(Critical) On follow up inspection found tcs food, chicken 41°F, and rice 41°F in the top portion of the 1 door prep top cooler. Employee states chicken is cooled and placed in cooler. Violation corrected. On follow up inspection found no falafel on top of grill. Employee states falafel is made to order and not left on top of grill. Violation corrected.

(Critical) On follow up inspection found sanitizer in the 3 compartment sink, and employee states he scrubs cutting board with bleach, uses soap to clean board, rinses cutting board with clean water, and applies sanitizer on cutting board. This violation was corrected on routine inspection, and reverified on follow up.

(Critical) Observed employee clean cutting board in the 3 compartment sink and found no ready to use sanitizer in the dishwashing area. Employee states he washed cutting board with soap and degreaser. Rinsed cutting board with water, and placed back on the prep top cooler without sanitizing cutting board. Employee corrected by manually mixing sanitizer and wiped down cutting board with sanitizer. ,

(Critical) Found time/temperature control for safety (tcs) food, a covered container of chicken 70°F, and a covered container of rice 60°F on the cutting board of the 1 door prep top cooler. Found tcs food a pan of falafel 70°F on top of the grill. Pic states these items were made about an hour and half ago. Pic temporarily corrected by removing lids of chicken and rice and placed in the walk in cooler to rapidly cool and discarded falafel. To permanently correct determine if tcs food are to be held hot 135°F and above, or cold (cool properly) and then hold at 41°F. Pic has been reminded tcs food may not be left out at room temperature. ,

Found the exterior surfaces of the soup well soiled with spills and build up. Found the bottom, door gaskets and interior and exterior surfaces of coolers soiled with accumulation and build up. Found the interior surfaces of microwaves soiled with accumulation of debris. Found equipment shelves on the cooking line soiled with heavy build up and accumulation. Found the door handle and surrounding areas of the walk in cooler door soiled with accumulation. To correct clean all noted areas and clean on a more frequent schedule

Found gaps along the bottom and sides of the back door. To correct eliminated gaps along the back door.

Found the lights above the dishwashing area and soup well area not working. To correct repair/replace lights.

Found the walls and floor in the cooking and prep areas soiled with splash, accumulation and build up. To correct thoroughly clean walls and floors.

(Critical) Used a chlorine test kit and found the dishwashing machine not chlorine sanitizing dishes. To correct repair dishwashing machine and use the 3 compartment sink to wash, rinse, and sanitize dishes, until the dishwashing machine is repaired. ,

(Critical) Found ready to eat (rte), time/temperature control for safety food, numerous containers of soup, baba ghannuj, grape leaves, hummus, tabbuleh, without old datemarks in coolers and the walk in cooler. Pic states all food was made either today or yesterday, and corrected by providing 7 day ( day made + 6 days) datemarks. Pic has been instructed to monitor and ensure employees are datemarking appropriate items. ,

Found various baked items and pastries available for sale without ingredients and food allergen information available. To correct have ingredients available upon customer request.

Found the interior of coolers (doors, lids, shelves and bottoms) and exterior door handles and door gaskets soiled. Found the shelves and countertops in the kitchen soiled with accumulation and build up. Found the interior of microwave soiled with accumulation of debris. Found the side of the cooking hood soiled with heavy carbon build up. Found the exterior sides of numerous containers soiled with old stickers and labels. To correct clean noted areas and clean on a more frequent schedule.

Found the lights in the back kitchen dishwashing area and prep area not working. Found 1 light in the cooking hood not working. To correct repair/replace lights.

Found tubs filled with water under the evaporator coil in the walk in cooler. To correct repair evaporator coil to drain properly.

Found a household crock pot used to hot hold soup. To correct replace crock pot with a commercial grade hot holding unit.

Observed employee working in the cooking and prep area not wearing hat/hair restraint. To correct provide hat/hair restraint and remind employees to wear them.

Found a covered container of rice cooling in the 1 door prep top cooler. Pic corrected by removing lid on rice and placed rice in the walk in cooler to rapidly cool. Found a covered container of chicken on the bar stool by the 1 door prep top cooler. Pic states chicken was just made and corrected by placing chicken uncovered in the walk in cooler to rapidly cool. Pic has been instructed to monitor and ensure employees are properly cooling tcs food.

Observed flies in the front counter area and the back kitchen area. To correct keep back door closed, keep facility clean and dry, and continue pest control measures.

Found the floor under equipment, edges and corners in the back kitchen area soiled with accumulation and build up. Found the wall on the side of the cooking line soiled with splash and accumulation. To correct thoroughly clean floors and wall and keep clean.

Found no manager certified in food safety. To correct provide a manager certified in food safety.

Found a stack of coffee filters stored on top of coffee machine. To correct store coffee filters in a protected manner, such as in a covered box.

Found the back door in the kitchen propped open. Employee corrected by closing door.

Observed the facility has no employee in the facility during a majority of business hours who is currently certified under a personnel certification program accredited by ansi, utilizing the conference on food protection standards. Correct by providing proof of certification for a member of staff who is regularly engaged in food preparation on a full time basis. Item shall be corrected within 90 days.

(Critical) The handsink on the main cook line has no soap dispenser. Employees are getting soap from a dispenser located on the opposite wall of the kitchen. This soap dispenser is located over food preparation equipment. Correct by providing soap at the handwashing sink. ,

(Critical) At the front counter area observed dishes in the handsink. Handsinks are for handwashing only. Item corrected by removing the dishes from the handsink and educating the employees about proper handsink use. Item corrected

(Critical) 1. At the handsink located in the dishwashing area observed no papertowel. Pic corrected by providing a roll of papertowel on the shelf above the sink. 2. At the handsink located on the main cook line observed no papertowel. Correct by installing a papertowel dispenser at the handsink. ,

(Critical) At the mechanical dishwashing machine observed the residual chlorine concentration of the final rinse at 0 ppm. The sanitizer reservoir is full and the tubing is properly routed. Pic primed the machine and made adjustments. After a few cycles the machine provided the proper final rinse sanitizer concentration. Item corrected.

(Critical) Water temperature at each handsink was observed to be 70 degrees f. Pic indicates the water heater is not working. Pic has called for service. As a temporary correction, employees shall: 1. Wash hands at the handwashing sink with soap and water 2. Then use alcohol based hand sanitizer continue until hot water is restored. On the main cook line at the handsink faucet observed the cold water handle missing. Correct by replacing the handle. Proper water temperature is important to thorough handwashing. This handle has been broken for over a year. This shall be repaired within 10 days

At the main cook line observed: 1. One knife stored in the gap between two prep top units. 2. Two knives stored in the gap between the wall and a prep top unit. The blades of each knife were touching the sides of the equipment. The sides of the equipment have an accumulation of food debris and grease. This material may contaminate the knives. Knives should be stored on cleaned and sanitized surfaces. Item corrected during inspection by obtaining clean knives and storing them in an approved location in the kitchen.
