All chemicals and toxics must be properly labeled with correct chemical common name at all times for proper employee identification. If facility is serving raw or undercooked food items such as beef, fish, eggs, poultry, lamb, or shellfish, the facility shall inform customers of the significantly increased risk of consuming such foods. To correct, please provide correct consumer advisory statement on "weekly features" menu insert as soon as possible. I will check on this item at follow up inspection. (Critical) Observed facility not sanitizing plates and utensils correctly as i measured 0 ppm of chlorine in dishmachine (sanitizer container was very low at time of inspection). All dishmachines must sanitize as per manufacturers directions (in this case, 50-100ppm). To correct, please monitor soap and sanitizer levels more often and verify concentration with proper test strips as needed. Pic corrected by replacing sanitizer container at time of inspection. (Critical) Observed 4 tightly covered pans of freshly prepared housemade salsa stacked in wic with internal food temperatures of 90-100f. Salsa was prepared an hour ago per pic. As stated in past, all phf must be rapidly cooled from 135f to 70f within 2 hours and from 70f to 41f or below within a total of 6 hours. Do not cover and stack food containers in wic while actively cooling. Need to utilize ice baths and/or ice wands before placing hot foods in wic. Pic corrected by putting all salsa containers in ice baths to finish cooling at time of inspection. Violation corrected. Please note: this priority violation has been cited at 2/3 routine inspections. If violation is cited at anytime in the next year, enforcement actions will be necessary.

All phf in bottom portion of same unit hot holding at 145f or more. All phf must be hot held at 135f or more at all times. Pic corrected by discarding meatloaf and gravy at time of inspection. Did observe a shell stock tag on recently received oysters in wic. All shellstock tags must be kept on-file in facility for 90 days and must be organized as per last date of sale. All chemicals/toxics must be stored below and away from all food, plates/utensils and single service items at all times to protect items from potential contamination. (Critical) Observed covered pan of freshly prepared housemade rte salsa in wic with an internal food temperature of 120f (salsa was made an hour ago per pic). Salsa pan was covered with a tight fitting lid and was stacked under other food containers at time of inspection. All phf must be cooled from 135f to 70f within 2 hours and from 70f to 41f or below within a total of 6 hours. Do not cover and stack pans when actively cooling foods as they trap the heat inside pan. Need to utilize shallow pans and ice baths/ice wands before placing foods in wic. (Critical) Observed several insert pans of rte housemade sauces, coleslaw, and sliced tomatoes in prep top unit in cooking area with internal food temperatures of 51-54f. All of these pans are double panned in the unit and not completely under refrigeration. All insert pans that were not double panned were cold holding at 40f. All potentially hazardous rte foods must be cold held at 41f or below at all times. To correct, do not double pan the insert pans and keep all pans in the cold rails at all times. Since above mentioned foods has only been in prep top unit for 1. 5 hours, pic corrected by removing all double pans and placing food completely under refrigeration at time of inspection. Violation corrected.

Observed employee purse stored on top of/next to single service plastic lids (for dressings) on shelf in salad prep area. All employee and personal belongings must be stored in an area of kitchen which cannot possibly contaminate food or single service items. To correct, please relocate purse to approved location as soon as possible.

(Critical) Observed 2 pans of housemade sauces in hot holding unit on end of cooking line with an internal food temperature of 65-80f. Pic stated that sauces were removed from wic and placed in unit less than 30 minutes ago. All phf must be rapidly reheated to 165f or more within 2 hours. Please remember that hot holding units are designed to hold food, not for cooking or reheating. Need to reheat all phf on range top or in microwave oven before placing into hot holding. Pic corrected by rapidly reheating both sauces to 165f then returning to hot holding unit at time of inspection.

Food employees cannot handle rte foods with bare hands and shall utilize proper utensils and plastic gloves at all times. (Critical) Observed several drain flies in basement area (near empty beer/pop bottles). Also observed drain flies in: (mop sink area, dishmachine area, salad prep area, and bar area). Facility already working with eradico to control problem (saw invoice). To correct, please ensure all pop and beer bottles are rinsed thoroughly before storing and maintain all cleaning (floors, drains, equipment) as required. Also, please continue to work with licensed pest control company to eliminate and control problem. I will check this item at follow up inspection. Correct within 10 days.

(Critical) Observed all phf (raw beef, raw chicken, raw fish, cooked ribs, cheese and cooked vegetables) in 4 door drawer cooler under grill in cooking area with internal food temps of 46-51f. Upon further investigation, discovered that gfi outlet behind unit had tripped and cooler is not operating. All phf must be cold held at 41f or below at all times. Pic stated that no phf is left in these coolers overnight. All above mentioned food items were removed from wic less than 2 hours ago per pic. Pic corrected by taking all phf from these coolers and placing in wif at time of inspection. Gfi outlet was reset and all drawer coolers now cold holding at 40f. Please continue to monitor all coolers and food for proper cold holding temps as required. Critical corrected

Observed all coffee pitchers in wait station coffee area with an accumulation of water at bottom of pitcher. Stagnant water can accumulate bacteria. To correct, please completely air dry all coffee pitchers and store inverted if possible before storing in wait station area

Please note at time of inspection the pic requested an extension of the time period for correction of this item for 60 days as the menu is scheduled for revision at that time.

Observed freshly prepared food item at 56 degrees f. Not cooled prior to being placed into an insert pan at raised rail section of the preparation area refrigerator. The pic corrected this items during the inspection by utilizing proper cooling methods as required.
