(Critical) Observed deli meats in front counter reach-in-cooler with incorrect discard dates. Refrigerated, ready-to-eat, potentially hazardous food items shall be clearly marked with the discard date; add 6 days to date opened, prepared or removed from the freezer. Corrected at time of inspection by changing discard dates to reflect a 6 day time frame from date sliced deli meats, ham and turkey, pulled from the freezer

(Critical) Time as a public health control policy not being implemented properly for milk products at self serve station. If time without temperature control is used as the public health control for a working supply of potentially hazardous food approved written procedures shall be followed. Corrected at time of inspection: skim milk found with a 5 hour discard time frame (10 - 3) marked on side of carafe; facility is using a 3 hour discard time frame; found at 2:15 pm. In addition pic stated that any leftover milk product at the end of the time frame is poured into another container and placed in the reach-in-cooler to use again. Employee pulled skim milk carafe and discarded milk. New carafe filled with skim milk from refrigeration unit maintaining the milk <41°F, with a new 3 hour time frame marked on side of carafe with an erasable marker. Discussed time as a public health control policy as stated in biggy sop's under "three hour checklist". The milk carafes shall be changed every 3 hours per policy; any milk in carafe after 3 hours from the point milk was removed from temperature contol shall be discarded; the carafe shall be washed, rinsed, sanitized, air dried and placed in freezer or wic; a properly cleaned and sanitized carafe will be placed on self serve station with new milk and properly marked to reflect a new 3 hour hold time at the self-serve station. Please discuss with all staff; all milk product leftover shall be discarded at the end of the 3 hour time frame. Facility does have three skim milk carafes and two 1/2 & 1/2 carafes. If needed please purchase additional carafes to keep the cycle running smoothly

Light bulbs burnt out in some light fixtures rear kitchen. Please replace light bulbs to provide adequate lighting

Handwash reminder sign in women's restroom worn. Please replace handwash reminder sign notifying food employees to wash their hands

Observed mop stored improperly rear kitchen. Please hang mop to dry between uses

Current food safety manager no longer employed at this location. Please provide a valid and current food safety manager certificate for one full time employee for this location within 90 days. Not corrected

(Critical) No paper towels or paper towel dispenser at dishwash area handwash sink. Paper towels shall be provided at all times at all handwash sinks. Temporarily corrected at time of inspection by providing roll of paper towels on shelf near dishwash area handwash sink until paper towel dispenser can be installed. ,

(Critical) Rear dishwash area handwash sink not accessible for handwashing. Handwash sinks shall be accessible at all times for handwashing. Corrected at time of inspection by relocating signs that were blocking sink

(Critical) Food employee not familiar with employee health reporting requirements. The permit holder shall require food employees to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. Not corrected. Discussed with food employee. Pic will need to inform employees of health policy and reporting requirements. Will provide employee health poster at follow-up inspection. ,

(Critical) Thermos milk found at improper temperature at front counter. Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Corrected at time of inspection by discarding skim milk found at 69°F and half & half found at 54°F. Pic thinks they are using time as a public health control but could not find written time policy. Will begin using time as a public health control. At time of inspection provided clean thermos's for milk products with time out and discard time marked on outside of the thermos. Facility has chosen to use a 3 hour discard time. A written policy will be provided at follow-up inspection

(Critical) No soap available at rear dishwash area handwash sink; soap dispenser broken. Soap shall be provided in a soap dispenser at all handwash sinks at all times. Not corrected. ,

Observed coffee filters stored improperly at front counter. Please provide a covered container or store in original packaging to prevent potential contamination

These areas in need of cleaning: area under blender front counter, interior display cooler, exterior some syrup flavorings and floor/wall juncture in some areas. Please clean now and as often as necessary to maintain in clean condition

No cold water available at handwash sink in men's restroom. Please provide hot and cold running water at all handwash sinks at all times

Some caulk missing near front counter handwash sink between counter/wall juncture under coving. Please replace caulk to eliminate any gaps under coving

(Critical) Observed two yogurt parfait containers in the customer grab and go cooler with yesterday's expiration date (2-23). Corrected by discarding. Ensure staff are purging expired foods at the end of each day

(Critical) No paper towels were provided in the back counter dispenser and the hand washing station in the back kitchen was nearly out of paper towel as well. Corrected by an employee purchasing two, three packs of household style paper towel rolls. Ensure staff are keeping an extra back-up supply of paper towels for emergencies

(Critical) Staff indicated the insulated coffee cream carafes are only washed and rinsed between the three hour use periods. Ensure staff are properly sanitizing this equipment after washing and before re-using. Corrected by performing this task

The mop head was resting in the bottom of the mop sink. Corrected by the manager inserting the mop handle into the holder above the mop sink basin

The floor sink under the front counter is over-due for cleaning. Ensure this floor sink is cleaned at least three times per day to remove debris accumulation

The ice pail was observed stored upright on top of the ice machine. Corrected by providing a clean tray to store this ice pail and the ice scoop inverted on top of this tray.

Observed the sanitizer in the two wiping cloth storage containers to be too weak. Corrected by providing fresh sanitizer in both of these containers at 350 to 400 ppm quat

(Critical) Again, no paper towel was provided in the paper towel dispenser in the back kitchen at the hand washing sink. Corrected by providing paper towel on this holder

1) the bulk metal container for iced tea was found on the floor of the walk in cooler. Elevate this food storage container off of the floor at least 6". 2) the coffee filters are being stored on top of the coffee brewing equipment. Provide a container or bag to store these filters in for protection prior to use

Observed the dispensing spouts for the three brown, insulated coffee dispensing containers to not be disassembled for cleaning after they were last used. Ensure staff fully disassemble these spouts for washing. The spouts were sent to the 3 comp. Sink for cleaning

The general manager, nicole minney, is no longer working at this establishment. She was the certified food safety manager for this facility. Within 90 days provide a full time manager with a nationally accredited food safety certification

Observed excessive dust accumulation located on two grates in the ceiling of the walk in cooler. Thoroughly clean these grates at this time and more frequently in the future

Observed the drain pipe manifold for the three compartment sink to be propped up by a box to keep it from sliding down. Repair as needed to keep the drain pipes stationary

Observed the door gasket for the back counter, one door cooler to be torn/split. Replace the door gasket to provide a smooth and easily cleanable surface

Observed two of the three containers of sanitizer at the front kitchen area to be too weak. Corrected by providing fresh quat sanitizer at 200ppm

Observed two water leaks in the facility: 1) the left-hand bunn coffee brewer on the back counter is leaking water from the top; 2) observed water dripping onto the floor from the ice making machine in the back room. Repair these water issues asap (must maintain the existing air gap for the ice machine/bin drain lines) to eliminate the water on the counter and floors

No paper towel/towel dispenser is provided in the back room hand washing sink. Provide a stocked paper towel dispenser w/in 45 days. A follow-up inspection will be required for this item

The following areas are in need of cleaning now and more frequently in the future: the floor sink located under the front counter (drain for the dipping well and espresso machine), and the floor under the false bottoms of the back counter base cabinets.

Observed the door gasket on the one door beverage air cooler at the back counter to be torn. Provide a replacement gasket that is smooth and easily cleanable

Observed dust and spilled oatmeal accumulation in the customer grab & go cooler (at the bottom, front of the unit where the cold air comes out). Regularly clean this cooler to keep it clean
