Bread stored in lined wicker baskets touching sides of basket front kitchen. Please provide adequate paper liners so that bread does not contact sides of wicker basket

Some clutter observed in the rear storage room and office. Please sort through and get rid of any items not used in the operation and maintenance of facility. This will make it easier to keep clean and prevent potential harborage conditions for pests

Some refrigeration door gaskets on prep unit front kitchen in poor repair. Please replace to provide good seal and so smooth and easily cleanable

(1) please re-establish proper air gap for soup holding tank drain line; existing air gap is less than one inch above the flood rim of the receiving receptacle. The air gap shall be twice the pipe diameter above the flood rim of the drain (appears to be a two inch line). (2) observed small leak under the wash compartment of the 3-comp sink. Please repair and maintain in good repair

Observed ice transfer bucket stored improperly on top of ice machine. Please invert when not in use to protect against potential airborne contamination and to properly drain. Clean tray is available on top of ice machine for storage

Quaternary ammonium sanitizer concentration low in wet wiping cloth solution front counter. Please monitor sanitizer concentration throughout the day with your quat test kit and change as needed

(1) holding tank with hatco heater not functioning properly. Please repair and maintain in good repair. Holding tank is used to maintain back-up soups hot; currently using one of the rethermalizers to maintain back-up soups hot. (2) the insert pans do not fit tightly on top of the prep unit front counter causing some gaps between pans. Please provide proper insert pans to eliminate any gaps to assist in providing proper temperatures in unit. (2) refigeration and freezer door gaskets in rear kitchen in poor repair. Please replace to provide good seal and so smooth and easily cleanable

Interior top section of ice machine rear kitchen not being cleaned at an adequate frequency to preclude accumulation of mold. Please clean now and as often as necessary to maintain in clean condition; please place on regular cleaning schedule

Observed wet wiping cloths stored in dry pans under steam table in front counter area. All wet wiping cloths must be stored completely submerged in proper sanitizer solution in between uses to prevent the accumulation of bacteria on wiping cloth.

Observed bag of cooked rte chicken strips and 2 packages of rte turkey thawing on shelf in back kitchen area (internal food temps of chicken and turkey were 28-41f). All phf must be thawed under refrigeration or under cool running water to prevent the accumulation of bacteria in food product.

Observed exterior door handles on true upright cooler in back kitchen area with an accumulation of sticky food debris. To correct, please clean now and on a more frequent schedule

Observed plastic tableware (spoons, knives and forks) and jumbled together in utensil racks on counter in front prep area. Also, observed tongs and serving spoons all mixed together in utensil bin on rack in dishwashing area. All tableware and utensils must be organized so the handles are all facing the same direction and food contact portion of utensil is protected from potential sources of contamination (like employee hands).

Observed couple of split/broken door gaskets on prep unit cooler in front prep area. To correct, please replace all damaged gaskets as soon as possible

Observed floor drain under pop machine in dining area with a heavy accumulation of slime mold and debris. To correct, please clean now and on a more frequent schedule
