Also, the tray of par-cooked fish in the reach-in was not marked "par-cooked on" date. The other trays of par-cooked fish in the wic were dated but not marked as "par-cooked". Corrected - the staff dated the milk as it was verified that it was opened yesterday, and the tray of fish in the reach-in was dated and labeled par-cooked
(Critical) Found the menu to be missing some food items that need to have an asterisk regarding cooking to order: filet, eggs, omelets. Also, since the hamburger is still on the child's menu, it must be fully cooked to 155*f when served to children. Corrected - pic added the required asterisk to these menu items. The children's menu burger is always fully cooked to 155*f.
(Critical) Found the sani bucket at the bar not at the correct concentration due to staff not using test strips as required. Corrected - staff was educated on the correct test strips to use and sani bucket solution was discarded and remade to the correct concentration
(Critical) Found citronella candles and cutter insect repellant stored above the condiment caddy's at the deck storage shelving. Corrected - these items are considered chemical in nature and as such must be stored below and away from food items. Pic relocated these items to the lowest shelf
Found the can opener blade to be in need of replacement as it is starting to become nicked up and may allow metal shavings to contaminate contents of canned food when they are opened. To correct - replace the can opener blade asap
Found the clean cutlery caddy at the dish area to be in need of replacement as it was no longer smooth, durable and cleanable. Also found a tub of clean plastic lids that need to be sorted and older, lids that are no longer in good shape should be discarded. To correct - replace the cutlery caddy and sort and discard old lids
Found the handles and doors of the double reach-in freezer on the cook line to have an accumulation of food debris on them. To correct - clean now and keep on a more frequent cleaning schedule
Found no hand wash signs in the public restrooms. The public restrooms need to be provided with signs that remind staff to wash their hands before returning to work. It is noted that the employee restroom has the required signage. To correct - post required signage in public restrooms
Found both the men's and women's restroom doors to be in need of self-closers. To correct - please install self closers on both the restroom doors.
Found the floor in the wic to be in need of repair as the diamond plate surface is cracked and is no longer a smooth surface that is easily cleanable. To correct - repair wic flooring as needed
Found the waste receptacles in the stalls of the women's restroom to not have covers. To correct - provide waste receptacles that have covers

(Critical) Found hose bib on front of building to be equipped with a "y" connector that does not have vacuum breakers at each outlet. Also found a hand sprayer attached to the hose when not in use. Corrected - hand sprayer and "y" connector was disconnected at the time of this inspection. In the future, if a "y" connector is needed at this location, each outlet of the "y" must have a hose bib vacuum breaker installed, and a hand sprayer cannot remain attached when not in use

Corrected - food items without datemark were discarded by pic. Because this critical violation is repeated from the last routine inspection, a risk control plan must be completed and submitted by sept. 15th, 2012.

Found hand sink in employee restroom to have slow drain lines. Water was run only long enough to achieve hot water and the sink was full of water, very slowly draining. Also found the wait station hand sink faucet in bad repair. To correct - snake and or repair drains now and keep them maintained. Repair the wait station hand sink faucet
Found single service containers stored uncovered in a broken down box in the back room with another box stacked on top of it. To correct - keep single service items protected in plastic sleeves and/or original container that is in good condition and stored such that it is not subject to crushing and double stacking
Found the floor in the back prep room to be in bad repair with tiles chipped, broken or missing. Underlying concrete is exposed and is not a smooth, cleanable surface any longer. To correct - repair or replace floor in this area before the next routine inspection

Corrected - pic thuroughly cleaned all components at the time of this inspection. Also, discontinue the use of a cardboard box as a cover. Use a clean plastic garbage bag that can be discarded each time ot a cover that is constructed to be be smooth, cleanable and durable

