(Critical) Observed the following food contact surfaces to be soiled: the potato dicer by the 2 comp sink was soiled with a build up of food debris, a scoop along with miscellaneous utensils in a small bucket by the main prep table in the kitchen, some bowls in a tub on the cleaned dishware shelve by the dishwashing station had food debris still on them. Food contact surfaces shall be kept clean to the site and touch. Corrected by person in charge taking these items and sending them to be re cleaned through the dishwashing machine. Prior to placing cleaned items into storage for reuse, they must be inspected to ensure they are clean. Pic will retrain staff to ensure cleaned items are thoroughly inspected prior to placing in storage. Items were reinspected prior to completing inspection and were found to be clean to the site and touch. Corrected

Observed the non food contact surfaces of the hobart mixer in the main kitchen to be soiled and in need of cleaning. The underside and the outside perimeter had a build up of food residue/splash debris. Observed the non food contact surfaces of the cocktail mixer at the bar to be soiled and in need of cleaning. The underside of the mixer subject to splash debris was soiled with food residue. To correct, please clean these pieces of equipment now and place on a more frequent cleaning schedule

Observed the ground around the grease dumpster to be soiled with a build up of grease and various debris. The ground around the dumpsters shall be kept clean to avoid attracting unwanted rodents and pests. To correct, please clean this area now and place on a more frequent cleaning schedule

Asked employee how slicer is cleaned and employee stated slicer is broken down, moveable parts are taken to dishwashing machine, and slicer base is washed with soap and rinsed with sanitizer. Corrected by educating employee that slicer base must be washed with soapy water, rinsed with clean water, and wiped with a sanitizing solution. Pic has been instructed to monitor and ensure slicer base is clean properly, that is washed, rinsed and sanitized.

Found 2 large garbage bags used to transport vegetables to facility. To correct discontinue using garbage bags to transport food, and use food grade bags or containers. Found single use portion cups (some with cracks) in food containers through out facility used to dispense food. To correct discontinue using portion cups as they are not durable and cleanable.

Found gaps between the handsinks in facility and the dishwashing counter and wall. To correct eliminate gaps by sealing handsink and dishwashing counter to wall.

Found the surfaces of the blender soiled with splash and accumulation. To correct clean blender and clean on a more frequent schedule.

Found single use utensils (knives) stored in a container mixed and jumbled on a shelf in the drive thru area. To correct store knives with handles in an upright position.

Found the door gaskets of the 2 door prep top cooler in the drive thru window trailer cracked and no longer sealing properly. Found the robot processer on the kitchen prep table with a large hole on the lid. Found the quat sanitizer dispenser in the 3 compartment sink area broken and staff manually mixing sanitizer. Found a broken and missing hood filter above the cooking hood above the fryers. To correct repair/replace noted equipment and keep equipment in good working condition.

Found the spouts of the diffuser of the pop gun on the end of the bar soiled with light accumulation. To correct follow manufacturer's directions and clean pop gun.

Found no thermometers in the cooking line coolers. To correct provide internal hanging thermometers in all coolers.

(Critical) Found time/temperature control for safety (tcs) food, several large containers of bechamel sauce 46°F to 50°F, in the walk in cooler, that was made the previous day. Pic corrected by discarding bechamel sauce, and has been instructed to monitor and verify that facility is properly cooling items, that is cool from 135°F to 70°F in 2 hours, and then from 70°F to 41°F in 4 hours. ,

(Critical) Found the faucet spout of the dipper well in the bar below the flood rim of dipper well. To correct provide an air gap of at least 1 inch from the faucet spout and rim of the dipper well. ,

Found a wood wicker type basket used to hold apples for sale in the mobile trailer drive thru. Found several wood spoons stored in utensil holder on the clean utensil storage shelf of the main kitchen. To correct replace with utensils that are durable, smooth, and cleanable.

Found a brand new ice machine installed outside of building in a covered structure which is not protected from possible weather and human contamination. To correct protect ice from this ice machine from contamination of various sources.

Found several food containers wet and stored on clean utensil shelf. To correct allow food containers to air dry, prior to stacking and storing.

Found containers of food in the hot holding unit covered with cotton towels. To correct discontinue covering foods with cotton towels and replace with either covers that are durable, smooth and cleanable, or use single use utensil such as plastic wrap.

Found the cooking hood missing spacer or correctly sized hood filters. Found the frying baskets of the fryers with damaged wire sides and loose. To correct repair/replace missing filter spacer/hood filter and fryer baskets.

Found the floor, under equipment, edges and corners in the mobile trailer drive thru window area soiled with accumulation of debris. Found the ceiling of the cooking area and back kitchen area soiled with exhaust and dusty build up. To correct carefully clean noted areas and keep clean.

