(Critical) Person in charge not familiar with employee health reporting requirements, including the big 5 illnesses. The permit holder shall require employees to report to the pic information about their health and activities as they relate to diseases that are transmissible through food. Corrected at time of inspection by: discussing employee health requirements including reportable symptons and reportable diagnosis. In addition provided facility with the mda employee health poster to review with all staff. Pic could not find any information in burger king sop's regarding employee health. She will check with her general manager

Observed standing water on floor in mop/furnace room. Pic is aware of leak, trying to determine source. Please repair and maintain in good repair

Floor tile grout missing in several areas rear kitchen. Please re-grout as needed to provide smooth, easily cleanable surface and to prevent standing water in these areas

Some clutter and unnecessary items stored near fryers (greasy box of miscellaneous items) and rear corner next to single-service storage shelving unit. Please sort through and get rid of any items not used in the operation and maintenance of facility. This will make it easier to keep clean and prevent potential harborage conditions for pests

Salad reach-in-cooler lid not attached properly; please repair and maintain. In addition please provide addtional spacer bar for the top of this unit to close off any gaps

(1) some single service items stored improperly on shelving unit rear kitchen. Some single-service items opened from incorrect end; please open from bottom to prevent potential contamination of food-contact end. (2) clean squeeze bottles stored improperly on shelving unit rear kitchen. Please invert clean bottles to prevent potential contamination

These areas in need of cleaning: floor/wall juncture throughout especially near fryer station and under equipment, base of handwash sinks near foot pedals, hood canopy over fryers, floors next to and under fryers and interior clean squeeze bottle lexan. Please clean now and as often as necessary to maintain in clean condition. Please reattach or remove black pipe wrap under ice machine drain line to make it easier to clean floors in this area

These areas in need of cleaning: floors under pop dispensing machine by drive thru, underside pop dispenser housing drive thru, floors under ice bin / coffee station, floor/wall juncture in several areas rear kitchen and top ledge of chest freezer near fry station. Please clean now and as often as necessary to maintain in clean condition

One of the ice machine drain lines not air gapped properly. Currently an air break; the air gap shall be twice the pipe diameter above the flood rim of the drain

Floor grout missing in several areas rear kitchen. Please re-grout to provide smooth, easily cleanable surface and to prevent standing water between the floor tiles

Observed gap at bottom of rear door. Please provide door sweep to eliminate any gaps

Cup dispensers overfilled front counter. Please do not overfill cups to prevent potential contamination of lip-contact end of cup

Inadequate light intensity in corner near fry station. Appears one light bulb in fixture burnt out in fixture closest to fry station. Please install highest wattage light bulbs in fixture to provide adequate lighting in this corner

(Critical) Observed four of the yellow plastic hot holding pans to still have food (egg, sausage) residue or grease build-up present. Additionally, two pair of blue tongs were observed to have old food debris on them. Corrected by re-washing these pieces of equipment

(Critical) Although the two pans of lettuce and the two pans of tomatoes on the make lines had stickers with discard times provided, all of the back-up pans of sliced tomatoes (four pans) and chopped lettuce (three to four pans) stored in pans below the make line were not provided with discard times. Corrected by marking these pans with 2 pm discard times (four hours past their prep time). Ensure all staff are familiar with the time control policy and are properly marking these foods once removed from the cooler and sliced/chopped

(Critical) Observed a pan of chicken nuggets in the hot holding unit to be at 126 to 132f. Corrected by discarding this food

Many of the stacked, recently washed plastic pans were observed to still be wet. Ensure staff are keeping these pans separated to fully dry prior to stacking them for storage

The floors under the drive through beverage station and under the icee machine are due for cleaning to remove misc. Debris

The stainless steel covers for the tops of the two dining room soda dispensing system ice bins are bent and do not fit properly. Repair these covers so they fit on top of these bins properly

Observed a cardboard box containing plastic breakfast platter trays/lids (stored in plastic bags) to be dripped on while in storage (located in the base cabinet under the dining room beverage station equipment). Relocate this case of containers to a protected location

Re-establish the drain line air gap for the for the ice machine / bin unit above the flood rim of the receiving basin. The vertical air gap distance must be no less than 2x the discharge pipe diameter

(Critical) Observed the drain line for the smoothie station ice bin to be now combined with the waste-water drain line for the blender rinser. The drain lines for these two pieces of equipment must be separated again (individual drain lines from equipment to final disposal location) or an air gap must be provided on the ice bin drain line at a location up-stream from the tee fitting that combines these two drain lines. ,

Observed two dial type air thermometers located in the one door low-boy cooler at the drive through station to be registering 20 to 22f. Foods in this unit were only 37f. Provide a replacement air thermometer in this cooler. You might consider a different type of thermometer (the narrow horizontal or vertical kind with red alcohol to indicate the temperature)

Observed the following areas to be in need of cleaning: the drain line for the smoothie ice bin, the floor by the hot water heater, the floor behind the grease trap and floor sink (under the three comp. Sink). Please clean these areas now and on a more frequent basis in the future

An employee set up the three compartment sink for dish washing during the inspection. The clean dish side drain board was not washed or sanitized to remove some debris. Ensure staff are cleaning off this drain board when setting up the sink for washing dishes

Health Report - 08/08/2012 (1 critical incident(s))

(Critical) Just prior to 2 pm an employee was observed removing the discard time stickers from several food pans at the chicken sandwich make line and replace them with stickers indicating a 6 pm discard time without discarding the potentially hazardous foods (cut lettuce and sliced tomatoes). Corrected by having the employee discard these two foods, providing fresh product in clean containers and marking them with a discard time of 6 pm. Ensure staff are discarding potentially hazardous foods that have reached the end of their four hour use time. To minimize/eliminate food waste have staff reduce the quantity of product that is brought out of the refrigerator so the supply is consumed prior to the end of the four hour use time

Observed the soda dispensing diffusers and their mounting surfaces of both the drive-through and the dining room soda dispensing systems to be overdue for cleaning. Ensure all staff are reminded of the requirement to clean these surfaces daily.
