In the dish room at the mop sink observed the atmospheric vacuum breaker (avb) to be leaking water. Correct by repairing the avb so it functions as designed

In the food storeroom observed a light fixture with four unprotected fluorescent lamps. Correct by providing proper shielding. This violation was observed during the last inspection

In the kitchen, observed the large 5°Foot chest freezer with a broken and dented lid. The lid has exposed insulation. Correct by repairing or replacing the lid and or unit. This violation was observed during the last several inspections

Observed the floor in the following areas with an accumulation of food debris and grime: 1. Store room under the shelving 2. Kitchen under the two true upright refrigerators correct by cleaning now and on a more frequent basis.

In the employee restroom off of the kitchen observed six cases of drink syrup. Food may not be stored in toilet rooms correct by relocating the food to an approved area

Observed the floor in the store room under the shelving with an accumulation of food debris. Correct by cleaning now and on a more frequent basis. Improvement has been made since the last inspection.

Observed several floor areas within the facility with an accumulation of food debris and/or grease: 1. At the front counter especially under the cash register area in the main kitchen 2. Under and behind the ice maker 3. Under and behind the deep fryer 4. In the store room under the shelving correct by cleaning now and on a more frequent basis. Improvement, fewer dirty areas, has been made since last inspection. However, please focus on deep cleaning the areas mentioned above and then cleaning them more frequently

In the rear dry storage area observed single service items stored under unshielded sewer lines. Sewer lines need to be shielded to prevent contamination from potential drips. Correct by installing shielding under sewer lines and other potential sources of contamination above single service storage area. This is a repeat violation. Progress has been made on this item. The pic has provided shielding for one main sewer line. However, as discussed during the inspection other sewer lines still need to be shielded.

(Critical) I observed several uncovered employee beverages on a prep table. As discussed, staff beverages must be covered, with a straw. Corrected at the time of inspection. The drinks were thrown out.

Provide chlorine test strips. Use them to check sanitizer levels at the three compartment sink. Sanitizing solutions should be at (50) parts per million.

I found that several refrigerators do not have thermometers inside them. All refrigerated units must have thermometers. Provide thermometers for these units.

I observed jackets and other items stored on top of a reach-in freezer. As discussed, personal items must be stored in a non-food operations area. This is to protect work surfaces and prep areas from contamination.
