(Critical) Observed no chlorine test kit available in the facility. Pic has been using a quat test kit to test the dish machine. Pic states he called ecolab earlier this month for service on dish machine because sanitizer was not registering. Ecolab found machine is functioning properly. During inspection dish machine is found to be functioning properly. Correct by obtaining a proper test kit

On the main make line above the hot holding unit observed two glass shelves. Each shelf is cracked. Small pieces of glass have broken off along the crack posing a contamination hazard. Correct by removing the shelves.

(Critical) Found sanitizing solution concentration above 200 ppm chlorine residual level for wiping cloth solution used for the food contact surfaces of the counter sandwich prep area. Pic person in charge corrected this item during this routine inspection by discarding the solution and refilling the wiping cloth container. Correct concentration verified.

(Critical) Found spray bottle without a label located at the dining area self service area counter. Pic person in charge corrected this item by providing a label on the side of the spray bottle to identify contents by common name of the chemical product

Observed the side service door located at the back area of the store room with the self-closiing device not attached to properly self close the door as required. To correct should schedule repairs to reattach the self closer to properly self close the door

Found open sections of ceiling missing ceiling tiles. To correct should finish off ceiling with appropriate tiles or vented grids as required

Found the outside garbage storage area with a container of standing water and debris. To correct should discard this container to prevent accumulation of standing water and unnecessary items

Observed the interior light fixture located in the walk-in freezer without a shield cover or shatter proof designed light bulb. To correct should provide shielded lighting inside the walk-in freezer as required

(Critical) Automatic warewashing machine not providing sanitizer. Critical corrected: at time of inspection sysco repair person on site to repair warewashing machine. Replaced squeeze tube; chlorine sanitizer now provided at proper concentration.

(Critical) Chicken not cooked off to proper temperature. Raw animal foods shall be cooked to heat all parts of the food to the proper cook off temperature; for poultry 165°F for 15 seconds. Critical corrected: at time of inspection placed chicken breast back on grill to finish cooking to >165°F; found at 128°F just taken off grill

(Critical) Sunbeam counter-top reach-in-cooler not maintaining potentially hazardous food at proper temperature. Critical corrected: at time of inspection discarded milk, whipped cream and pina colada drink mix found at 49°F. Relocated the other non-potentially hazardous drink mixes in unit to the walk-in-freezer until unit repaired

Excess frost build-up evident in sunbeam counter top reach-in-cooler. Please defrost and maintain. Please install proper paper towel dispenser for front counter handwash sink mounted to interior cabinet door as discussed. Currently paper towels on stand-up holder under the cabinet making it difficult to dispense paper towels