Found several locations were cleaning is needed, such as: the grill area where shelves have an accumulation of crumbs and debris, floors in dishwasher area, floors in prep area, floors in walk-in cooler, beer cooler floor, and where there is mold growth on overturned crates, floors in dry storage areas. To correct - clean now and keep on a much more frequent cleaning schedule. This is another reoccurring non-critical violation that has been repeated since the last 2 inspections. It's time to attend to these areas within the next 6 months. Continual repeat violations will result in an in-office conference
Found back door not fitting tightly - gaps around door that can allow pests to gain entry. To correct - provide weather-stripping and/or door sweep to make the door more tight-fitting
Found the lighting in the wal-in cooler to be dim. Lighting in the grill area is dim as well. To correct - in the walk-in, replace current bulb with 100 watt bulbs, including the fixture above the door that has no bulb currently. At the grill area, a new light cover in the ceiling, or. A clean light cover will help this situation
Found several cottage cheese tubs stacked wet on shelf next to dish machine. Also found several clean hotel pans to be stacked wet, and upright on the shelf as well. To correct - allow all pans, tubs, equipment to air dry before storing in a stacked manner
Found moldy overturned crates in beer cooler. To correct - remove crates as they are not easily cleanable and must be moved often to clean underneath. Replace with additional storage shelves or dunnage racks
Found ceiling tiles in kitchen and the motors on top of the reach-in coolers, as well as the shelves to the right of the ovens to have an excessive amount of greasey dust accumulation. To correct - clean now and keep on a much more frequent cleaning schedule. This is a consistant non-critical item that needs to be addressed within this next 6 months before an in-office conference will be required
No mop sink found, mop water being transported to the employee restroom toilet for disposal. To correct - a mop sink should be conveniently located at the mop filling faucet. Current code requires a mop sink be provided. This issue was mentioned in 2010. The addition of a mop sink must be addressed within 6 months
Found old sanitizer strips to be zip-tied to the dish machine. These strips were visibly old, with an accumulation of dusty debris inside the container without a lid. It was evident that the strips are not being used. To correct - replace old strips with a new container and locate the container away from moisture. Strips should be used daily to ensure dish machine is properly sanitizing
Found water line dripping at mop fill faucet. To correct - repair the leak at this faucet within the next 6 months
Found condenseate line in bar cooler to be dripping onto the floor of the cooler causing water to stand on bottom around beer bottles. Also found reach-in cooler on cook line to be dripping on rte foods. To correct - repair condensate line in bar cooler and repair leak/drip in reach-in cooler to avoid possible contamination of rte foods. Pic removed foods that were stored under drip
Found an excessive amount of old equipment and other various items that are being stored in the establishment. To correct - remove these items as they are taking up valuble storage space for the items you do need to store safely
(Critical) Observed the cook assembling hamburger toppings (lettuce, pickle, onion, etc. ) with bare hands. Corrected by discussing the requirement to use single use gloves or utensils for handling ready to eat foods, and by the employee putting on gloves
(Critical) Observed several open top mouse bait stations (residential type) on the floor in the dry store room. Eliminate these bait stations and the spilled bait and only allow your pest control contractor to supply approved bait stations (there were a couple approved stations present in this location).
Observed the drain line that receives wastewater from a water cooled compressor (located on the left side of the doorway on the way back to the back bottle storage room) to be leaking. Repair as needed.
Observed some food containers in the walk in cooler that were not covered to prevent misc. Contamination. Please ensure foods are kept covered for protection (except hot foods that are actively cooling on top shelves)
Observed three particle board wooden shelves at the dish washing station that has exposed particle board as well as cracked/deteriorated surfacing. Provide replacement shelving that is smooth, easily cleanable and does not absorb moisture
A thorough deep cleaning is needed in many out of the way areas such as the following: under the bar hand washing sink, the floor sink under the bar glass washing basins, the floor to the right of the sliding door server station cooler, the ceiling vents in the kitchen and the server station, the floor of the walk in cooler, under kitchen three compartment sink and dish washing machine, the floor in the dry storage room, the floor drains in the kitchen and back rooms, and the mat above the back kitchen room floor drain. Maintaining these areas clean and dry will eliminate odors and debris that can be a food source/breeding area for pests.
Observed the interior of the draft beer dispensing spouts to be in need of more frequent cleaning. Consider obtaining an appropriately sized bottle brush to facilitate cleaning of the spout
Observed spilled food and mold growth on the under side of the walk in cooler shelves. Clean these shelving units and place them on a periodic cleaning schedule
Observed gear oil seeping out of the hobart mixer head and an oil droplet present on the underside of the bolt head that is located above the mixing bowl. Repair this mixer to eliminate the oil leak.
(Critical) Provide a thread-on hose connection back-flow preventer (a. S. S. E. 1011 device) on the back kitchen room wall mounted faucet. You can then attach your fill hose to the back-flow preventer, however, no down-stream shut off valves are allowed (open ended hose only)
(Critical) Provide drain line air gaps for the following pieces of equipment: the ice machine/storage bin combo in the back kitchen room, the bar area ice bin, and the coffee station ice bin. Observed two of the microwave ovens with missing ceiling covers. The exposed microwave oven parts are not easily cleanable--provide replacement covers
Dishwashing machine: 1) ensure your dish washer is removing and flushing out debris build-up from the top and bottom rinse arms; 2) ensure the housing for the food catch screen is cleaned to remove residual food build-up.
Some of the storage rooms are very full. Please sort through these items and eliminate any items that are broken or unnecessary to the direct operation of the business.
Kitchen ice machine: the ice bin deflector shield is in need of cleaning at this time
Ensure staff are utilizing the test papers to verify the sanitizer concentrations at the bar
The floor in the following areas need cleaning now and more frequently in the future: the black mat and the floor below the mat at the dish washing station; the floor at the bar (under hand sink, under ice bin and three compartment sink), back kitchen bottle sorting area
The fan guards in the walk in beer and the server station cooler were observed with mold growth. Thoroughly clean these areas now and more frequently in the future.
The grease cup in use is sitting at the bottom of the hood splash wall and therefore grease is dripping down from the trough onto the splash guard and into the catch receptacle. Replace cup in proper location to avoid leaving a grease trail which may be a fire hazard as well. Clean splash guard once grease cup is reattached to proper catch location.
Protective splash cover missing from the ceilings of both microwaves. Replace to eliminate splash build up on inside of microwave. Interior of the upright cooler had excess water build up. May be a plugged drain. Unplug drain to eliminate build up.
Fryer cabinets, hood side splash and outside of some of the equipment need cleaning. Clean now and place on a more frequent cleaning schedule. The underside of the storage racks in the walk in cooler and the fan cover are dusty. Clean now and place on a more frequent cleaning schedule.
A couple of inaccurate broken thermometers found in the prep unit and in the upright cooler. Replace thermometers so that the air temperature of the unit can be monitored. Corrected by replacing thermometers.
Manufacturer's containers are being reused. Discontinue this practice and provide approved food grade containers that are reuseable.
Bulk foods found in the rear storage area uncovered, retighten bags after using or provide containers with lids so that foods are properly protected and not exposed.
Several pieces of domestic equipment nin the facility including microwaves. Replace with commercial equipment when in need of repair.
