The lamps are directly over exposed food. 2. In the walk in freezer observed a missing light shield. Correct by providing proper shielding

In the origins food service area observed a broken electrical outlet on the floor adjacent to the cash station. The outlet electrical connections are exposed. Correct by replacing or repairing the outlet

(Critical) During inspection did not observe a pressure backflow prevention device at the cleveland steamer creating an unprotected supply line. Correct by installing an approved back flow prevention device to meet asse 1012 or asse 1022 this is a repeat violation. A willow run community schools work order was issued for this item on 10/26/2011. After discussion with k. Schweighoefer, washtenaw county food supervisor, the requirement for a risk control plan as required by the washtenaw county procedures for the administration and enforcement of the michigan food law of 2000 is being waved. The requirement is waved because of the nature of the violation and lynne schweinsberg, the pic has done everything she is able to have the item corrected. ,

Observed threshold for wic in cooler door to be unsealed plywood. Correct by repairing or replacing threshold with a material that is smooth, non absorbent, and easily cleanable

Observed cold water faucet at prep sink without a handle. Correct by providing a handle for the faucet

Observed several pieces of equipment no longer in use including, soda dispensing machine. These items should be removed if possible to prevent the accumulation of debris and harborage of pests. Correct by removing unused equipment

Observed a garbage disposal connected to the wash compartment of the three compartment sink. Please note garbage disposal is not allowed in any sink. Typically garbage disposals are installed in drain boards under spray nozzles only. Correct by scheduling removal of the garbage disposal

I observed several light tubes stored on a shelf over catering dishes. As discussed, provide complete separation between all maintenance items and food, utensils and single service items.

(Critical) Observed baited mouse trap on shelf in dry storage area directly above boxes of food (chips). All chemicals and items containing toxics must be stored below and away from all food and clean equipment at all times to prevent possible contamination.

(Critical) Observed 3 trays of wrapped rte hamburgers, cheeseburgers and chicken patties in upright hot box in prep area with an internal food temp of 101-120f. Unit is not functioning correctly. All phf must be hot held at 135f or more at all times. Do not use this hot box until properly repaired. Since all hamburgers, cheeseburgers and chicken patties were prepped 20-30 minutes ago, pic corrected by reheating mentioned food items to 165f and transferring food to another hotbox in kitchen. Critical corrected

Observed old equipment and debris in hallway in back kitchen area. To correct, please remove all old equipment and clutter from facility as soon as possible

Observed upright hot holding cabinet in prep area not able to hold food at 135f or more. Do not use this unit until properly repaired. To correct, please repair until as soon as possible

Observed floor and walls near wif door with an accumulation of ice creating a possible slip hazard. To correct, please remove ice so all surfaces are smooth and cleanable
