(Critical) Found 1 severely dented can and a few dented cans on the ready to use shelf. Pic corrected by placing dented cans on cart for return to supplier for credit. Pic has been instructed to monitor and ensure dented cans are not used and sent back for credit.

Found hood filters soiled with greasy build up. Found the interior of microwave soiled with accumulation. To correct clean hood filters and microwave and clean on a more frequent schedule.

Found light in cooking hood not working. To correct repair/replace light.

Found numerous soiled utensils in all compartments of the 3 compartment sink and clean side drainboard. To correct provide additional cart/table to hold soiled utensils as soiled utensils should not be in all compartments of the 3 compartment sink and clean side drainboard.

Found numerous personal food items (raw chicken, 6 half gallons of yogurt) in the walk in cooler shelf, and various other personal food (bread and chicken) on prep table. To correct limit personal food in school facility, and store personal food items away from lunch program supplies.

Observed the lids on dumpsters behind school left open. To correct close dumpster lids after each use.

Found the floor under equipment, edges and corners in the cooking and prep area soiled. To correct thoroughly clean floor and keep clean.

(Critical) Found several cartons of raw shell eggs stored on shelf above and next to ready to eat foods. Employee corrected by moving raw shell eggs to the bottom shelf below and away from ready to eat food.

(Critical) Found a large container of fruits containing apples on the self service table without sneeze guards or protection. Employee corrected by placing container of fruits on the service line where employees placed fruit on student trays.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, several plastic containers of chicken salad made the previous day without 7 day (day made + 6 days) datemark. Employee corrected by providing datemark. Pic has been instructed to monitor and ensure rte, tcs food lasting more than 24 hours is datemarked.

Found employee personal food stored with food through facility. To correct designate employee storage areas, and store personal food in designated areas only.

Found the floor under equipment, edges and corners in the cooking and prep areas, storage room and dishwashing area soiled with accumulation and build up. To correct thoroughly clean floors and clean on a more frequent schedule.

Found 2 of 3 lights in the cooking hood not working. To correct repair/replace lights.

Found no waste basket by handsinks. To correct provide waste baskets by handsinks for paper towel disposal.

Found fruits, apples and oranges in a case on the counter top on the service line, and asked employee if any has been done to fruits prior to service, employee replied they were told they did not have to be washed. Corrected by educating employee that fruits have to be washed, prior to serving. Employee corrected by rinsing fruits in a colander

Observed food improperly at two locations: 1. In the walk in cooler observed crates of milk stored on the floor. Correct by placing the milk crates on a dunnage rack or shelving. Alternatively, an additional cooler may be needed to properly store the volume of milk used by the facility. 2. In the dry storage area observed cases of single service items stored on inverted milk crates. Correct by placing cases single service items on dunnage racks or similar shelving. Food shall be stored off the floor to prevent contamination. Also, storage shelves shall have a minimum clearance of six inches above the floor to facilitate easy floor cleaning and inspection for pests

At the main cook line observed the hood filters with a heavy accumulation of dust and grease. Correct by cleaning now and on a more frequent basis to prevent the accumulation debris. This violation was observed during the last inspection

At the walk in freezer observed the door striking the threshold. The door is creating gouges in the threshold. The door is difficult to close. Correct by repairing door so it closes easily
