(Critical) A small amount of ham (1/5th of a bag/ +/- 16 slices) were out on the counter during breakfast service without a discard time. This ham was at 64f. Corrected by discarding. In the future, any phfs not held under refrigeration must have a discard time (not to exceed four hours) label on it. Time control foods may not be re-refrigerated--they must be discarded at the end of the time period (or at the end of breakfast even if the full amount of time has not elapsed).
(Critical) The grease trap was overflowing onto the floor during the inspection as employees were using the dish washing sink. The two back corner screws were missing from the cover. The gm indicated the service company is en route to the store to clean out the system. The proper hardware must be installed on this trap cover
(Critical) Observed pulled pork that was opened and portioned this morning to have a last day of use of 10-22. Corrected by providing a new sticker with a 10-6 discard date
The one door silver king low-boy cooler next to the ice machine was not operating at 41f or below. This unit was unplugged to possibly defrost. If this unit doesn't maintain 41f or below after plugging back in, have it adjusted or serviced as needed
Observed a few stacks of paper clam-shell type food containers on a back kitchen storage shelf to be stored upright. Discard the container on the top of the stack and invert these containers or store them in a plastic bag for protection
Soda dispensing system: the white diffusers and the surfaces they mount to are over-due for cleaning. These surfaces need to be washed, rinsed and sanitized nightly
It appears water (from a rain event the previous day) is migrating into the deep fryer exhaust hood. Adjust/repair the roof-top exhaust system as needed to prevent water from entering the duct and traveling down into the hood filters
A number of the floor sinks/drains have a build-up of food/slime. These areas need cleaning at least 2-3 times per week.

(Critical) Observed 11 of the 14 available phu pans to have food (ground beef or egg) or oil residues still on them after being washed last night. Ensure staff are very thorough in their cleaning (especially the corners, dimple in the center and the ledge around the perimeter). A cleanliness inspection is necessary to evaluate this equipment the after fully air drying. Please implement any necessary changes to your washing/drying/inspection process. Also observed the onion ring scoop at the hot holding well (not used yet today) to have oily residue on it from yesterday. Some of the mounting surfaces for the soda system diffusers to be unclean. Please thoroughly clean these items today and maintain them clean in the future
(Critical) Observed two pans of sliced tomatoes (one was a full pan and another was half empty from being used on the assembly line last night) in the one door low-boy reach in cooler by the ice machine. No discard time stickers were on these pans. Once sliced tomatoes go on time control they may not be saved and must be discarded. Staff need to discard all foods on time control at the end of the night. Corrected by discarding
(Critical) Observed all three drain pipes from the three compartment sink to be directly plumbed to the new grease trap. At least one sink basin (manager indicated the sanitizing basin is used) must have an air gapped drain line for food prep activities. Provide an air gapped drain for this sanitizing basin
No thermometer was provided in the one door low boy cooler next to the ice machine. Provide an air thermometer so staff can monitor cooler temperatures.
Observed nearly all of the plastic phu pans an a few metal kitchen pans to be stacked on a storage shelf while still wet. Allow this equipment to fully air dry separated from each other prior to stacking them on a storage shelf for re-use
Observed two difficult to reach square floor sinks to be overdue for cleaning. They are located under the ice machine and under the ice bin at the smoothie beverage station. Thoroughly clean these drains now and about 2-3 times per week in the future to eliminate this debris build-up that will attract insects and allow them to breed
Some tiles are damaged around the mop sink drain and grout is worn/missing in this same area as well as under the ice machine. Repair as needed.
(Critical) Observed the new drain line for the walk in cooler to be installed without an air gap. To correct cut and re-route the drain line to come across the wall at a higher elevation to provide an air gap of 2x the discharge pipe diameter above the flood level of the receiving plumbing
Observed a new smaller storage shelf located in the walk-in cooler to be located directly under a pump for the condensate/defrost melt water. This pump housing has some water droplets located on the bottom of it. Also this shelving unit is located under an uninsulated copper drain pipe for the cooling system drip pan. To correct move the shelf so the food is not stored under the pump and insulate the drip pan drain line (only the horizontal portion from the drain outlet to the wall)
Observed an employee working the drive-through station who prepares drinks and places food into bags for customers to be wearing engagement and wedding rings with jewels in them. Please either remove the rings prior to starting your shift or wear a single use glove over your rings when preparing/handling foods
Observed no sealant in the gap of the three compartment sink back-splash to the wall. Provide a silicone sealant to prevent moisture and food debris from migrating behind the sink

Observed the drive through employee wearing a bracelet and a ring w/small jewels in it. Instruct employees to remove all jewelry from hands and wrists (except a smooth ring/wedding band) prior to starting their shift
Observed recently washed metal pans that were stacked and placed on the chrome storage rack prior to being fully air dried. Allow time for dishware to air dry before stacking.
The floor under the make line (especially around the electrical conduit) and the floor sink under the ice machine are in need of cleaning now, and more frequently in the future
The mop sink faucet (contains an atmospheric vacuum breaker) has a hose connected to it with a spray gun at the end of it. The built-in back flow preventer is not compatible with down-stream shut off valves. To correct provide either 1) a flow-through/bleed off back flow preventer installed between the mop sink faucet and the hose or 2) install a pressure vacuum breaker
Observed a couple wiping cloths not stored in the sanitizer solution (one on the counter by the shake machine and the other in the front area hand washing sink basin). To prevent bacterial growth between uses only store wiping cloths in your sanitizer pails.
Observed an employee with artificial nails working with exposed foods (french fried potatoes, onions, and ice for drinks) with out the use of gloves. Corrected by the gm discussing this requirement with the employee and by the employee washing her hands and putting on gloves
The floor sink located under the ice machine bin is not draining properly. When the ice machine cycles the basin overflows. Clear the blockage in the drain line so the water is carried away as intended
Back kitchen--observed the following areas to be in need of tile & grout repair: the walkway at the sandwich make line, in front/under the deep fryers, and at the back hallway/mop sink area. Repair these areas to eliminate the current depressions that allow debris and moisture to accumulate
1) the under side of the metal dispensing flaps for the french fry dispenser, and 2) the black plastic dispensing spouts for the coffee machine were observed unclean. Please clean these items now and on a daily basis in the future
Observed a few small flies at the large bag of raw onions on the bottom shelf of a prep table. Please ensure staff are inspecting the facility for the presence of pests
Missing grout and/or cracked grout and tile were observed in the following areas: 1) behind the floor sink for the three compartment sink, 2) at the sandwich make line area, 3) at the mop sink drain and back splash area. Repair these areas as needed to eliminate depressions and gaps where water can enter
Observed significant gaps under the back kitchen delivery door. Provide a replacement door sweep on the exterior of this door (suggest the nylon bristle type for better wear resistance) asap as mice will be trying t find ways to enter buildings this fall to keep warm
1) the rubber gasket/seal for the shake machine cover has shrunk(allowing this seal to fall into the shake hopper), 2) the drive up window is in poor repair (one of the doors is non-functional). Repair or replace these items as needed
1) onion rings and french fries observed on the floor under the deep fryers, 2) the wall and water supply line under the mop sink faucet are unclean, 3) there appears to be a leak in the exterior drain cap on the waste oil drain line. The above areas are in need of the cleaning at this time (and repair of #3 if needed).
