Carmelized onions in a shallow pan 47f. B. In the wait staff holding heat lamp area: 1. Hollandaise sauce 62f for 2 hours. 2. Salsa for cooked eggs at 67f for 2 hours. Potentially hazardous food: 1. That is to be held cold shall be held at 41f or below. 2. That is to be held hot shall be held at 135f or higher. To correct: a. Move the noted items to the walk in cooler to rapidly cool them down to 41f or below. B. Reheat the noted food and hold at 135f. Partial correction: a,b. Food packages shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Food shall be protected from cross contamination by: (1) except as specified in (1)(c) below, separating raw animal foods during storage, preparation, holding, and display from: (a) raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables,(p) and (b) cooked ready-to-eat food; (p) to correct: sperate raw foods, by storing them down and away from ready to eat foods and store the raw foods by final cook off temperatures. (Critical) Observed the following potential cross contamination: 1. French toast mix made with raw shell eggs stored with and above ready to eat foods in the walk-in cooler. 2. Raw pooled eggs stored with product containing raw shell eggs in the far right top loading reach-in cooler on the line. Food shall be protected from cross contamination by: (1) separating raw animal foods during storage, preparation, holding, and display from: (a) raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables,(p) and (b) cooked ready-to-eat food; (p). To correct: 1. Move the french toast mix down and away from all ready to eat foods and store by cook off temperature. 2. Move the raw pooled eggs away from the raw shell eggs. Corrected: person in charge made all the above changes

(Critical) Observed the following: a. In the top loading reach-in cooler: 1. Cut ham 46f, for an unknown amount of time. 2. Pepper chili 45f, for an unknown amount of time. 3. Cooked onion 46f, for an unknown amount of time. B sitting out at room temperature in the cook line areas: 1. Salsa 80f, for an unknown amount of time. 2. Holidaze sauce 85f, for less than an hour. 3. Pooled eggs 53f, for an unknown amount of time, in an improper ice bath. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct: 1. A. 1,2,3 and b. 1,3 discard the noted food into the garbage. 2. B. 2, move the noted food into an ice bath and place in the cooler to rapidly cool it down to 41f or below. Corrected: person in charge discarded all food that was out for an unknown amount of time and moved the holidaze sauce to an ice bath and into the cooler.

Observed the following: 1. Top loading section of the far right side top loading cooler with an ice build up. 2. Gasket torn on the left door of the left top loading reach-in cooler unit. 3. Wall hanging cooler in the wait staff area had blocked air flow. Note: all food was with in range, but elevated between 41-43f. 4. Walk-in coolers without emergency exit releasers. Equipment shall be maintained in a state of repair and good condition. To correct: 1. Defrost the noted cooler. 2. Repair or replace the noted gasket. 3. Unblock air flow and train staff to not block air flow in the noted cooler. 4. Provide emergency exit handles to the noted coolers. Corrected: 3. Person in charge unblocked air flow in the noted cooler

Observed the ice chute on the coke machine with a pink mold like build up. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. To correct wash, rinse and sanitize the noted ice chute

(Critical) Used a chlorine test kit and found the dishwashing machine not chlorine sanitizing dishes and found the chemical sanitizing bucket empty. Employee corrected by replacing empty sanitizing bucket,primed machine with sanitizer and later verify proper concentration with test kit. Found the sanitizing bucket in the cooking area, by the handsink with a very low concentration of quat sanitizer. Pic corrected by emptying bucket and refilled with fresh sanitizer. ,

(Critical) Found paper towel dispenser empty and paper towels not available. To correct fill paper towel dispenser and fill when empty. This is a repeated priority foundation violation and a risk control plan must be submitted. ,

(Critical) Found the chemical sanitizing dishwashing machine and sanitizing bucket low on chemical sanitizer. To correct retrain staff to use appropriate test kit to verify proper concentration, that is use chlorine test kit to verify dishwashing machine is at least 50 ppm minimum and maximum 200 ppm, and quat sanitizer is at least 150 ppm minimum and maximum 400 ppm. ,

Found 2 opened cases of single use utensils, napkins and containers, stored on the floor in the basement. To correct store utensils on shelf at least 6 inches off the floor.

Found the doors of the walk in coolers missing emergency door releases. To correct re-install emergency door releases for walk in cooler doors.

Found the floor and surrounding areas around the grease trap soiled with accumulation of debris. To correct clean area and keep clean.

(Critical) Found no paper towels dispensing from the automatic paper towel dispenser in the back kitchen area. Pic corrected by providing paper towels and states batteries need to be replaced. Pic was reminded to monitor and ensure paper towels are available and paper towel dispensers are properly functioning at all times.

(Critical) Found time/temperature control for safety (tcs) food, 2 and a half trays of raw shell eggs 67-69°F in the bottom portion of the 2 door prep top cooler. Later, determined eggs were left out as all other items in this cooler were at 41°F. Pic corrected by discarding eggs and was instructed to monitor and ensure eggs are left in cooler and held at 41°F.

Observed employee eating in the dishwashing area. Employee corrected by leaving the dishwashing area and was instructed to eat in the dining area.

