Staff stated that spray rinse arm was recently replaced with longer hose. (Critical) Observed 3 packages of commercially prepared rte guacamole in stagnant water in back kitchen area prep sink. Internal food temperatures of guacamole was observed at 68-70f. Pic stated that frozen guacamole was placed in stagnant water to thaw +/- 1. 5 hours ago and staff forgot to take it out of water. All phf must be cold held at 41f or below at all times. Thawing violation addressed later in report. Pic corrected by rapidly cooling guacamole at time of inspection and storing guacamole in prep cooler. Violation corrected

(Critical) Observed covered pan of cooked chicken breasts (whole and heaped in pan) in upright cooler at end of cooking line with internal food temperatures of 100-105f. Chicken breasts have been in cooler for less than 30 minutes per pic. All phf must be rapidly cooled from 135f to 70f within 2 hours and from 70f to 41f or below within a total of 6 hours. Need to utilize shallower pans and leave uncovered when actively cooling. Pic corrected by spreading chicken breasts out on a sheet pan and placing in wic at time of inspection. Pic to educate staff on proper cooling methods as needed. Violation corrected

(Critical) Observed one of the pop guns (near garnish container) and a pop gun holder (next to dishmachine) in bar with a heavy accumulation of mold and debris.
