Health Report - 10/23/2013 (2 critical incident(s))

(Critical) Found pizza dippers, chicken nuggets and tater tots holding @ 120* f in the hot box, and holding @ 108 -120* f on the line. Corrected - all above food items were pulled and re-heated to 165* f before putting on the line for service. Temperatures must be checked after removing food from ovens to ensure proper cook-off temp of 165* f, before placing in hot box, and checked again when removing from hot box to ensure temps are 135*f or more before placing on the line

(Critical) Found ambient air thermometers on the hot box reading @ 160* f when actual themperature of food was reading 121* f with probe thermometers. To correct - replace ambient air themometers. Service tech was called to check on hot box function

Found hot box holding food @ 120* f when the mechanical thermostat was reading 160* f. Corrected temporarily - food was reheated to 165* f. At the time of this inspection, service tech was called to check hot box

(Critical) Found two cans in general storage that were dented. Corrected - cans were relocated to the designated area away from general food storage area. This is the second time in a row for a routine inspection that dented cans were found not segregated. As such, a risk control plan will be required to be completed and submitted at the time of the follow-up inspection in 10 days (on/about may 6th, 2013),

(Critical) Found the panini press and the microwave in the dining area to have an excessive accumulation of food debris. To correct - thoroughly clean these pieces of equipment now and keep on a more frequent cleaning schedule. Note* - the microwave is not a part of the school kitchen. The microwave is supposed to be maintained by student council. However, since the microwave is used by students in the school cafeteria, it is subject to inspection and is required to be maintained in a clean safe manner. A follow-up inspection will be made in 10 days to verify correction of this violation

Found the hand sink next to the dish machine and the hand sink on the service line to be short-cycling which makes it difficult to achieve and maintain 100* f. To correct - there are other hand sinks that are located conveniently for use, therefore this is not a priority violation. However, maintenance should be made aware that these two sinks need to be checked and repaired or replaced to correct the short-cycling and consistently provide the required 100* f temperature for handwashing

Found staff not wearing hats as required. Corrected - staff put hats on as required,

Found the outside dumpsters to be left open. To correct - dumpsters must be closed after use. Note* - this is the responsibility of the custodial staff not kitchen staff

Found sveral mild/moderately dented cans that were not separated from general storage. Corrected - pic relocated the cans to the designated area for return to supplier

Found several steel hotel pans stacked wet on the shelf. To correct - all pots and mans must be allowed to completely air dry before stacking for clean storage

Found 3 of the 4 light bulbs under the hood in stove /oven area to be burnt out. To correct - replace burnt out bulbs

Found whole apples displayed in a bowl without protection. To correct - relocate apples under protection of sneeze guard. Oranges and bananas are ok to have out in an unprotected bowl because the peel is removed before consumption

The mop sink faucet only contains an atmospheric vacuum breaker, however, the faucet is connected via a black rubber hose to a chemical dispenser containing a shut-off valve. These atmospheric vacuum breakers are not approved for use with down-stream shut-offs. Obtain and install a flow-through back flow prevention device (such as eco-lab "sidekick" unit, or equivalent) that threads on to the end of the faucet. The existing black hose would then connect to the threaded fitting on the flow-through back flow preventer. Contact me if there are any questions regarding this matter

Observed a oily/fatty residue in the floor sink that receives drainage from the grab & go salad, sandwich and soup well station. Please clean this drain and also the floor sink that services the three compartment sink at this time and as frequently in the future to maintain them clean

Observed a few areas within the dish washing machine soiled with food particles (at the beginning of the day). These areas were pointed out to the pic. Please ensure staff are made aware of these areas of build-up so they can be maintained clean
