Please clean now and as often as necessary to maintain in clean condition

Floor tile grout missing in several areas of kitchen. Please regrout as needed to provide a smooth, easily cleanable surface and to prevent standing water between floor tiles

Brooms and maintenance tools stored improperly rear kitchen. Please store maintenance tools in an orderly manner that facilitates cleaning the area used for storage

Observed water squeeze bottle without label. Working containers of food shall be properly labeled with the common name of the food to avoid misuse. Corrected at time of inspection by properly labeling bottle

Observed soiled dishes stored on the floor in dishwash area. Please provide adequate drainboards for soiled and clean dishes

(Critical) Bagged ham found at improper temperature in top section of speciality prep unit. Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Corrected at time of inspection by discarding bagged ham found at 57°F (stored in thick plastic bag in a plastic pan). The other food items on top and bottom of this unit maintaining at 41°F or below. Pic will begin removing the ham from package and store in a metal insert pan on top of unit. There was also a gap on the top of the unit; new pans were provided to close off the gap

The ventilation system above the hamburger broiler not removing the smoke properly; the smoke is rolling out of the hood canopy and entering the rear kitchen. Per pic the ventilation system is scheduled to be repaired soon. This needs to be repaired asap; please let me know the scheduled date of repairs and send written confirmation. The ann arbor building department should also be aware of this issue. This has been cited in the past and per pic was temporarily repaired; has been malfunctioning for about a month now per pic.

(1) handwash sink draining slowly at handwash sink in women's restroom. Please repair and maintain so handwash sink drains properly. (2) extend drain line over center of floor drain under ice bin near kitchen entrance to prevent water build-up onto floor

Single-service cups at drive thru window stored improperly. Please do not overfill cup dispensers to prevent potential contamination of the lip-contact end when removing cup from dispenser

Provide hook or latch for ice machine lid when dispensing ice so the top cover does not need to be removed each time ice is dispensed

Some of the waste receptacles in rear kitchen need to be emptied more frequently. Please remove waste at adequate frequency

These areas in need of cleaning: interior cabinet under frozen coke machine, floors under prep table near wic and floor/wall juncture under shelving/equipment in some areas. Please clean now and as often as necessary to maintain in clean condition.

(Critical) Observed an open carton of liquid eggs (1/3°Full) and the measuring cup were stored briefly out on the counter to be at a slightly elevated temperature (48f). Corrected by relocating the eggs to the one door prep cooler

(Critical) Observed a large trash can that was left in front of the back kitchen hand washing sink, blocking access. Corrected by relocating this trash can

No probe thermometer is provided at the dish washing sink for staff to verify the wash water minimum temperature (110f). Provide a probe thermometer at the dish washing station so it is convenient for staff to use when washing the dishes

Health Report - 08/12/2013 (3 critical incident(s))

(Critical) Observed an open tray of bait provided under the base cabinet of the dining room beverage station. The manager indicated this bait was likely provided by the contracted pest control company (eradico). Contact this company and have them replace this bait station with one that is covered and tamper-proof

(Critical) Upon arrival, observed an employee placing discard times on several pans of phf's (lettuce, tomatoes). A 1/2 pan of lettuce and tomatoes on the bottom shelf of the burger make line, and a 1/6th pan of tomatoes & 1/3 pan of lettuce at the chicken sandwich make line. These food containers must be marked at the time of preparation with the discard time that is no more than 4 hours

(Critical) The following food contact surfaces in storage were observed unclean: four of the plastic hot holding food pans, one clear plastic 1/2 pan and the inside of a pump spout. Ensure staff are taking enough time to thoroughly clean these items to remove food debris and grease. These items were sent to be re-washed

Spill tray at dining room ketchup and iced tea dispensing area: a gap is present between the trough basin and the counter top. Seal this gap thoroughly with silicone sealant to make cleaning easier

1) the splash guard and the removable drain pan for the blender rinsing station appears they haven't been removed for cleaning in a while due to the food build-up on them. 2) dining room beverage/ketchup dispensing area: observed ketchup on the side-walls and under the lip of the counter top at the station spill tray. Ensure staff are removing all ketchup from this difficult to clean area. These items/areas need daily cleaning.

