Yogurt machines not being cleaned at required frequency. Surfaces of utensils and equipment contacting potentially hazardous food may be cleaned less frequently than every 4 hours if contents maintained at 41°F or below and the container is cleaned and sanitized properly when empty. After discussion with manager appears there was some confusion on cleaning frequency of yogurt machines. Please refer to e-mail sent from orange leaf headquarters july 16, 2014 addressing cleaning fequency of yogurt machines. The yogurt machines shall be cleaned and sanitized every 7 days. Any product removed from machine regardless of date marking will need to be discarded. Only new product can be placed into a cleaned and sanitized yogurt machine. Per manager will begin cleaning and sanitizing half of the yogurt machines on saturday and the other half on sunday. There are a total of 8 machines (16 yogurt choices)

Once removed from the original container a working container of chemical shall be clearly marked with the contents to avoid misuse. Corrected at time of inspection by properly labeling all chemical spray bottles with name of contents

Clean dishes stored improperly on shelving unit rear kitchen. Please invert all clean dishes to prevent potential airborne contamination

Sanitizer concentration high in wet wiping cloth solution front counter. Please provide and use quat test kit to monitor sanitizer concentration throughout the day to assure proper sanitizer concentration. Pic was preparing sanitizer solution using disinfectant instructions; will change procedure, will provide proper quat test kit and inform all staff. Sanitizer solution not set up at manual warewashing operation at time of this inspection

Please provide covered waste receptacle in women's restroom for proper disposal of sanitary products

Observed some canned food items and boxed food items stored on the floor. Please provide adequate shelving to store all food items a minimum 6 inches above floor
