(Critical) Observed several hot shot ant killing pesticides being used in the kitchen. Note: no pests observed during inspection. Poisonous or toxic materials shall be: (a) used according to: (1) law and this code, (2) manufacturer's use directions included in labeling, and, for a pesticide, manufacturer's label instructions that state that use is allowed in a food establishment, (p) (3) the conditions of certification, if certification is required, for use of the pest control materials, (p) and (4) additional conditions that may be established by the regulatory authority. To correct discontinue use of all pesticides, hire a professional to eliminate pests and discard the noted items into the garbage.
(Critical) Found 5 individual portions of cole slaw and tartar sauce in a plastic grocery bag on the bottom of the 2 door upright refrigerator in the kitchen without a prep or discard date. Corrected - cole slaw and tartar sauce cups were discarded. Any in-house prepared phf must to date marked with a 7 date discard date or discarded at the end of a serving event
Found several single service items in the cupboard under the coffee machine in the dining room to be loose and unprotected from handing; including paper plates, cups, bowls and service ware. Also found an insulated beverage dispenser in the old men's room, stored directly on the floor. To correct - all of these items must be stored in their original wrappings or be stored in smooth, cleanable, durable containers to provide protection from dust, debris, splash and excessive handling. All food dispensers must be stored at least 6" above the floor and in a location safe from contamination. The beverage dispenser must be moved to the more appropriate location of the dry storage room
(Critical) Found dish machine to not have the correct level of chlorine sanitizer dispensing into machine. To correct - contact service technician for repair as required. Follow-up inspection to be performed within 10 business days, after the holidays for this inspection since county offices are closed until january 2, 2013. Inspection set for thursday, january 3, 2012 @ noon
(Critical) Found a can of whipped cream and a container of cottage cheese that was opened, but without a date. The container of cottage cheese had a date of december, 2011. Corrected - whipped cream and cottage cheese was discarded. When whipped cream is opened, a date should be written on the can and discarded within 7 days. The same is true for the cottage cheese. More care needs to be taken to ensure the refrigerator is purged on a regular basis, and proper datemarking is observed
(Critical) Found the ladies restroom on the old side to not have any hot water. There was hot water in the old men's room. To correct - please check the supply lines to this restroom and ensure sufficient hot water for hand washing is available in this restroom. This violation must be corrected within 10 days. A follow-up inspection will be made at that time
(Critical) Found an unlabeled spray bottle under the 3 compartment sink. Contents appeared to be bleach sanitizer. Used a test strip which indicated concentration too high at over 200 ppm. Corrected - contents of the spray bottle was discarded, bottle left on drainboard of dishwasher. When making sanitizer for the spray bottle, always use test strips to determine correct concentration.
Found opened, unprotected single service items in the dry storage area. To correct - single service items must be stored in their original container, or provide cleanable, durable, covered container to store opened single service items. Plastic bins with covers work well
Found the men's restroom on the new side of the basement to be without the required handwash signage for workers. To correct - provide a sign that says workers must wash hands before returning to work
Found the ladies restroom in the old part of the basement to be without the required covered waste receptacle. To correct - provide covered waste receptacle in this restroom
Found the hand sink no providing enough hot water to wash hands. When the hot water faucet was turned on, the water pressure was very low and continued to decrease until water stopped flowing. Cold water side is fine. To correct - check mixing valve and correct as needed to provide sufficient amount and pressure of water to adequately wash hands
Found fan in men's room on the north side of the dining room to be in very loud and in need of repair. To correct - repair fan as necessary
Found loose single service styrofoam cups, bowls, and to-go containers stored under the coffee machine in the kitchen and in the dry storage area. To correct - all single service utensils, cups, bowls and containers must be stored in original packaging, or in a covered, durable, cleanable container in a manner such that the items will be protected from contamination, and such that these items can be handled without touching the food surfaces