Observed two, uncovered containers of food (salsa and diced onions/peppers) from breakfast to be stored in the walk in cooler. Corrected by covering these food containers with plastic wrap

1) the metal ice transfer pail is stored upright on a bottom shelf between the wif and wic. 2) the plastic salad containers (bottoms and lids) were observed to be stored upright at the salad station. Keep these containers inverted to prevent debris accumulation

The flexible drain line used to connect the dining room counter top beverage station trough drain to the pvc drain pipe is slumping down and not draining properly. Replace this flexible drain pipe with a rigid pipe and directly connect it to the existing pvc pipe so there is positive slope for proper drainage

The exhaust hood system located above the broiler is not evacuating the smoke properly. When foods are cooked smoke rolls out of the hood canopy into the kitchen. To help aid in the removal of smoke two of the three hood filter sections have been removed. Asap repair this unit to work as designed

(Critical) Observed an employee mark the discard time on an existing label for a pan of sliced tomatoes already in the sandwich make line upon my arrival. Additionally, two back up pans of sliced tomatoes located on a bottom shelf under the make line had labels without any discard times identified. Tomatoes need to be marked with a maximum of a four hour discard time once they are sliced

Observed some residual lettuce on the lettuce chopper. Ensure staff are fully cleaning this piece of equipment after use. A thorough inspection is necessary--recommend using a flash light

Observed some depressions in the floor located behind the floor sink located between the three compartment sink and the hand washing sink. After thoroughly cleaning this area have someone repair the tile/grout/sealant in this area to be smooth and at the same level as the surrounding floor to eliminate the areas that collect water

Observed the food service license to be located in the manager's office. Corrected by relocating to the front counter area where it is visible to your customers

Observed no "non-smoking" signs at the two public entrances to the facility. Provide a non-smoking sign/symbol at your two entrances.

(Critical) 1) at 9:15 am observed an opened, partial package of sliced ham in a metal pan at the breakfast make line to be marked with a discard time of 9 am. Corrected by discarding. It appeared too much ham was removed from the cooler. To minimize waste, have staff bring out a smaller quantity of food that is placed on time control. 2) at 9:15 am observed four containers of sliced tomatoes out on a bottom shelf of the sandwich make line to have discard times of 2 pm. Per the manager the tomatoes were sliced just prior to 9 am today. Corrected by providing a new discard time label indicating a 1 pm discard time

(Critical) Observed the following food contact surfaces to be unclean: the cutting blades and/or push blocks on the tomato, onion and lettuce slicers; the chute area inside the ice making machine; a serrated kitchen knife in a storage drawer; four white plastic dispensing spouts in a storage drawer; the mounting surfaces for the beverage station diffusers/dispensing spouts. Clean (wash with hot soapy water, rinse and sanitize) these areas asap

Observed a significant leak in the drain connection to the dining room beverage station trough drain. Repair as needed to ensure a water tight seal

Observed the following areas to be in need of cleaning now and more frequently in the future, as needed: the floor sinks under the drive through beverage station and under the ice making machine, and the floor under the large ice bin, icee machine and under the leaky drain for the dining room beverage station trough drain

Provide a way to hang your mops with the head at the bottom over the recessed mop drain area. This way the wall and mop handles will not be soiled

(Critical) Observed the time control mark for the chicken fries to still be reading 12:30 pm at around 2:15 pm. One employee indicated new chicken fries were cooked at approximately 1:30 and the discard time was not updated. Corrected by discarding the chicken fries, cooking a new batch and providing a new one hour hold time. Observed a bag of lettuce on a shelf below the sandwich make line without a use-by time. Corrected by providing a use-by time based upon when the lettuce was pulled from the refrigerator. Ensure staff are providing accurate use-by times for potentially hazardous foods that are not being held under proper temperature control

