff that soup must be cooled from 135°F to 70°F in 2 hours and from 70°F to 41°F in 4 hours. Left cooling chart for staff to review and fill out. This violation was corrected on routine inspection, and reverified on follow up.

Observed employee working in the dishwashing area not wearing hat/hair restraint. To correct provide hat/hair restraint and remind employees to wear them.

Found salad bowls made of a wood/bamboo like material on the counter top by salad prep area. To correct replace salad bowls with commercial grade bowls that are smooth, durable and cleanable.

Found light accumulation on the interior front shield of the ice machine. To correct follow manufacturer's directions and clean ice machine on a more frequent schedule.

Employee corrected by dividing gravy into several smaller metal pans and containers, and then place in ice bath and walk in freezer for rapid cooling. Pic has been instructed to monitor and ensure employees are properly cooling tcs food, that is to divide tcs food into smaller portions in metal pans/containers and to rapidly cool tcs food from 135°F to 70°F in 2 hours and then from 70°F to 41°F in 4 hours.

(Critical) Observed an employee wearing single use gloves handle raw chicken then wash/rinse gloves off in handsink. Employee corrected by removing gloves, wash hands, and then placed on a new pair of single use gloves. Pic has been instructed to monitor and ensure gloves are used for a single task and not washed or rinsed when soiled.

(Critical) Found time/temperature control for safety (tcs) food, feta cheese 50°F in a bowl on top of container of feta cheese, hard boiled eggs 51°F in the top portion of an overloaded container, and raw shell eggs 77°F in the top portion of an overloaded container in the top portion of the door and drawer prep top cooler. Pic corrected by discarding tcs food above 41°F, and has been instructed to monitor and ensure tcs food is held at 41°F by not overloading containers or placing containers on top of other containers.

Found the floor under the cooking line and prep area soiled with accumulation and build up. Found the floor under the equipment and counter in the waitstation soiled with accumulation. To correct thoroughly clean floor and clean on a more frequent schedule.

Found the drainage area of the steam table leaking and a bucket catching drips. Found the beverage machine's backflow prevention device in the wait station leaking and a cup catching drips. To correct repair steam table and backflow prevention device to be free of leaks.

Found the light cover in the cooking hood and edge of the cooking hood soiled with grease. Found the interior of the microwave soiled with splash. . Found the door gaskets of coolers soiled with accumulation. To correct clean noted areas and clean on a more frequent schedule.

Found the water spout of the ice cream dipper well below the rim of the ice cream dipper well. Pic corrected by resecuring water spout to clip and providing adequate air gap. Pic has been instructed to monitor and ensure water spout air gap is maintained at all times.

Found no paper towels in the waitstation paper towel dispenser. To correct provide paper towels.

Observed waitstaff handling utensils (forks, knives, and spoons in the utensil rack) on the food and lip contact surfaces. To correct utensils should be touched/handled on the handles, that is have dishwasher with clean gloves place utensils in utensil rack with handles up after final wash.

Found a dial faced thermometer left in food in the door and drawer prep top cooler not accurate. To correct either calibrate thermometer or replace thermometer.

Found the door and drawer gaskets of coolers with cracks. To correct repair/replace door gaskets.

Found no handwashing sign in the mens public restroom. To correct provide sign reminding employees to wash their hands.

Found the ground surrounding the dumpster soiled with debris. To correct work with neighbors and keep dumpster area clean.

Found a stack of coffee filters stored unprotected on top of the coffee urn. To correct store coffee filters in a more protected manner such as in a covered storage box.

Found rte, tcs foods in the walk in cooler with 8 day discard datemarks. Pic corrected by determining dates products were opened/prepared and had employees provide 7 day (day opened/prepared + 6 days) discard datemarks for all rte foods in coolers. Pic has been instructed to re-train employees on proper datemarking, and to ensure items are properly datemarked.

Found the employee restroom door propped open and door closer not attached to door frame. To correct re-attach door closer to door frame and keep door closed.
