(Critical) Upon arrival, observed an employee with artificial fingernails scooping french fries. Food employees must either remove artificial nails or utilize single use gloves when handling exposed foods (french fries, ice, etc. ). Corrected by discussing this issue with the employee and by the employee using a single use glove when handling food

(Critical) The following equipment is in need of back flow prevention devices: 1) the new latte machine, 2) the relocated bunn coffee maker, and 3) the icee machine. Install an asse 1022 device with a "y" strainer on the individual water supply lines to these units. These back flow prevention devices need to be installed in a serviceable location

(Critical) The attachment surfaces for the drive through soda station dispensing spouts to be unclean. Thoroughly clean these surfaces and maintain clean on a daily basis

(Critical) Dining room beverage station base cabinet: observed a water leak from some plumbing at the north end of this cabinet (quite possibly from the back flow preventer). Repair as needed to eliminate the source of moisture

1) the floor drain in the back hallway near the co2 tank is nearly plugged. 2) the floor sink under the dining room beverage station is unclean. 3) the floor under the drive through beverage station is also unclean. Please thoroughly clean these areas now and multiple times per week

Observed the clean dish drain board to be unclean. Corrected by cleaning and sanitizing this surface

Observed utensils (mostly tongs) hung up above the three compartment sink to be subject to splashing when equipment is pre-cleaned and washed. Relocate these utensils to a higher location on the rack or to another storage shelf for protection

The walk in cooler is poorly lit. Provide a brighter bulb or additional lighting

Provide no smoking signs at the entry points to your facility

(Critical) Observed the following food contact surfaces to be unclean: the interior of the ice machine, between the push block fins of the onion slicer, the interior of the drive-thru window ice holding bin, and all of the soda dispensing spouts. Clean all of these items/areas now and as appropriate in the future

Observed a tube of soft serve cups with the end of the package open and some dust and a mosquito in the end cup. Corrected by removing the end cup and sealing the bag to prevent misc. Contamination. Also, observed a stack of burger containers stored upright. Corrected by inverting these food containers to prevent misc. Contamination

Observed a couple out of the way floor areas that are in need of cleaning. 1) behind the drain pipes and rear corner leg of the two compartment food prep sink, 2) the floor under the drive-thru window ice bin & soda station, 3) observed two low floor areas with accumulated standing water (one behind the hot water heater and another by the unused first drive-thru window). Please thoroughly clean and/or mop up these areas.

(Critical) Water faucet at the three compartment sink: provide a water hose meeting nsf standard-61 (or equivalent) in place of the black rubber hose that is not approved for a potable water installation

Observed cardboard being used as a splash guard to the right of the juice dispenser at the drive through window station. The gm indicated they have someone working on providing a metal splash guard. Once installed ensure any joints or seams are sealed with a bead of silicone sealant

Observed the floor sink located under the ice machine ice bin to be in need of a more thorough cleaning to remove the black build-up

(Critical) Observed the following items to be unclean: 1) most of the spare duke phu pans on the clean equipment storage shelf; 2) the interior of the ice making machine and the ice dispensing chute area in the kitchen next to the three compartment sink; 3) the mounting bases for the soda dispenser stations (all four units). Clean these items at this time and maintain them clean

Observed many plastic food containers stacked in storage with moisture still on them. Please allow dishes to fully air dry prior to stacking

Observed the floor under the drive through station ice holding bin to be in need of cleaning, and several floor sinks in the facility. Maintain these areas clean to minimize the attraction of pests

Observed three areas that are in need of sealing to eliminate gaps: 1) the joint between the drive through station beverage dispensing unit and the top of the ice holding bin; 2) the customer drink bar station counter top at the side wall; and 3) the bases of the toilets at the floor tile

The door sweep for the back delivery door is worn. Replace this sweep to seal out pests. Consider installing the type with nylon bristles--they seem to have a longer life