(Critical) 1) observed several areas of the ice transfer pail (on the bottom) to be cracked/damaged. Someone repaired this pail with clear tape. Once a food contact surface like this is damaged it cannot be properly repaired to be smooth and easily cleanable. The tape can trap debris and moisture allowing for bacterial growth. Purchase a new ice pail. 2) observed an interior portion of the ice making machine (white plastic liner) to have a piece missing, thus, exposing the insulation. Water used to make ice is flowing across this un-cleanable surface. Replace the ice machine

(Critical) Observed the shake mix in the top refrigerated hopper of the shake machine to be at 45 to 47f. Corrected by discarding this shake mix

(Critical) Ice machine/ice bin: observed unclean water dripping down onto the ice from the top portion of the ice bin through an unsealed joint between the ice bin top cover and the front of the ice bin. The ice machine was turned off and staff were going to discard the ice, thoroughly clean and sanitize the contaminated areas prior to reassembling the ice bin yet today. The original source of the water was the inverted ice transfer pail. If this ice pail is going to be stored above the ice bin it needs to be on a clean and sanitized surface such as a tray so the moisture is collected and not allowed to seep into the ice bin. Ensure the joints/seams at the top of the ice bin are thoroughly sealed

1) observed the floor located under the drive through soda station ice bin to be in need of cleaning. 2) the floor sink located under the ice machine is in need of cleaning now and more frequently in the future

Repair as needed the shake machine so it maintains the shake mix at 41f or below.

Observed missing/worn tile grout located under the soda system ice bin at the drive through station. After cleaning and drying this area provide a new layer of grout to prevent debris and moisture build-up

The ownership of this establishment recently changed. The owner or their representative must submit a license application and the applicable license fees to this office by the close of business on 8-12-11

Observed the racks of hamburger buns (wrapped in bags) stored adjacent to the back kitchen hand washing sink. The bags are being splattered when staff are washing their hands. Relocate these racks of buns away from the sink to prevent accidental contamination

(Critical) Observed the bag of whipped topping (a new brand from what has been used in the past) to be out at room temperature in a holder at the front counter. Corrected by discarding this whipped topping and placing a replacement bag in a reach-in refrigerator

Observed the door sweeps that were added to the bottom of the exterior customer doors to be missing or coming off of the doors. These door sweeps were attached using a double stick tape. Replace these door sweeps with a heavy duty commercial door sweep that is screwed to the doors to prevent pest entry and eliminate drafts

Contact your solid waste hauling company and have them provide a replacement cover (left side) for the dumpster, as it is currently missing

The back kitchen hand washing station near the broiler and office was out of out of paper towel. Corrected by replacing the paper towel

Observed several kitchen pans observed stacked while still wet. Corrected by separating these items to facilitate quicker and thorough drying

Mop sink area: 1) the concrete floor has several low spots that are accumulating moisture. Re-surface this concrete to slope to the drain. 2) observed missing cove tile and missing/crack cove tile grout. Repair/replace as needed. 3) observed the bottom edge of the frp paneling (located just above the cove tile) to not be sealed to prevent moisture entry. After repairing the tile (#2 above) ensure the joint of the frp and tile is sealed to be water proof

Observed daylight & significant gaps under the two customer entrances (three exterior doors in total). Install door sweeps on the bottom of the outsides of these doors to prevent pest entry. Consider the bristle type door sweep (they seem to last longer than the rubber strip type)

Observed the mounting surfaces for the soda dispensing spouts on both the dining room and the drive through soda stations. The removable spouts and the mounting surfaces on the dispensing machine need to be cleaned daily at closing

1) observed the floor sink located under the ice machine to be over-due for cleaning. 2) observed a significant amount of spilled/leaked oil under the deep fryers. Please thoroughly clean these areas now and more frequently in the future
