(Critical) Observed sanitizer concentration in spray bottles used to wipe down lunch tables at 1000ppm of quat. Please remember that quat can become toxic at excessive levels. To correct, please mix quat bottles as per manufacturers directions on chemical bottle and verify concentration with test strips as needed. Pic corrected by remixing all sanitizer bottles to 300ppm at time of inspection. Violation corrected

(Critical) Lunchroom staff need to be verifying sanitizer concentration in sanitizer bottles with test strips more often. Corrected by educating all staff on proper sanitizer concentrations and test strip usage (including test strip location). Violation corrected

(Critical) Observed no soap at handsink in prep area (dispenser empty). All handsinks must be stocked with soap and paper towels at all times for proper employee handwashing. Pic stated that custodial staff has soap on order, but it has been over a week that dispenser has been empty. Hand soap needs to be on site at all times. Please obtain proper soap for dispenser as soon as possible. Pic corrected violation by supplying dish soap at handsink for handwashing until hand soap is refilled. Violation corrected

Observed missing lids on outside cardboard recycle dumpster in parking lot. All dumpsters must be equipped with tight fitting lids and doors to prevent entry of unwanted pests, insects and birds. To correct, please contact trash hauler and have lids on recycle dumpster replaced as soon as possible

Observed some old mice droppings on floors under oven and cooler in prep area. Old rodent droppings can attract mice and other pests. To correct, please remove all old mice droppings as soon as possible.

Observed floors under rethermalizer oven and under 2 door upright cooler in prep area with an accumulation of debris (observed moldy fruit under cooler). To correct, please clean now and on a more frequent schedule

(Critical) Observed can opener blade in prep area with an accumulation of dried on food debris and metal shavings in gear area. All can openers must be washed, rinsed and sanitized every 24 hours and must be clean to sight and touch. Pic corrected by washing, rinsing and sanitizing can opener at time of inspection. Observed dumpster enclosure area in need of cleaning. Food debris, cardboard boxes and litter observed on ground. To correct, please clean dumpster enclosure area now and on a more frequent schedule

(Critical) The milk cooler was found to be holding milk at 67 degrees f. Potentially hazardous foods must be maintained at 41 degrees f or below. Corrected by discarding milk. The milk cooler had circuit breaker had been tripped and cooler was not on, period of time unknown

The milk cooler circuit breaker was off. Most likely tripped off by adjacent wall outlet usage used by day care, etc. As per discussion, this has been a problem in the past and not been permanently remedied. A separate adequate circuit breaker should be designated for all appliances to avoid this problem in the future. Correct permanently asap. In the interim the cooler has been redirected to another outlet that is providing electricity, but this is only a temporary solution as the wire runs across the floor and over the sink to get to the working outlets.

(Critical) Ants observed throughtout the kitchen. Exterminate to eliminate infestation. Routine extermination may be required to eliminate infestation. Invoice for extermination required. Evaluation by licensed exterminator is necessary to determine proper course of action for permanent resolution of problem

(Critical) Cut melon found in the upright cooler at 49 degrees f. Potentially hazardous foods must be maintained at 41 degrees f or below during transport. It is determined that this is a transport issue. Food had beenn delivered in the am approximately 1. 25 hours prior to inspection. You must immediately provide sop's addressing this issue and retrain employess to maintain proper temperature control of potentially hazardous foods during transportation note: a non potentially hazardous food delivered at the same time was temped at 60 degrees f. It is most like that food is being transported without proper means of temperature control. Potentially hazardous foods and maintaining proper temperature control during that time. Corrected by all food being consumed during service time. Any remaining melon will be discarded if not consumed. Sop's for proper cold holding during transportation must be provided within 10 days

The domestic cooler has a bulb that is not shielded or plastic coated. Replace with a plastic coated bulb when it needs to be replaced.