Found the tiled floor in the kitchen with worn grout and cracked tiles. Found the wall in the mens public restroom surrounding the soap dispenser and the wall in front of the restroom damaged and uneven. To correct repair floor and walls to be smooth, durable and cleanable.

Found the cold water valve of the faucet in the handsink by the cooking/prep area not functioning properly. To correct repair/replace faucet handle to function properly.

Found the emergency door releases of both walk in coolers in the kitchen missing. To correct repair/replace missing walk in cooler emergency door releases. Found the paper towel dispenser not automatically dispensing paper towels. To correct repair/replace dispenser to function properly. Found peeling paint on the back interior wall of the walk in cooler by the dishwashing area and a hole not completely sealed where refrigeration lines enter the walk in cooler. To correct repair wall to be smooth, durable, and cleanable, and seal walk in cooler wall where refrigeration lines enter.

Pic corrected by providing 7 day datemarks for all noted items, and has been instructed to monitor and ensure employees are datemarking appropriate items.

(Critical) Found hot time/temperature control for safety (tcs) food, onion soup 115°F, corn chowder 125°F, and chili 125°F in the hot holding well. Pic corrected by reheating noted items to 165°F and held items hot on grill top at 135°F. Pic states hot holding well may not have been filled with enough water and added more hot water. Pic has been instructed to monitor and ensure food items are heated properly and held at 135°F and above. Found hot tcs food sausage 110°F in a container on top of an egg crate on the grill. Pic corrected by reheating sausage to 165°F and held sausage on grill without egg crate. Pic has been instructed to monitor and ensure raw sausage is cooked to at least 155°F for 15 seconds and held at 135°F or above. Found cold tcs food sliced sausage 60°F and chorizo sausage 70°F in the bottom of the 2 door prep top cooler. Employee states these items may have been left out during busy times as other items in cooler were at 41°F. Pic corrected by discarding these items and has been instructed to place items back in cooler after each use. Found cold tcs food a gallon of milk 47°F in the 2 door sliding glass cooler in the waitstation. Pic temporarily corrected by discarding milk, and adjusted cooler to a colder setting. Pic has been instructed to monitor and ensure cold tcs food is held at 41°F or lower.

Found the glass sliding door cooler in the wait station not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F. Found no emergency door release on the walk in cooler doors. To correct provide emergency door release on walk in cooler. Pic states she is looking for door release parts.

Found no hot water flowing from the faucet in the kitchen cooking/prep area. Pic states a new faucet and handsink have been ordered and it should be installed within a week. Pic called contractor who verified on phone handsink and faucet was ordered and scheduled to be installed next week. To correct provide and install working faucet in handsink that provides hot and cold water for proper handwashing. Note: another functioning handsink in the back area, was available for employees to wash their hands. Found the hose bibb vacuum breaker that was attached to the basement utility sink on the side of the sink not attached to hose bibb. To correct reinstall hose bibb vacuum breaker to faucet.

Observed employee cleaning slicer and asked how slicer is cleaned. Employee states slicer is broken down and wiped down with sanitizer water. Temporarily corrected by educating employee that slicer is broken down, washed with soapy water, rinsed with clean water, and wiped down with sanitizer. To correct monitor and ensure employees are cleaning slicer properly and re-train as needed.

(Critical) Found ready to eat (rte), food, covered containers of tuna and coleslaw stored next to raw meats (raw chicken, beef, and hamburger patties) in the bottom portion of the 3 door prep top cooler. Employee corrected by moving rte food to a location in another compartment away from raw foods. Pic has been instructed to monitor and ensure rte food is not stored next to raw foods. Note: this is not a repeated cross contamination violation.

(Critical) Observed cook handle ready to eat (rte) food, lettuce with bare hands. Cook corrected by discarding lettuce touched with bare hands, placed gloves on hands, and then handled rte food with gloved hands. Pic has been instructed to monitor and retrain employees when handling rte food, that is to handle rte food with gloved hands or utensils.

Found wiping cloth left on the prep top cooler cutting board. To correct store wiping cloth submerged in sanitizing bucket.

Observed employee on the cooking line not wearing hat/hair restraint. To correct provide hat/hair restraint and remind employees to wear them.

Found some accumulation in the pop machine spout and diffuser in the waitstation. To correct follow manufacturers directions and clean pop machine spout and diffuser on a more frequent schedule.

Found the fan covers in the walk in cooler soiled with dusty build up. Found the shelf under the dishwashing counter soiled with accumulation. To correct clean fan covers and shelf and clean on a more frequent schedule.

Found the grout of the tiled floor in the cooking area worn and missing. Found the wall panel behind the cooking area handsink water damaged and no longer smooth. To correct repair/replace tile grout and panel to be durable, smooth, and easy to clean.

Observed cook in the cooking area not wearing hat/hair restraint. Cook corrected by wearing hat/hair restraint. Pic was instructed to remind employees working in food prep areas to wear hat/hair restraints.

Found the walk in cooler (in cooking area) fan cover soiled with dusty build up. Found the door gaskets and surface sealing areas of cooler doors soiled with accumulation and build up. Found the shelves holding clean plates soiled with accumulation of debris. To correct clean noted areas and clean on a schedule to preclude accumulation.
