(Critical) Observed chicken nuggets in the duke hot holding unit to be at 123 to 136f. Corrected by discarding these nuggets.

Now that more equipment has been added to the kitchen extra storage space is at a premium. There are many boxes of paper work taking up significant space. Please remove these boxes and store in another location to free up additional space

Observed a previously opened box of french fries stored on the floor in the walk in freezer. Relocate this food to the storage shelf

Observed the oil/grease catch pan to be missing from the deep frying exhaust hood drip tray. Provide a oil catch pan

Several areas of the back kitchen are in need of tile and grout replacement such as at the mop sink and near the ice machine & dish washing sink. Repair as needed to eliminate the depressions that accumulate moisture and debris

Observed the following locations to be in need of cleaning at this time: the floor under the drive through beverage station, under the bag in a box soda syrup rack, under the ice machine bin and behind the grease trap

Observed areas of the tomato slicer to be worn. Replace the blade assembly and the push block

Observed the press board counter under the coffee maker and juice dispensing station at the drive through window to be sagging due to deterioration. Replace this counter with a material that does not absorb moisture

(Critical) Management indicated the broiler loading tray and dispensing chute on the duke flexible broiler are only wiped down every fours hours with a sanitizer towel and then fully cleaned at the end of the night. These food contact surfaces (along with your tongs, phu pans, etc. ) used with potentially hazardous foods require cleaning, rinsing and sanitizing every four hours. Corrected by discussing this issue with staff who will remove these broiler food contact surfaces no later than four hours from the point in time it was first used and on intervals not to exceed four hours through-out the rest of the day

Observed a back kitchen employee wearing single use food gloves opening doors on a locker to place something in the locker and then return to food activities without removing the gloves. Corrected by the employee removing the gloves

Observed multiple phu pans stacked while still very wet on a clean dish storage cart. Please keep these dishes separated until they are air dried

Observed a front counter employee who was involved with bagging fries and making drinks, etc. Wearing a jeweled ring. Please ensure all food handling staff with rings either remove them before starting their shift or wear gloves on that hand to prevent jewels from falling into food or the accumulation of food in the jewelry.

(Critical) Observed an employee on the make line assemble a sandwich with bare hands. Corrected by the gm requiring the employee to wash his hands and put on single use gloves

Observed an open hole (due to a missing plug) in the top of the three compartment sink grease trap cover. Please provide a replacement threaded metal plug to seal off this opening (at a minimum a rubber plug if the threads are damaged)

Observed the sidekick/flow-through back flow preventer to not be functioning properly (the water supply was turned on and no water was flowing through the small orifices at the bottom of the unit). This situation is likely due to mineral build-up. Please clean/repair this back flow preventer as needed so water exits the bottom of this unit when the water is turned on at the faucet

Observed the sliding door on the side of the dumpster to be left open. Please discuss with staff about the requirement to keep the doors and lids of the dumpsters closed to prevent pest entry

Observed the mounting bases for all of the soft drink dispensing spouts to be in need of cleaning more frequently (drive through and dining room). Please thoroughly clean and sanitize these clean in place surfaces

Observed the following areas to be in need of cleaning now and more frequently in the future: 1) the floor under the drive up window area ice bin and the adjacent wire rack shelving unit; and 2) the floor sink and the surrounding tile area located under the three compartment dish sinks

A large gap is evident under the back delivery door. Please adjust/repair/replace the door sweep to prevent pest entry

Mop sink: observed the hot water valve to be inoperable (it is stuck in the on position), and the handle for the cold water valve is starting to slip. Provide replacement valve inserts and handles for this mop sink faucet
