(Critical) On follow up inspection found a paper towel dispenser, filled with paper towel installed in the kitchen prep/service area handsink, and paper towels in the paper towel dispenser in the dishwashing area handsink. Pic states a new paper towel dispenser was installed and they try to keep paper towel dispensers stocked. Violation corrected.

(Critical) On follow up inspection found a t-valve/sidekick installed on faucet and chemical tower water supply line connected to t-valve/sidekick, thus no back pressure of system pressure. Violation corrected.

(Critical) On follow up inspection found a quat test kit available to staff to check solutions. Pic states quat test was ordered and now available to staff. Violation corrected.

(Critical) On follow up inspection found no tcs food left out on prep tables in the back kitchen area, no tcs food left out on prep table behind front counter, but did observed an employee mixing coleslaw on prep tabe in front of smokers. This violation was corrected on routine inspection, and reverified on follow up. On follow up inspection found tcs food, whipped butter cups 41°F in the 1 door reach in cooler by the front counter. Pic states they are leaving whipped butter in cooler. Violation corrected.

(Critical) Found a hose bibb vacuum breaker attached to a mopsink faucet with an atmospheric vacuum breaker, that was connected to chemical tower that is left on at all times, as no back pressure of system pressure is allowed. To correct remove hose bibb vacuum breaker, attach a t-valve/sidekick to faucet and attach water supply line from chemical tower to sidekick.

(Critical) Found time/temperature control for safety (tcs) food, coleslaw 57°F, chicken 60°F, and an opened gallon of milk 57°F on the prep table in the back kitchen area. Found tcs food, an unopened bag of shredded cabbage 55°F on top of an aluminum take out container, on top of a prep table behind front counter. Employees state someone probably brought out tcs food to work on (about an hour), had to leave and left tcs food on prep tables. Employee corrected by placing tcs food in the walk in cooler for rapid cooling. Found tcs food, a container of whipped butter cups 76°F on the table behind the front counter. Pic corrected by discarding butter and was instructed to either provide written information that whipped butter can be left out at room temperature, or store in cooler at 41°F. Pic has been instructed to monitor and ensure tcs food is held at 41°F, and to minimize time tcs food is left out.

(Critical) Found quat test kit water stained with multiple colors and no longer accurate. To correct provide a quat test kit.

(Critical) Found no paper towel dispenser in the kitchen prep/service area handsink and found holes in wall where original paper towel dispenser was mounted. Found no paper towels in the paper towel dispenser in the dishwashing area handsink. To correct provide working paper towel dispensers and stock with paper towels in all handsinks. Pic has been instructed to monitor and ensure paper towels are available in all handsinks at all times.

Found no thermometer in the 2 door prep top cooler and 1 door worktop cooler. To correct provide internal hanging thermometer in coolers.

Found the floor, under equipment, edges and corners in the entire facility soiled with accumulation and build up. To correct thoroughly clean floor and clean on a more frequent schedule.

Found facility had no manager certified in food safety. To correct provide manager certification.

Found numerous single use cups in food storage bins used to dispense food. To correct replace single use cups with commercial grade scoops, and store with handles up.

Found the hood filters in cooking hoods soiled with grease and dust build up. Found ashes building up on the sides of cooking equipment. Found the door handles of the 2 door reach in hot holding unit soiled with greasy build up. To correct clean noted areas and keep clean.

Found lights in the cooking hood not turned on and 1 light not working. To correct repair/replace light, and turn lights on when working under cooking hood.

Found growler filling tube left on the bottom of sink in the front counter area by beer taps. Pic corrected by cleaning filling tube, and storing growler filling tube in a clean container.

Found dirty plates of food on top of garbage can, stored next to a bulk container of whole potatoes on the cooking line. Found a bulk container of sugar without lid. To correct provide covers for food containers and protect food from contamination.

Found the dishwashing hot water rinse thermometer not working. To correct repair/replace hot water rinse thermometer.

(Critical) Found hot time/temperature control for safety (tcs) food, chicken 122 to 130°F in the hot holding unit. Pic corrected by reheating chicken to 165°F for 15 seconds and raised hot holding unit to a hotter setting. Found cold time/temperature control for safety (tcs) food, portioned cups of honey butter 67°F in a container on the front counter. Pic corrected by discarding honey butter (whipped butter with seasoning) and was instructed to keep honey butter in cooler at 41°F. ,

(Critical) Used a quat test kit and found the sanitizing solution flowing from the quat dispenser and in sanitizing buckets below manufacturer's recommended range of 150 to 400 ppm (note this is a multi range quat sanitizer and its exact range was not available at time of inspection). To correct call for repair, and use quat test kit to prepare sanitizinng solutions, until dispenser is repaired. ,

Found insulated containers used for transporting hot food soiled with spills. To correct clean containers and clean after each use

Found a half filled fryer which was soiled with accumulation in the back kitchen hood. Pic states they are not using fryer at this time. To correct determine if fryer is needed and either remove fryer of place fryer in storage.

Found growler filling tube in the bottom of handsink. To correct properly clean growler filling tube and store on a clean surface when in use. Found a pair of tongs used to handle cooked food stored on the door handles of smoker. To correct store tongs in a clean and protected location.

Found the door gasket on the hot holding unit torn with missing corner. To correct repair/replace door gasket. Found smoker not in kitchen not working. To correct repair/replace smoker.

Found an opened package of carry out boxes, case of paper towels stored on the back kitchen floor. To correct store single use items on shelf at least 6 inches off the floor.

Observed employee with beard working in the cooking/prep area not wearing beard net. To correct provide beard net and remind employees with beard to wear them.

Found clean cups stored inverted but under the beer tap drain line. To correct relocate cups to a location away from drain line.
