(Critical) Found time/temperature control for safety (tcs) food, sliced tomatoes 55°F in the front counter cold hold well. Pic corrected by discarding tomatoes and has been instructed to cool tomatoes to 41°F prior to placing in cold hold well, as manager states tomatoes were cut this morning. Found tcs food, kraut from refrigerated bag 77°F, red and green peppers slices from jar 77°F, and canned mushrooms 77°F in the top portion of the 2 drawer prep top cooler (no food in bottom drawers). Pic corrected by discarding tcs food, and has been instructed not to use this cooler, until repaired/replaced to hold tcs food at 41°F.
(Critical) Found a y type shutoff valve/splitter installed downstream of a mopsink faucet with atmospheric vacuum breaker, with water left on allowing back pressure of system pressure. To correct install a t-valve/sidekick on faucet, install y type shuttoff valve/splitter on t-valve/sidekick, connect chemical water supply on y type shutoff, and connect water hose on the other side of y type shutoff. Note: water must be turn on when in use, and shut off when not in use.
Found the 2 drawer prep top cooler and cold hold well not holding tcs food at 41°F. To correct repair cooler and cold hold well to hold tcs food at 41°F and lower.
(Critical) On follow up inspection used a quat test kit and found the sanitizing solution flowing from quat dispenser at about 200 ppm, between manufacturer's recommended range of 150 to 400 ppm. Pic states quat dispenser was repaired. Violation corrected.

Pic temporarily corrected by manually mixing chlorine (bleach) sanitzing solutions, in the 3 compartment sink and sanitizing buckets and used chlorine test kit to verify proper concentration. To correct call for repair and have quat dispenser repaired to dispense quat at manufacturer's recommended range of 150 to 400 ppm. Employee states they do use test kit to verify quat sanitizer. ,
Found wrapped rolls of sandwich wrap stored on the floor under shelf. To correct store sandwich wrap on shelf at least 6 inches off the floor.
Found facility had no manager certified in food safety. To correct provide a full time manager certified in food safety.
Found the interior surfaces and interior door panel of bread oven soiled with accumulation. Found the interior of the microwave soiled with accumulation. To correct thoroughly clean oven and clean on a more frequent schedule.
