(Critical) Several flies observed at bar. The presence of insects, rodents and other pests shall be controlled to minimize their presence. Not corrected. Please contact extermination company. Provide tight-fitting covers for open liquor bottles. At time of inspection discarded some liquor bottles with several flies in them. ,

(Critical) Observed food employee contacting ready-to-eat food with their bare hands. Critical corrected: at time of inspection interrupted food employee to wash hands and put on gloves when dispensing washed lettuce into individual bowls. You may also use a utensil to dispense lettuce into bowls

(Critical) Some refrigerated, ready-to-eat, potentially hazardous food items found without date marking. These food items shall be clearly marked once opened, prepared or removed from freezer. Critical corrected: at time of inspection clearly marked cut tomatoes and cooked noodles; discarded opened whipped cream and opened ice cream mix at bar.

(1) three door reach-in-cooler leaking at fan housing. Please repair and maintain. (2) garbage disposal leaking. Please repair and maintain in good repair.

Provide iodine test kit for bar sanitizer compartment and monitor iodine concentration. Iodine concentration at proper level at time of inspection

Provide light shield with end caps for light bulb above 4-compartment sink at bar (left side when facing sinks)

Please store single-service utensils and multi-use utensils with handles facing the employee to prevent potential contamination of the food-contact end

No paper towels available at handwash sink end of cook line; no paper towels available at bar handwash sink and paper towels do not fit in paper towel dispenser in small room with 3 door ric. Please provide paper towels at all times at all handwash sinks

Please store wet wiping cloths in wiping cloth solution at proper sanitizer concentration

Please continue to sort through and get rid of any items not used in the operation and maintenance of facility. (Critical) Time as a public health control policy for sush rice not being followed. Critical corrected: at time of inspection preparation time and discard time added to sticker on side of sushi rice container. Facility makes small amount of sushi rice for lunch; discards end of lunch (time less than 4 hours); and same process for dinner. Facility closes between lunch and dinner. Facility used to use chart; will begin marking side of sushi rice container

(Critical) Pic not familiar with the employee health reporting requirements. The permit holder shall require food employees to report to the pic information about their health and activities as they relate to diseases that are transmissible through food. Critical corrected: at time of inspection discussed employee health reporting requirements including reportable diseases, reportable symptons and responsibilities of the pic and the employees

(Critical) Mop sink chemicals stored in close proximity to new hot holding table. Critical corrected: at time of inspection relocated chemicals below any food or food contact surfaces. Pic will relocate chemical rack near mop sink and provide a permanent barrier between the mop sink and the hot holding table.

(1) large chest freezer in basement in poor repair including top interior lid and gasket. In addition their is excess frost build-up in this unit partially due to gasket. Please replace to provide smooth, easily cleanable surface and a good seal. (2) please provide 2 additional sink stoppers for 3-compartment sink in dishwash area.

No paper towels available at (1) bar handwash sink and (2) employee restroom handwash sink and (3) paper towel dispenser not dispensing properly at dishwash area handwash sink. Please provide drying device at all handwash sinks at all times

Please provide smooth, easily cleanable stand or table for storage of sushi rice to replace overturned crates currently in use

Please clean now and as often as necessary to maintain in clean condition

Floors at bar and near mop sink in poor repair. Please repair to provide a smooth, easily cleanable surface in these areas. This violation has been cited on previous inspections. The floor repairs were to be completed by this routine inspection. A follow-up inspection will be required within 60 days for the floor repairs.

(Critical) Observed dishwasher not washing hands between contacting soiled dishes and utensils and clean dishes and utensils. Critical corrected: at time of inspection interrupted dishwasher to wash hands before contacting clean dishes and utensils

(Critical) Salad dressing and sliced tofu at salad station found at improper temperature. Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Critical corrected: at time of inspection discarded sliced tofu found at 66°F and salad dressing found at 51°F. In future these will need to be in refrigeration unit to maintain at 41°F or below or if you will be using ice through the lunch and dinner serve periods the ice level must be above the food level and all of the food must maintain at 41°F or below

(Critical) Observed open cup in rear kitchen. Food employees shall drink from closed beverage containers. Critical corrected: at time of inspection removed open cup. Discussed with pic use of closed beverage containers, sipper cups or cups with lids and straw
