Corrected at time of inspection by discarding yogurt found at 45°F in display case. In future employees will place yogurts in rear section of display cooler and monitor food temperatures with thermometer to assure yogurt maintains at 41°F or below at all times

Refrigerated, ready-to-eat, potentially hazardous food items must be clearly marked with the discard date once opened, prepared or removed from the freezer. Corrected at time of inspection by discarding some wrapped deli meats and marking the two with the proper discard date that the pic removed from the freezer this morning.

The permit holder shall require food employees to report to the pic information about their health and activities as they relate to diseases that are transmissible through food. Corrected at time of inspection by providing and discussing content of mda employee health poster. Pic will review with all staff, including the big 5 illnesses and the reportable symptoms

Food employees shall clean thir hands and exposed portion of their arms immediately before engaging in food preparation. Corrected at time of inspection by discussing when employees should wash hands. Observed cashier contact money, put on gloves then prepare a bagel sandwich. Employee now understands that she will need to wash her hands after contacting money before she puts on gloves to make a sandwich. See item (h) " before donning gloves for working with food" cashier was the only employee present at time of this inspection

Refrigeration door gaskets on both reach-in-coolers front counter in poor repair. Please replace to provide good seal and so smooth and easily cleanable
(1) soap dispenser in poor repair at handwash sink in rear kitchen. Please repair; still able to dispense soap but difficult. (2) backsplash between wall and counter behind handwash sink front counter pulling away from the wall. Please reattach and caulk to provide good seal

Refrigeration door gaskets both reach-in-coolers front counter in need of cleaning. Please clean now and as often as necessary to maintain in clean condition
Observed several fruit flies at front counter near handwash sink. Please continue to work with your extermiantion company to eliminate the flies. Facility using traps and are keeping cabinets under counter very clean. Did observe a gap above the backsplash between the counter and the wall behind the handwash sink; please recaulk this area to prevent any standing water behind backsplash.
(Critical) Observed two portions of deli meat in the one door low-boy cooler by the cash register to be expired on 3-1 (two days prior). Corrected by discarding this food
(Critical) Observed +/- 6 portions of bacon and deli meat in the one door low-boy refrigerator without last day of use dates on them. The barista indicated she just pulled these from the freezer this morning. Corrected by providing a last day of use on each of the packages. Ensure staff provide this date promptly when these foods are pulled from the freezer
The plastic ice machine bin ice scoop has a large crack in it. Provide a replacement ice scoop (if this is a common occurrence, consider purchasing an aluminum scoop)
The bottom edge of the ice machine bin deflector shield has some slime build-up on it. Clean and sanitize this surface now and more frequently in the future to prevent this accumulation
The outsides of many syrup bottles have excessive syrup accumulation on them. Maintain these surfaces clean
The large plastic ice scoop for the ice machine was found in the ice bin with the handle touching some ice. Ensure the scoop is stored in the ice scoop holder or outside of the ice bin, upside down on a clean surface. Corrected by placing the scoop in the holder
Back counter base cabinet under hand washing sink: re-establish the drain line air gap for the two white, flexible drain lines from the back-flow preventers under the coffee brewing machines. Re-attach these drain lines with cable ties to the pvc hand sink drain pipe
(Critical) Back kitchen hand washing sink: the soap container was empty. Corrected by providing a replacement soap insert. Maintain soap at your hand washing stations so employees can properly wash their hands
(Critical) Back kitchen hand washing station: no paper was available in the dispenser. Corrected by replacing the paper towel. Ensure paper towels are provided so employees can properly dry their hands
(Critical) The employee in charge indicated the blender pitchers, sandwich knife, cream cheese spreader, mixing spoons, cream pitchers, etc. Are washed at the end of the night. These food contact surfaces are in contact with potentially hazardous foods and require formal washing, rinsing and sanitizing in the three compartment sink on intervals not to exceed four hours. Per a biggby log chart, this task is to be carried out every three hours. Ensure all staff are reminded of this requirement and that it is performed when required. Corrected by discussing this issue with the employee and reminding him to clean these items on the required frequency
The floor is in need of cleaning under the ice making machine and the adjacent one door upright refrigerators/freezer.
Several surfaces inside the ice making machine and the ice deflector in the bin below are over-due for cleaning. Schedule this unit for cleaning

1) observed the men's bathroom door to have a damaged and/or loose hinge(s). This is causing the upper part of the door to hit the door frame on the latch side. Repair/replace as needed so the door closes properly. 2) back counter dump sink: observed moisture seeping under the sink counter flange and dripping into the base cabinet area below. Remove the sink basin, remove old sealant, and after cleaning and drying the mating surfaces apply a new sealant to prevent moisture from migrating under this flange
Observed no soap provided in the dispenser at the back of the house hand washing sink. Obtain an additional soap container for this station asap

Observed the wet mop head stored hanging to the left of the mop sink basin. Please ask staff to hang the wet mop above the mop sink in the mop rack provided so all moisture drains in to the mop sink basin and doesn't leave moisture on the wall or floor
Observed two containers at the front kitchen area with wiping towels to have a low sanitizer concentration (<100 ppm). Corrected by providing fresh sanitizer at 200 ppm
Observed the open drain located in the back counter base cabinet below the sink basins to be unclean. Thoroughly clean and rinse this drain at least 2x per week to remove soil accumulation. Also, the floor under the ice making machine is due for cleaning
No hand washing soap was provided at the back counter hand washing sink. The dispenser came off of the wall. Temporarily corrected by providing a large bottle of hand washing soap on the counter. Ensure hand washing soap is always conveniently available.
