(Critical) No test kits were available in the facility to check the concentration of the quat solution for the wiping cloth bucket and for set up of the 3 comp sink. To correct, please obtain a supply of qt-40 test papers and test the concentration after each use of the sanitizing dispeser. ,

Observed the wiping cloth bucket to be setup without checking sanitizing solution concentration. Observed the following areas in need of cleaning: the back of the counter behind equipment along the wall on the take out window side of the facility where debris and grease has built up and the receiving drain for the soda machine under the cabinet in the dining area which has a build up of slime mold. To correct, please properly clean these areas now and place on a more frequent cleaning schedule

Observed excessive oil on the top of the "winston" hot holding unit from some deep fryer utensils. Regularly wipe down this unit to remove oil residue

The sliding door on the trash dumpster was left open. Corrected by closing the door. Ensure the doors and lids stay closed to prevent pest entry

Observed the handle of the dispensing scoops for the onion chips and one other frozen item to be resting in the food directly. Ensure your scoop handles are stored such that they do not touch the food

The following areas are in need of cleaning now and more frequently in the future: the floor sink at the dish washing station; the top counter and the interior surfaces (including the drain pipes) of the dining room beverage station base cabinet; and under the lip of the toilets

(Critical) Staff do not have a brush cleaning kit for the sani-serv shake machine. These units have many difficult to clean surfaces and tubes. Obtain a brush cleaning kit from the manufacturer for staff to use when cleaning this machine

The sliding door was found to be left open on the trash dumpster. Maintain this door closed to prevent the entry of pests. Birds were seen inside the dumpster today

The floor and walls beside and behind the deep fryer are in need of cleaning to eliminate excess fryer oil build-up. Clean these surfaces as needed. Ensure the exhaust hood drip pan is not leaking oil as well

1) the back-flow preventer for the drive through area beverage station/ice making machine (located behind the unit) is leaking. Repair/replace this back-flow preventer to eliminate the water leak. 2) the hand washing sink located next to the dish washing area drains slowly. Unclog the drain as needed

Observed several stacks of coffee filters under the front counter not stored in packaging. Provide a plastic bag or a plastic container to protect these filters

1) excessive dust accumulation was noted behind and along side the drive through station ice machine and soda dispensing machine. Thoroughly clean these machine and wall surfaces as needed and on a much more regular basis to prevent this kind of accumulation. 2) two back kitchen floor drains need to be removed for thorough cleaning of the underside and the piping below: at the walk in cooler/freezer area and by the side delivery door below the pull handles for the exhaust hood fire suppression systems

Observed one lid and a sliding door on the outside trash dumpster to be left open allowing birds to gain access to the trash. Close and keep closed your doors and lids to the dumpster. Correct today

(Critical) Observed the following food contact surfaces to be soiled: 1) all of the fountain soda/juice dispensing spouts, diffusers and the mounting surfaces these two components attach to on the machine; 2) three clear yellow plastic food trays for the hot holding unit; 3) the interior surfaces of a spout for a bulk iced tea dispensing unit found on the shelf above the three compartment sink. Items 2 & 3 were washed and sanitized during the inspection. Correct item 1 asap today.

Observed the back-flow preventer for the ice machine/soda dispenser at the drive through to be continuously leaking water. Replace the failed internal components to this device

Please clean the following areas now and more frequently in the future to eliminate debris build-up that can attract pests: 1) the grate at the side entry to the kitchen to be in need of removal for cleaning. 2) the drain pipes in the base cabinet and the counter under the dining room beverage station. 3) the track for the sliding door upright cooler. 4) the shattered florescent light bulb appears to have been inside the bulb tube for over six months (i remember seeing this condition at the previous inspection). Properly discard this broken bulb and provide a replacement bulb inside the protective tube with the end caps

Observed the sticky board for the light trap at the back of the kitchen to be overloaded with flies. Discard this board and provide a replacement one

Observed the handle of the plastic scoop to be resting down in the onion chips. Corrected by repositioning the scoop so the handle was extended out of the food

(Critical) 1) observed stuck-on oil residue on the plastic hot holding pans and insert trays for the hot holding unit for fish and chicken, etc. These items were sent to be washed and sanitized. 2) dining room and drive through beverage dispensing stations: again the soda diffusers and the surfaces they mount to on the machines have excessive accumulation on them. These items need to be cleaned every 24 hours. As soon as possible have an employee remove the spouts and diffusers for cleaning and ensure the clean in place mounting surfaces are washed, rinsed and sanitized. A tool, such as a small diameter bottle brush, will be needed to thoroughly scour the difficult to reach surfaces of the diffuser attachment locations.

Observed no paper towel in the following paper towel dispensers at hand washing sinks: men's bathroom, the drive through station and the back kitchen station by the utility sink. Corrected by providing paper towel

Observed the wiping cloth solution at the grill station near the deep fryers to be less than 150 ppm. Corrected by providing fresh sanitizer at 200 ppm

Safely remove and clean the fan guards for the circulation system in the walk in cooler and freezer to remove the dust build-up

The following drain locations are in need of cleaning now and more frequently in the future to eliminate debris accumulation as well as possible fly breeding areas: the drain through the floor for the walk in cooler & freezer, the floor drain located near the mop sink and the difficult to access drain in the base cabinet for the lobby beverage station

(Critical) Observed the mounting surfaces (a clean in place area) for both of the soda dispensing stations to be unclean. Staff need to use a small brush sized to fit the gap in the soda dispenser (+/- 1/4 inch) to thoroughly clean with hot soapy water, rinse with tap water, sanitize then air dry. A flash light will be needed to verify the thoroughness of the cleaning. These surfaces need daily cleaning and inspection.

Observed numerous green scrubbing pads and another scrubbing pad made of metal in the bottom of the hand washing sink next to the dish washing area. Corrected by removing these items from the sink basin

Observed the hand washing sink faucet (next to the dish washing area) to have an intermittent malfunction whereby it does not dispense water. Trouble shoot the problem and repair/replace as necessary

Provide qt-40 test papers for measuring the sanitizer concentration accurately. The test papers currently available (two kits) are not useable. One of them had become wet and discolored, and the other was lacking a color comparison chart. Further, the yellow test paper kit doesn't have a color for 150 ppm concentration (the usage concentration for this chemical per the label directions).

Observed four iced tea, one gallon containers (empty) stored under the front counter. Please protect the interior of these containers by storing them in a bag. Corrected by placing these containers in a plastic bag

Again observed the soda dispensing spouts, the diffusers and the mounting bases for the drive through and the dining room beverage dispenser machine to be over-due for cleaning. Obtain an extra set of these spouts & diffusers to switch out, and a small diameter bottle brush to fit in the recessed area on the mounting bases for these removable spout parts to thoroughly clean the area. These removable parts and the mounting bases need cleaning every 24 hours

The waste water drain and the interior bottom of the base cabinet in the dining room for the beverage dispenser and the ice machine are in need of cleaning at this time to remove debris build-up, moisture and mold growth

Remove and clean the circulation fan guards for the walk in cooler and freezer to eliminate the dust/debris accumulation on them
