(2) some single-service items stored under drain lines in dining area cabinets. Please relocate single-service items or provide waterproof containers to protect against potential contamination

(Critical) Observed raw bacon package stored improperly on top of salad reach-in-cooler. Food shall be protected from cross contamination by separating raw animal foods from ready-to-eat food items. Corrected at time of inspection by relocating raw bacon to the bottom shelf in walk-in-cooler and discarded product (salad ingredients) on top of salad prep unit. Raw bacon is the only thawed raw animal food stored in wic; burger's remain frozen. Chicken is fully cooked; eggs are pasteurized.

These areas in need of cleaning: interior bottom cabinet under slushie machine and floors in rear storage area. Please clean now and as often as necessary to maintain in clean condition

Please repair or replace toilet tissue dispenser in employee restroom so that toilet tissue can be stored properly in dispenser

Observed soiled dishes stored on floor in dishwash area. Please provide adequate shelving or cart to store soiled dishes properly

No waste receptacle in employee restroom. Please provide waste receptacle for proper disposal of soiled paper towels. Corrected at time of inspection by providing waste receptacle

(Critical) Lettuce was just prepped around 9 am. Some of the pans were placed in the walk in cooler and about 4 to 5 pans of lettuce were placed out on the make line areas. The provided stickers had discard times of 2, 2:30 and 3 pm. These discard times exceed the maximum of four hours allowed. Corrected by providing new discard stickers for 1 pm

(Critical) Observed fryer equipment stored in front of the employee restroom hand washing sink. Corrected by relocating these parts so the hand sink is properly accessible

Observed the side kitchen delivery door to have gaps around the perimeter. The manager indicated the dead bolt is only partially latching because the door doesn't fully close (could be due to damage of the door/frame or ice heave). Repair as needed so the door fully closes to prevent pest entry and heat loss

The light fixture in the employee restroom (towels and paper goods are stored in there) does not have a cover. Provide a cover or protective bulb tubes and end caps to prevent bulb breakage

Observed an employee who placed a case of tomatoes on the floor prior to washing and prepping them. Ensure staff maintain foods off of the floor to prevent contamination of the food and on any subsequent surfaces the case is placed

The floor drains in the following areas are over-due for cleaning: between the ice machine and the three compartment sink, and the drain in the base cabinet below the dining room drink station

Observed the mounting surfaces for the soda spouts and diffusers for many of the drive through beverage station to be in need of more frequent cleaning. Also, ensure the pump spouts for drink flavorings are cleaned after the containers are empty

Observed cardboard being used as a divider for packaged plastic ware at the deep fryer station. This cardboard absorbs moisture/oils and is not cleanable. Eliminate this cardboard and provide a material that is smooth, easily cleanable and non-absorbent

The clear blue ice transfer pail has the bottom corners broken out of it. Asap provide a replacement ice pail. You may wish to consider a different ice pail that is constructed of less brittle plastic

Observed a gap at the bottom of the kitchen delivery door (east side). Repair the weather stripping as needed to eliminate the gap that could allow pests access to the kitchen

Provide a metal stem food thermometer at your dish washing station so staff can verify the required 110f minimum dish washing water temperature

(Critical) Observed a carton of one dozen raw, in-shell eggs to be stored on the top shelf in the walk in cooler. Corrected by removing the eggs from the facility

(Critical) Observed the recently reheated commercially packaged mushrooms in a hot holding pan in the duke warmer to only be 80f. Corrected by reheating to above 135f

Observed the lid/cover for the egg cooker to be in need of daily cleaning. Ensure staff are removing this piece to be cleaned

Observed a custom sheet metal liner installed in the bottom of the mop sink and up the front edge that was installed since the last inspection. The perimeter of this metal and the cut-out around the drain are not properly sealed to the surrounding tile to prevent moisture and debris from migrating under the sheet metal. Additionally, the black sealant used was apparently not for this application as it was found to be mushy. This situation will likely allow flies to breed in the debris build-up in the future. Remove this sheet metal so any damaged tiles can be replaced and then grout as needed to eliminate any depressions

