(Critical) In the kitchen observed the following phf items not held 135 degrees or greater: 1. On a shelf under the grill , observed three pans of rice with an internal temperature ranging from 70 degrees f to 120 degrees f. The rice was finished cooking at 11am today. Item corrected by rapidly heating the rice to 165 degrees f. 2. At the hot holding units nearest the upright cooler observed two pans of cooked chicken and one pan of cooked steak sitting on top of the holding unit. The food was finished cooking at 1 pm today. The internal temperature of each food item ranges from 110 degrees f to 130 degrees f. Item corrected by rapidly reheating the food to 165 degrees f. Item corrected.
In the dining room at the cash register area observed plastic utensils for customer self service. The utensils are stored jumbled with the mouth contact parts exposed. Correct by storing with the mouth contact parts protected
Observed the can opener blade to have jagged edges. Pic states the blade was recently replaced. It appears the blade is making contact with the gear drive causing damage to the blade. Correct by replacing the blade and or can opener
At the handsink in the kitchen observed the drain pipe to be loose allowing water to leak from the pipe during operation. Correct by repairing the pipe so water does not leak from it
In the basement observed the following items stored under unshielded sewer lines: 1. Two in use ice buckets 2. Napkins 3. Single service cups and papers correct by providing shielding for the sewer line or relocating the items
At the handwashing sink observed no handwash reminder sign. Correct by installing an approved sign
At the ice machine in the basement observed a build up of black mold like residue on the inside of the machine. Correct by cleaning now and on a more frequent basis to prevent the buildup of grime
Observed the door gaskets on the following pieces of equipment broken: 1. On the main make line the reach in portion of both prep top coolers 2. At the end of the main make line the upright refrigerator. 3. In the kitchen the two door upright refrigerator used to store chicken. Correct by replacing these door gaskets.
On the main make line observed a wiping cloth, used to wipe equipment, not stored fully submerged in a chemical sanitizer solution correct by storing wiping cloth fully submerged in a sanitizer solution
At each of the prep top coolers on the main make line observed cutting boards with deep grooves and dark discoloration. The cutting boards ar clean to sight and touch. Correct by resurfacing or replacing the cutting boards
At the outside entry door to the basement area. With the door closed observed a large gap (1/2 inch plus) between the door and the threshold. Correct by repairing/replacing the door so it is tight fitting
Observed the floor drain in the kitchen area with a heavy accumulation of food debris creating a harborage condition for drain flies. Correct by cleaning the drain now and on a more frequent basis to prevent the creation of a harborage condition
No certified managerial employee on staff. Brent hegwood's certification expired on 5/28/2013. Correct by submitting an approved certificate
In the rear food prep area observed a fluorescent light fixture without protective shielding. Correct by installing protective cover

In the dishwashing prep area observed the following ceiling areas in need of repair: 1. The ceiling tile(s) are missing over the mop sink/water heater area. 2. A ceiling tile around a circular vent appears to be water damaged. Correct by repairing or replacing each of these ceiling areas
In basement storage room with the ice machine observed a two tube fluorescent light fixture without light shield. Correct by installing an approved shield
Observed several clean pans stacked wet after dishwashing. Correct by air drying prior to stacking.

There is a large hole in the wall. This must be repaired as soon as possible. Holes in walls can allow pests to get into the restaurant. Maintenance issues must be addressed quickly
The light shield in the rear prep area, needs to be replaced.
(Critical) I found two boxes of tortillas stored on the same shelf as dishwashing chemicals. As discussed at the time of inspection, there must be complete separation between toxic items and food. Storage areas must be inspected frequently to ensure that cleaning products, degreasers and other hazardous chemicals are properly stored as described above. Corrected at the time of inspection. The dishwashing chemicals have been relocated as directed above.

The cold water faucet on this hand sink is not working. This faucet must be repaired right away. Hand sink fixtures must be maintained to facilitate hand washing.
I found an unlabeled large food storage container. Foods and working food containers must be clearly identified by their common name. Corrected at the time of inspection. The container of salt is now labeled.