(Critical) Found 2 large covered containers of chili 49 to 56°F in the walk in cooler, which were cooled the previous day but facility added more base to thicken chili to desired texture and consistency, then covered chili in large containers without properly cooling chili. Employee stated to pic base was added less than 2 hours ago, and pic corrected by having employee divide chili into smaller containers and stir with ice wands. Pic has been instructed to monitor and ensure tcs food is cooled properly when products are added that raise the temperature of tcs food.

Found bread stored on counter top leaning against wall. To correct store bread in a more protected manner away from wall. Observed employee grinding raw beef on the mixer with grinding attachment next to ready to eat food, stacked trays of bread. Pic corrected by moving bread away from mixer and grinding attachment.

Found a pair of tongs stored in sanitizing water on the cooking line. Employee corrected by placing tongs in a container of hot water on the grill. Found the handle of ice scoop laying on ice in the bar ice bin. To correct store ice scoop either in a holder/container or in ice bin with handle up.

Found the left bar cooler not in use and not working. Found the cooler drawer door handles on the cooking line with missing and broken trim/hand inserts. To correct repair/replace cooler to hold tcs food at 41°F, and cooler door handle/hand inserts.

Found the exterior of cleaned food containers with partial labels and sticky adhesive. To correct re-wash containers and remove labels before placing in dishwashing machine.

Found the door gaskets and bottoms of most coolers and freezers in facility soiled with accumulation. Found the shelves in the storage room and walk in coolers soiled with build up. To correct clean as soon as possible, and clean on a more frequent schedule.

Found a covered container of prepared cheese cooling in the walk in cooler. To correct divide cheese into uncovered, shallow pans and rapidly cool cheese. Found several trays of para cooked/fried sweet potatoes and potatoes left cooling on rack by dessert/salad prep after 2 hours have passed. Found numerous coolers- 1 door under the counter cooler, 4 drawer prep top cooler, right 6 drawer prep top cooler, left 6 drawer prep top cooler, and the 4 drawer, 1 door prep top cooler not holding tcs food at 41°F or lower. To correct repair/replace all coolers to hold tcs food at 41°F or lower. Found several wood planks used to cook fish with deep cracks. To correct replace wood planks that have deep cracks which are hard to clean. Found the can opener blade with nick and worn surface, found caulk hanging from edges of cooking hood, and found ice bucket with chipped bottom. To correct replace can opener blade and ice bucket, and re-caulk hood edges.

Found soiled pans and buckets stored in front of the handsink between the salad/dessert prep area and the dishwashing area. To correct re-locate items in front of handsink and keep accessible at all times.

(Critical) Found time/temperature control for safety (tcs) food, a container of facility made russian dressing 51°F, and chili 47°F in double pans on the top portion of the 4 drawer prep top cooler on the cooking line. Found tcs food, coleslaw and facility made sauce 49°F in the top portion, and fish and chicken 49°F in the bottom portion of the 6 drawer prep top cooler on the cooking line. Found tcs food. Cream cheese 48°F and chicken salad 50°F in the top portion, and half and half, mozzarella cheese, cream cheese and chicken salad 51°F in the bottom of the 2 door prep top cooler in the drive thru service area. Pic corrected by discarding all tcs food in all of the above coolers and has been instructed not to use these coolers, until repaired/replaced to hold tcs food at 41°F. Found tcs food, a container of pulled chicken 120°F in the 2 door sectional hot holding, and a container of pulled chicken 120°F in the 4 drawer hot holding unit on the cooking line. Pic corrected by heating chicken to 165°F for 15 seconds, and has been instructed to ensure that after tcs food is cooked to hot hold chicken at 135°F or above. ,

Employee corrected by discarding bread, hold bread with utensil, cut bread, and place sliced bread into bread basket with tongs. Observed employee cut plastic wrapped cucumber on cutting board, and use bare hands to place sliced cucumber on glass. Corrected by instructing employee to use utensils when handling garnishes on drinks. Management has been instructed to monitor and ensure employees are handling ready to eat foods with utensils (not bare hands). , observed employee use utensil to hold ready to eat food, bread, cut bread with knife, and then use bare hands to place bread in to bread basket. On follow up inspection observed employee use deli tissue to hold bread, cut bread, and use another deli tissue to place bread in basket. A sign was posted above bread station instructing staff to use utensils to handle bread (no bare hands). Pic states staff has been reminded on the proper handling of bread. Violation corrected. On follow up inspection did not observe employee handle cucumber, but employee states they are to use gloves when cutting fruits and vegetables for drinks, and to use gloves when handling all garnishes on drinks. Violation corrected.
