Observed a large plastic tray with 3 bags of packaged pasta on the floor of the walk in cooler. Food shall be stored at least 6" off of the floor not exposed to dust or splash. Corrected by person in charge moving tray containing the packaged pasta to a shelf in the walk in cooler that is more than 6" off the floor. Corrected

Observed several mops in the mop closet area to be on the floor or in the mop bucket. When not in use, mops shall be hung to dry to preclude the growth of bacteria. To correct, please hang mops on the wall mounts provided in the mop closet

Observed the wiping cloth sanitizing solution for the wipe down rags to be very weak. The proper concentration of sanitizing solution is required to preclude the growth of bacteria on the wipe down rags. Corrected by person in charge refilling the wiping cloth solution buckets with a fresh batch of quat sanitizing solution that tested at 200 ppm. Corrected

Observed the waste receptacle in the woman's restroom to not have a cover. It is required to have a cover for the waste receptacle in woman's or unisex bathrooms. To correct, please provide a cover for this waste receptacle

Observed the drain lines for the rinse and sanitization basin at the 3 compartment sink to no longer be maintaining a proper air gap. These two basins shall maintain an air gap that is 1. 5x the diameter of the drain line. To correct, please trim/file/maintain the drain lines so the proper air gap is achieved

(Critical) Cleaning chemicals observed next to and above pizza boxes, pizza racks, pans, "to go" boxes, and food contact surfaces. In the dry storage area and food prep area. In addition, cleaners and spray bottles were observed stacked on top of boxes of single service items. Correct by storing chemicals, cleaners, toxics away from food prep areas, food contact surfaces and single service articles in a separate designated location to minimize for potential for contamination of these items in those areas. ,

(Critical) Employee observed to be eating and going back to do food service with handwashing. Handwashing is required prior to going back to work prior to taking a break/eating food. Corrected by employee told to wash hands by inspector after being told to do so. ,

(Critical) Thermometer for the pizza prep unit was missing. Thermometers are required to monitor the temperatures of the units. Provide a thermometer for this unit in order to monitor the temperature of the unit. A metal stem product thermomter was found sitting in the red peppers container laying on the food. Keep the thermometer with the dial end up to minimize the contact with the food when retrieving the thermometer. ,

(Critical) Assorted potentially hazardous foods including chicken, pepporoni, bacon and sausage, found at 43-47°F in the pizza prep unit. Potentially hazardous food must be stored at 41°F or below to minimize the potential for the growth of bacteria. Corrected by consuming or discarding potentially hazardous foods out of temperature control within 4 hours of removing foods from the walk in cooler and placing into the prep unit. , ,

(Critical) Tubs used for dough prep with remaing debris of dough were observed to be reused for bagged cheese storage without wash/rinse/sanitize. Once food contact surfaces are dirty, you must use a 3 step process for cleaning and sanitizing prior to reusing the container for storage. Correct by retraining the employees as indicated above. ,

The trash receptacles for both the mens and womans restroom is missing. Provide trash receptacles for both restrooms. The womans restroom receptacles must be covered to provide for proper disposal of tampax. ,

Pizza oven conveyor was found to be broken with jagged/sharp edges and is no longer cleanable. Repair or replace. , , ,

25 cartons of mozzarella cheese found stacked on dough tubs awaiting prep and thawing at room temperature. Cheese was delivered in the am (frozen) and had thawed out to the temperature of 26°F. Thaw cheese by placing in cooler in advance with enough time to thaw properly. Arrange delivery date and time to allow for adequate thawing in the walk in cooler. ,

The hood filters and seals to the reach in cooler doors under prep table are in need of cleaning, clean now and place on a routine cleaning schedule. ,

The following non food contact surfaces need cleaning. The pizza oven ledges. Clean now and place on a more frequent cleaning schedule to preclude accumulation build up.

The following non food contact surfaces were broken, the outside edges of food containers for pizza toppings and the catch trays for the pizza prep unit. These surfaces are no longer smooth and cleanable. Correct by replacing all broken non food contact surfaces/equipment.

(Critical) Observed chemical spray bottles stored on a chrome wire rack shelving unit in the back prep station stored with food and to-go containers. Corrected by relocating these spray bottles to the bottom shelf of the prep counter located adjacent to the wire rack shelving unit

(Critical) Spinach with a last day of use of 5-17°Found in the bottom of the pizza prep cooler. Corrected by discarding this spinach. Ensure staff are purging expired food on a daily basis (end of work day or beginning of next work day)

The men's restroom is still lacking a door closer. Please install a door closer to maintain this door closed

The back delivery door is still lacking a door closer. Provide a door closer so this door closes automatically

Observed the wall to be unclean behind/below the utensil (dough docker, oil brush, counter brush) storage rack at the dough station. Thoroughly clean this area and maintain on a regular basis

The exhaust hood and filters above the pizza oven are in need of cleaning at this time to eliminate oils and dust

(Critical) Observed a chemical spray bottle labeled sanitizer to be filled with a blue glass cleaner chemical. Corrected by draining out the chemical and flushing the bottle and sprayer

In four locations single service items (paper plates, aluminum foil pans and bowls) were observed to be stored upright. Maintain these items inverted or covered for protection of the food contact surfaces

Ceiling tiles are out of position in two locations: the dead end hallway behind the pizza make line and just inside the back door. Re-install these ceiling tiles to seal off the attic area

The back door still does not have a door closer. The owner indicated they tried to repair it themselves, but with no success. A contractor will need to be hired to repair this door

The sanitizer for wiping cloths at the cut table was too weak. Corrected by providing replacement sanitizer
