(Critical) In the wic observed two deep 1/3rd pans of chicken at 60 degrees f. The chicken was cooked on 2/11/2015 in the evening. The cooler is at the correct ambient temperature and other food items in the wic are at approved temperatures. Pic corrected the item during the inspection by discarding the chicken. Item corrected. ,

(Critical) At the ice machine observed several surfaces with an accumulation of black debris and grime. Also, inside the water circulation reservoir observed about a cup of black sand like debris. During the inspection the machine was placed out of service for cleaning. A followup inspection will be completed for this item. ,

(Critical) In the wic observed two deep 1/3rd pans of chicken at 60 degrees f. The chicken was cooked on 2/11/2015 in the evening. The cooler is at the correct ambient temperature and other food items in the wic are at approved temperatures. The pans are tightly wrapped with plastic wrap and a lid. The chicken was discarded during the inspection. Food items need to be cooled as outlined above. A followup inspection will be completed for this item. ,

(Critical) Observed the following previously cooked food/temperature combinations at the hot holding steam table: 1. Two 1/3 pans of chicken at 110 degrees f. 2. Two 1/6 pans of grape leaves at 100 degrees f. 3. A 1/3 pan of lentil soup at 96 degrees f. Each item had been removed from refrigeration with in the last hour. The items were not properly reheated prior to being placed into hot holding. Pic corrected by heating each item so all parts were 165 degree f of greater. Item corrected.

Chopped lettuce the pic indicates these food items were prepared on 2/11/2014 in the evening. Pic corrected the item during the inspection by placing labeling the food items with an appropriate discard date. Item corrected a followup inspection will be completed for this item. ,

(Critical) In the wic on the main floor observed 6 trays of eggs (about 180 eggs) stored on a shelf above rte foods including a case of cucumbers. Pic corrected during the inspection by relocating the eggs to an approved location. Item corrected

At the clean storage rack in the dishwashing area observed several pans wet stacked. Correct by completely air drying prior to stacking for storage

Observed the back storage/hallway area with an accumulation of grime and food packing debris. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grime and debris

In the storage area by the back door to south university pizza observed the following items in need of repair: 1. Above the reach in freezer observed the ceiling with water damage. No obvious water was observed leaking from the ceiling. Correct by investigating source of leak and repairing the ceiling. 2. In the closet across from the reach in freezer observed missing floor tile. 3. In the closet across from the reach in freezer observed the ceiling drywall to be falling down. Correct #2 and #3 by repairing or replacing the missing floor and ceiling to provide a smooth and easily cleanable surface. Some progress was made on each of these items. However, the condition of the areas appear to have deteriorated again.

Observed a food working prepping a case of iceberg lettuce. The employee did not wash the lettuce prior to chopping the lettuce or placing it into a storage container. Pic corrected by having the employee wash the chopped lettuce

In the basement in several different rooms, observed several unused and/or broken pieces of equipment, old boxes, and other items. This area needs to be sorted and unused items discarded to provide a clutter free area. The amount of items has increased significantly since the rendez-vous cafe closed. Cluttered areas: 1. Can become habitat for pests 2. Prohibit proper cleaning 3. Prevent inspection of storage areas for leaks

In the basement storage area observed fluorescent light fixtures with unprotected bulbs. These fixtures are over bulk food storage bags. Correct by installing approved shielding.

At south university pizza at the reach in portion of the prep top pizza make table observed all three doors with broken and open door seals. Correct by replacing the door seals. This violation was observed during the last routine inspection. Please correct as soon as possible.

Observed improper storage of single service items at the following locations: 1. In the dining room under the soda machine observed three boxes of plastic utensils stored next to the trash can correct by relocating to a clean area 2. In the basement observed several cases of single serve items stored on the floor including cases of cups, cases of pizza boxes, and a case of to go containers correct by storing at least six inches above the floor

Observed the following items in need of repair: 1. Dishwashing sink faucet leaks water when valves are shut off 2. Spray rinse nozzle spring no longer supports the nozzle therefore eliminating the air gap for this water supply. 3. The faucet at the preparation sink is repaired with duct tape and putty. Correct each item by repairing or replacing the item

(Critical) At the rear prep area observed an employee chopping lettuce. The employee was handling the lettuce with his bare hands. Item corrected during inspection by educating the employee about proper food handling. The pic also discussed the situation with the employee
