(Critical) Numerous drainflies were observed in the employee restroom/mop sink area. Call exterminator to eliminate infestation. Correct as indicated above.

(Critical) A severely dented can of spanish olives was observed on the can storage shelf. Make sure all cans are inspected prior to stocking and place dented or damaged cans in a separate designated location away from the in use storage. Corrected by removing can from storage rack and placing on a separate designated shelf away from in use can storage.

(Critical) The following food contact surfaces were found to be soiled. The underside of the shake mixer and the vegetable dicer. Clean now and retrain employees on proper cleaning technique. Correct as indicated above.

The pizza oven rollers and ledges are in need of cleaning. Clean now and place on a more frequent cleaning schedule.

The faucet for the handsink in the pizza prep area is loose and has been previously caulked most likely due to leaking. Repair or replacethe condensate line in the walk in cooler is leaking and dripping into a bucket. Repair leak and remove bucket from walk in cooler.

The following non food contact surfaces need cleaning: the shelves in the prep storage area and the outside of the cans. Clean now and place on a mroe frequent cleaning schedule.

The large bucket for storing dirty pans found to be in poor repair and no longer cleanable. Replace witha new bucket that is smooth and cleanable.

The hood filters for the pizza oven are improperly sized leaving gaps in the filter system of the hood posing a potential for improper ventilation of the hood system. Provide proper sized filters for the pizza oven. Correct as indicated above.

(Critical) The slicer was found to have food debris build up. Clean now and place on a more frequent cleaning schedule. Provide proepr scrubbing tool to properly clean the slicer. Retrain employee on proper cleaning technique. The mixer/blender was found to be soiled with food debris on part of the rod and the underside. Clean now and plac eon amroe frequent cleaning schedule. Retrain employees on proper cleaning techniques. Assorted utensils and dishes found to be soiled/greasy on the clean storage rack. Clean now and place on a more frequent cleaning schedule

(Critical) Hot water at handsink near dishwashing room and in the employeee bathroom not reaching 100f. After 3-compartment sink was filled for manual dishwashing, there was inadequate hot water supplied to all handsinks upon further testing. The existing hot water heater is inadequate for its intended use. It is a 40 gallon whirpool water heater from 1999. Adequate hot water must be available during peak demand. Provide a new hot water heater with sufficient capacity to meet peak demand. Health department will review minimum size requirement prior to installation

(Critical) Mop water is being disposed of in the sanitizer compartment of the 3c sink. This is prohibited. Due to mop sink being broken the mop water is being disposed of improperly. Discontinue this practice immediately. Repair/replace mop sink for proper vessel for sewate disposal asap.

(Critical) Observed food employee coring raw tomatoes with bare hands. Whole tomatoes are ready to eat foods used on salads. Bare hand contact is prohibited with ready to eat foods. Use gloves or other suitable utensils to minimize potential for contamination. Corrected by food employee putting on gloves

(Critical) Dented cans found stored on the can storage shelf including artichoke hearts, olives and red peppers. Do not stock damaged cans on the can storage shelf. Provide a separate designated location away from can storage awaiting return to the supplier. Corrected by relocating these cans at the time of the inspection

(Critical) Found bimetallic probe thermometer at the prep area not calibrated. Foods need to be accurately checked for temperature. Calibrate thermometer now, and on a more frequent schedule. Corrected: thermometer replaced by one that is functional and calibrated

Pizza oven rollers and ledges and outside ledges found to have excess food debris build-up. Clean now and place on a more frequent cleaning schedule.

Drain line for hand sink in prep area is leaking onto floor. Connection from the sink basin to drain line is loose, and needs to be tightened. Corrected by tightening at time of inspection. If leaking continues, call for repair. Faucet for hand sink in employee restroom is leaking. Repair leak. Drain lines for both mop room and mid-prep area are plugged with standing water. Eliminate plug and repair so drain lines are functioning properly. Mop sink is in poor repair. Doesn't have a faucet. Replace mop sink for proper disposal of sewage

The following non-food contact surfaces need cleaning: outside of pizza oven, under the catch trays, hood canopy, attached piping, fan covers in walk-in cooler, dough rack in the proofer. Clean now and place on a more frequent cleaning schedule

Observed whole tomatoes being prepped without prior washing. Upon further discussion with pic, indicates that tomatoes and green peppers are not washed before service to consumer. Fruits and vegetables are required to be washed prior to service to the consumer in order to remove any contaminates. Correct as indicated above

Numerous light shields missing throughout facility in the prep area, dish room, and storage room. Lights need to be shielded to prevent glass fragments from getting onto prep areas, foods, and food contact surfaces. Correct by providing shields or plastic-coated bulbs

Filters missing from pizza oven hood. Hood filters are required for proper ventilation. Replace hood filters now

Caulking on 3-compartment sink and all hand sinks found to be in poor repair. Replace caulking so that sinks are firmly attached to the wall without gaps, so food and debris cannot accumulate. Large buckets used to store dirty pizza pans found to be in poor repair and no longer cleanable. Replace with new buckets that are smooth and cleanable.

Assorted potentially hazardous foods including chicken, beef, cut tomatoes, and roasted red peppers found to be holding at 44-50°F on the top rail of the prep unit on the right side. Potentially hazardous foods must be maintained at 41f or below to minimize potential for growth of bacteria and toxin. This may be a circulation problem due to large containers blocking airflow on this side of the prep unit. Eliminate large containers to allow for better air flow. Continue to monitor food temperatures. Call for repair of unit if foods are not being maintained at 41f or below. Corrected by discarding foods out of temperature control at time of inspection

(Critical) The handsink was blocked for access for routine handwashing by large tub for storage of dirty pizza pans. Do not keep this tub in front of the handsink. Handsinks must be accessible for routine handwashing. Corrected by relocating this tub to under the pizza oven.

(Critical) The metal stem thermometer for checking food temps was measuring 100 degrees f room temperature. Calibrate thermometer as discussed in ice water in order to obtain an accurate food product temperature reading when used.

Several cloth towels found on counters and shelves not in wiping cloth solution in between uses. Provide a wiping cloth solution with 50 ppm chlorine to disinfect cloth towels inbetween uses.

The light bulb in the walk in cooler is not plastic coated or shielded. The light bulb in the storage room is not plastic coated or shielded. Provide plastic coated bulbs and shields for bulb in the walk in cooler to minimize potential for glass fragments to get onto foods if bulb should shatter.

Food employee manager found not be wearing proper means of hair restrain. Use a ball cap, hair net or other effective means of hair restraint. Hair restraint is required to minimize potential for hair to get onto foods. Correct by providing hair restraint.