Observed a food handler to be wearing a jeweled ring on her hand. Ensure staff either remove all rings (except for a smooth band), wrist watches, or bracelets prior to starting their shift. Alternatively, individuals wearing intricate/jeweled rings my wear single use gloves over the ring(s) as long as they are changed as required through-out their shift

(Critical) Approximately 7 of 8 phu pans, a plastic food pan and lid, and elevated phu drip trays: observed most of these pans with old stuck-on food debris, and/or an oily layer remaining and/or fresh food residue still remaining on the food contact surfaces after they have been cleaned and dried. Observed 4-5 squeeze bottle spout tops with food residue still in them. The interior surfaces of the french fry dispenser were observed to have an oily residue and some brown & black specks on the rotating drum, the chute area and the hopper. This dispenser was evaluated in the morning during breakfast hours prior to it being put into use for lunch. These items were sent to be re-washed prior to being used further. Management and staff must ensure food contact surfaces are thoroughly inspected for cleanliness after cleaning, sanitizing and air drying.

The door gasket is torn on the beverage air one door low-boy cooler containing the macaroni & cheese, eggs, cheese and ham. Replace to be easily cleanable and to keep in your cold air in the unit

Upon arrival the walk in cooler was operating at a slightly elevated temperature. A repair technician was already on-site fixing a variety of items at the store. He made an adjustment to a refrigeration setting and the cooler was operating in the upper 30's in short order

The mounting surfaces for the soda diffusers and spouts at the drive-thru soda dispensing station are in need of more frequent and thorough cleaning. Ensure these surfaces are maintained clean on a daily basis.

Observed the trough drain area for the customer self service drink station to be in need of more frequent cleaning

(Critical) Observed the dispensing hopper with french fries to not have the discard time indicated on the chart. An employee indicated the fries had been in the unit for over the one hour maximum time. Corrected by discarding the fries, providing a new bag of fries from the freezer and marking the chart with a one hour hold time

(Critical) Observed the conveyor feed devices for the broiler not being cleaned at the required frequency (every four hours). The broiler is first used at +/- 10 am, and these parts are not cleaned until 4 pm. Corrected by cleaning and sanitizing immediately. In the future, ensure the managers are requiring these parts to be cleaned and sanitized at 2 pm, 6 pm, 10 pm and at closing

(Critical) Observed several plastic food storage containers and bacon cooking trays to be difficult to clean due to cracks and melted interior surfaces. Provide replacement containers/trays as needed. In the future, instruct dish washing staff to bring these damaged items to the attention of the manager

The manufacturer of the pressure vacuum breaker (watts regulator) requires the internal components to be replaced every five years. Water was observed to be leaking out of the vent cap during the inspection today. No documentation is available indicating the internal components have been replaced at this store. Have a state approved certified tester replace the internal components on the watts regulator pressure vacuum breaker located at the mop sink faucet and then test the unit for proper operation. Your contractor is required to provide you with a testing form at the conclusion of the testing. Forward this test form to me as verification the work was performed.

Observed several areas of the floor tile to be missing or worn away near the deep fryers and by the floor drain located in front of the walk in cooler door. Please remove any remaining grout in these deteriorated areas, thoroughly clean and dry the surface prior to re-grout

Observed an in-training employee only wearing the cut resistant gloves for slicing tomatoes. The manager was not aware of the requirement to have single use food safety gloves worn over the cut resistant gloves. Corrected by providing food service gloves over the cut resistant gloves

Observed paper towel not dispensing from the main kitchen hand washing sink dispenser. The paper towel were mis-fed in the dispenser. Corrected by properly feeding the paper towel through the unit (this dispenser still does not provide paper towel very well). A new paper towel dispenser was just delivered. Install this unit as soon as possible

No soap was provided in the dispenser at the side kitchen hand washing sink (near the three compartment food prep sink). The manager was not able to find any extra soap packages in the storage room. The manager indicated soap is on the order that is being delivered tomorrow. Have a staff person purchase some pump soap at a nearby store tonight to use until soap is delivered tomorrow
