These are repeat items. Correct each item as indicated

At the wic observed no ambient air thermometer. Correct by providing an approved thermometer

(Critical) Several foods that were cooked/held hot last night were improperly cooled by not spreading them out into a thin layer and by covering them. Ensure your bulk, hot foods to be cooled down are placed in shallow pans and left uncovered on a top shelf to rapidly cool down

(Critical) Observed two ready to eat food items made/re-heated the previous day [beef stew (small walk in cooler) and chicken breasts in broth (reach-in, one door upright refrigerator)] to be at 45f and 52f, respectively. Corrected by discarding these foods.

The one door upright refrigerator near the dish washing machine was operating at a slightly elevated temperature. The manager indicated this unit was operating erratically a few weeks/month or so ago and it was looked at three weeks ago. The contractor didn't find anything wrong with the unit. Foods in this refrigerator were relocated to another refrigerator. Have this unit serviced/adjusted as needed to ensure it maintains food at 41f or below

This facility has purchased a utility tub to install for use as a mop water disposal sink, however, the manager wanted to verify this would work prior to installing. The legs will need to be cut off some to allow the tub to fit under the existing faucet

The temperature and pressure regulator for the dish washing machine booster heater in the basement is leaking again. A catch pail that was placed to catch the dripping water was full and over-flowing onto a wooden cabinet. The pail was emptied during the inspection. Have this valve serviced/replaced as needed to eliminate the leaking water

The three under counter light bulbs at the bar are currently un-shielded. Provide either shatter-proof bulbs or bulb tubes with the end caps for these bulbs for protection of the food and equipment at the bar should the bulbs break

(Critical) Bar: observed a quart size carafe containing a few inches of bloody mary mix that was growing mold. Corrected by discarding

Observed some mouse feces in the wooden storage cabinet (with the roasting units) in the back of the facility. Please clean out and sanitize this cabinet and monitor this location regularly for evidence of mice. Mouse traps and bait stations are provided and monitored by a pest control company on a monthly basis

Observed the valve and or the water pressure gauge for the high temperature sanitizing dish washing machine to be not functioning properly. Contact your service provider (gfs) and have them repair this issue as needed

Main kitchen: observed significant amount of wood and metal shavings in the utensil drawers of a wheeled utensil storage cart. Please find a way to either upgrade the drawers/drawer guides to prevent this deterioration or provide another way to store these eating utensils. The manager indicated different cabinet of drawers is to be secured soon to replace the above mentioned unit

(Critical) Observed a previously opened gallon of milk in the one door upright kitchen refrigerator with a last date of use of 3-5-12. Corrected by discarding the milk

(Critical) Observed an unlabeled small jar of home canned salsa in the one door upright kitchen cooler. The manager indicated this was a sample from a couple who would like to start up a business manufacturing salsa at this facility. Only foods from an approved source may be present in this licensed facility. Corrected by the manager removing the salsa from the facility

(Critical) Observed an infestation of small flies (fruit flies) breeding in the residual beer and soda in the bottom of the returnable bottles and cans, as well as some small flies at the main bar. The manager indicated griffin pest control will stop by for their monthly visit yet today or tomorrow. Ensure fly breeding areas (moist debris build-up on floors or in drains, residual beer/soda, etc. ) are eliminated by keeping the facility clean and dry.

(Critical) Observed seven liquor bottles at the bar with pour spouts to have small flies in them. The contaminated liquor was dumped down the drain. Protect your food and beverage by keeping it covered to prevent pest access

(Critical) Observed sliced and diced tomatoes and portioned bags of deli meat in the refrigerator marked for too many days (expiration dates of 9-24). The manager indicated the tomatoes were prepped and the meat was pulled from the freezer yesterday by an employee. Ready to eat, potentially hazardous foods that were prepped yesterday (9-12) would need to have a last day of use of 9-18. Revise these labels accordingly

(Critical) Observed two cans of liquid chafing fuel stored on the top shelf of a wire rack adjacent to the kitchen stove. Corrected by relocating these cans of fuel to the bottom shelf in the dry storage room. Observed a squeeze bottle of pure chlorine bleach stored on the bar counter among beverages, and misc. Bar equipment. Corrected by relocating to the floor

The soda system carbonator unit was switched out since the last inspection with another old unit. This unit lacks the required back flow prevention. Provide an asse 1022 back flow device (double check valve with an intermediate vent) between the carbonator and your copper water supply line

Observed the gap between the bar ice bin and the adjacent drain board to be in need of re-sealing to eliminate the gap that can allow debris and moisture to accumulate

The door gasket on the turbo chef one door upright cooler is torn. Replace this gasket to prevent your cold air from escaping and to provide a cleanable surface

(Critical) Observed a dozen brown eggs (from a private farm) in a paper egg carton (observed some straw and a bird feather on the eggs) from "trader joe's" with a sell-by date of december 19th. All foods in this establishment must be from an approved source

(Critical) One door upright refrigerator in front kitchen: observed one container of expired sliced deli turkey (corrected by discarding), and some single portion bags of other sliced deli meat (pulled from the freezer one or two days ago) that only had the freezing date on them (corrected by providing a six day use-by date).

Observed the ice in the bar ice bin to be subject to possible contamination by a nearby insulated line that is deteriorating. Remove this old pipe insulation and re-install new insulation

(Critical) Observed the following items in the one door upright cooler near the dish washing station without expiration dates: roast beef, pastrami, sliced tomato, chicken salad, etc. Corrected by providing proper expiration dates based upon when these items were opened/sliced

(Critical) Observed raw chicken and raw ground beef stored in the four door refrigerator above raw vegetables, bread and butter, etc. Corrected by relocating the raw animal foods below all ready to eat foods

Observed some mouse droppings in the base cabinet in the back storage room that is used to store the roasters. Please thoroughly clean and sanitize this cabinet and the equipment in it

Again, observed narrow necked juice bottles being saved and re-used. These containers with narrow openings are not easily cleanable. Please recycle these containers when the original product is finished and purchase commercial beverage dispensers with either large openings or those that come apart for thorough cleaning
