Observed three igloo 5 gallon water service coolers stored, upright with lids removed, on top of a storage cabinet. Also, observed a large stock pot sitting upright on an adjacent shelf. Between uses, these items should be stored either covered or inverted to prevent the accumulation of dust inside and debris inside the cooler

During the rinse cycle, the water pressure gauge for the dish machine final rinse is registering 0 psi. It appears the gauge may be faulty because the dishmachine is providing proper sanitizing as shown by heat sensitive labels. Correct by calling maintenance contractor to inspect and repair the dishmachine

Gas range: observed the food spill trays for both cook tops to be lined in aluminum foil that have a significant amount of food residues on them. Discard this foil and provide a replacement layer of foil if desired

(Critical) Observed three spray bottles on the kitchen stainless steel counter all labeled comet commercial cleaner with bleach. One of the chemicals was clear and the other two were blue. The clear liquid had bleach in it and the two blue chemicals did not. Corrected by removing these three chemical bottles from the kitchen and providing proper labels as to their contents

Observed the soap container for the dish washing machine to be empty. Corrected by installing a new container of dish washing machine soap

Observed some aluminum foil pans and pan covers (located in opened gfs boxes on top of a tall cabinet in the kitchen, and some food containers (plastic and aluminum foil) on a kitchen counter to be stored upright/uncovered. In the future, maintain these items inverted or covered/bagged to prevent misc. Contamination of the food contact surfaces

Observed the receiving floor drain for the dish washing machine to not be accepting all of the waste-water from the dish machine when the unit was turned off and the drain valved was opened. It should be noted that no water overflowed during the normal cycling of the dish washing machine

Observed a wet mop head to be resting on the floor in the dish washing room. The provided hanger (rubber device that pinches the handle) is no longer working as designed. Provide another way to hang the mop head (consider using a nail to hang the mop handle after drilling a hole in the handle). Mops that are hung up are able to dry more quickly

Observed the light bulb in the 2 door true refrigerator to not have a shatter-proof coating. Replace bulb with one that is shatter-proof

The water pressure gauge for the dish machine final rinse is only registering 9 psi. Adjust to provide 15 to 25 psi

Observed a small amount of mouse feces located on some dry storage shelves (wooden shelving unit) in the storage room up the 4 stairs off of the back of the kitchen. Clean asap

(Critical) Two door upright refrigerator: observed hamburger buns inside one bag to be moldy; also, observed the spouts of five squeeze bottles (containing ranch dressing, mayonnaise, and mustard) to have mold growth on the spouts and around the thread on spout top. The bag of hamburger buns were discarded and the squeeze bottles were placed on a kitchen counter for disposal of the contents and washing of the bottles

Observed two uncovered trash cans in the kitchen dish washing room to have garbage still in them (+/- 1/3┬░Full). Please either remove all garbage from the kitchen facility at the end of the day or provide a tight fitting, pest proof cover to keep odors down and to prevent the attraction of pests

(Critical) Observed an open quart of 1/2 & 1/2 in the two door upright refrigerator without a last date of use applied when the container was opened. In the future, please date mark your ready to eat, potentially hazardous foods once they are opened. Add six days to the day of opening and write the month and day of month on the container (ex: 1/2 & 1/2 opened on sunday february 13th would have a last date of use of saturday feb. 19th). Another option would be to purchase individual coffee cream that does not require refrigeration. Corrected by discarding the quart of 1/2 & 1/2.

Observed mouse droppings on the wooden shelving unit in the dry storage room. Thoroughly clean and sanitize these surfaces and monitor this area for additional pest activity. If additional mouse feces are detected set out mouse traps to eliminate these pests and try to locate and seal off openings that can allow mice to gain access to this area

Observed the left hand cover on the dumpster to be left open. Please keep the dumpster covers closed to keep out pests

The exposed light bulb in the two door, upright cooler is not provided with a bulb guard or a shatter-proof coating. Remove current bulb and replace it with one that has a shatter-proof coating to prevent food/equipment contamination should the bulb break

Please post your current food service license (pink in color) at the entrance to your kitchen. The license was observed on a clip board in a kitchen drawer labeled "misc. " adjacent to the three compartment sink. Corrected by posting the license on the wall during the inspection

Observed dead insects, food crumbs and misc. Debris on the floors in the kitchen / dish washing room in out of the way areas (such as the toe kick areas of the base cabinets, the floor under the rolling carts for cup storage, in the base cabinets where the sink drains are located, and under shelving/tables in the dish washing room). Please clean these areas to eliminate this debris

Observed mold growth inside the two door upright cooler (on some shelves, behind the grates for the circulation fans, and on two jars of salad dressing--these partially filled containers were discarded). Please thoroughly clean and sanitize the interior of this cooler and consider providing a container of baking soda to help control moisture

The microwave oven (manufactured to be a wall unit) on the counter in the kitchen is still not elevated on 4" high legs (minimum) to facilitate cleaning under the unit. Please provide legs as described above

Observed some spilled food that has accumulated on the two range top spill trays. Please thoroughly clean these trays and maintain them clean. Consider lining them with a layer of aluminum foil to make clean-up easy

This licensed facility does not currently have a certified food safety manager. Documentation was provided during the inspection indicating a member has registered for the serv-safe exam on sept. 22, 2010 in livonia. Once this person's certification is received please forward a copy to me
