(Critical) The following items were found to be unclean: the blades for the lettuce chopper, the agitator for the chocolate frosty machine, and a mustard bottle. Corrected by discarding the agitator (damaged), and cleaning the other two items

(Critical) The following items were observed to be unclean: the lettuce chopper (lettuce stuck in the upper push block), the tomato slicer (tomato skins found in the push block and tomato flesh on the blades) and a few of the clear, yellow hot holding 1/3 pans (had oily residue on the bottom). These items were re washed to clean these surfaces

(Critical) Upon arrival (+/-10:15 am) no soap was provided in the dispenser at the front kitchen area hand washing station. No other soap refills were available in the building. A manager arrived about an hour later with a soap refill. Ensure enough extra soap and paper towels are always available

Your food service license is located in the managers office. Relocate this license to the front of the store where it is visible to your customers

Several out of the way floor to wall junctions are over-due for cleaning: behind the hamburger refrigerator, and nearly the entire perimeter of the back kitchen (under the two compartment food prep sink, under the dish washing sink, under the prep counter, soda system syrup rack, etc. ). Thoroughly clean these areas now and more frequently in the future to eliminate debris that can attract pests and give off odors

1) repair or replace the paper towel dispenser in the dish room so the cover stays closed. Correct within 30 days. 2) the spout for the lemonade dispenser is rough--provide a replacement spout. Correct within 10 days

(Critical) Observed three partial bags of chicken nuggets to be stored in the one door freezer located below the fry/nugget hot holding well to be at 52/53f. Corrected by placing these in the waste food bucket. All phfs must be held at 41f or below for food safety. A full, but in-use 1/6 pan of sliced tomatoes was found to be 41f (bottom) to 51f (top layers) in the make line cooler. This temperature differential is due to its proximity to the bun toaster. Other foods, such as pickle slices stored three containers further away from the toaster were holding at 41f. Corrected by swapping the locations for these two items

Observed the one door reach-in low-boy freezer under the french fry well to be not operating properly (holding between 50 & 55f). Repair/replace as needed

Observed the unused floor sink located under the front counter (across from the hand washing sink), and the shelf the soda system carbonator is located on (spilled syrup) at the other end of the front counter

Repair or replace the back kitchen paper towel dispenser--currently the cover doesn't stay closed

(Critical) Observed an uncovered back-up container of sliced tomatoes in the top of the burger prep cooler to be at 45f. The smaller pans of pico de gallo and the guacamole (lower volume foods) were found at 44f and 48f, respectively, in the top of this same unit. These containers were also uncovered and stored on the left end of this unit near the hot holding for the cheese sauce and bacon. The tomatoes were relocated to the walk-in cooler and the other two foods were shifted further away from the heat source and provided with covers. These foods were 41f and below by the end of the inspection

Observed a round, white plastic lettuce container that has cracked. Corrected by discarding. Obtain an additional pail as needed

Observed single use drinking cups stored in a base cabinet below a drain pipe for the hand washing sink at the front counter area. Corrected by relocating these cups to a suitably protected location

Observed staff storing a mixing spoon for the flavored lemonade in a 1/9 pan of sanitizer. Corrected by providing a clean dry pan to store the mixing spoon in when not in use

Observed the floor sinks in the back prep room and the floor below the maintenance equipment storage area (outside the walk in cooler by the shelving unit) to be in need of cleaning now and more frequently in the future

Front of store between the coffee and the employee hand sink: observed a small rack storing inverted drink cups for lemonade and shakes to be splashed with water from hands being washed. Contaminated cups were discarded and the others were relocated to near the lemonade station

Health Report - 10/12/2011 (2 critical incident(s))

(Critical) Observed the following food contact surfaces to be unclean: 1) the soda dispensing nozzles (the gm indicated the wendy's procedure is to remove them and soak/rinse them in sanitizer--these nozzles once removed the two pieces need to be separated and scrubbed with a brush in hot soapy water, then rinsed and sanitized); 2) the top of the lettuce chopper had lettuce still in it; 3) the stainless steel hot holding wells for french fries/deep fried chicken still had excess grease build-up on them from yesterday; 4) one of the salt shakers didn't get cleaned from yesterday; 5) the lemonade dispensing spout was not disassembled when it was cleaned last night (some lemon pulp was found inside the spout); 6) many areas of the frosty machine were not fully cleaned last night (observed a hardened patch of chocolate frosty mix stuck on the upper rear of the freezing drum, small bits of debris found in the chocolate hopper and on the miscellaneous removable interior pieces and the front cover for dispensing frosties). These items were corrected by cleaning and sanitizing. Ensure your closing staff are made aware of these items and provide any required training. Also, ensure staff use the flash light to verify the frosty machine is clean on the interior of the freeze drum.

(Critical) Black flexible water hoses that are connected to the drinking water supply (without a back flow preventer) to supply water to the chemical dispensing system have not been approved for potable water. As a correction replace the existing hoses with ones that meet nsf standard 61 drinking water rating (or equivalent).

Observed the following floor areas to be in need of cleaning now and more frequently in the future: 1) the floor at the wall under the three compartment sink; 2) the floor at the wall under the sandwich prep cooler and burger grill station; and 3) the floor at the back wall under the deep fryer station

(Critical) Observed the clear yellow plastic food containers in storage for re-use (cleaned previously) for burgers and chicken to have stuck-on oil residue on the food contact surfaces. Thoroughly soak and scrub these containers clean with a plastic safe scrubbing pad. Ensure staff are inspecting these pans for cleanliness after they have been air dried

Observed the floor under the front counter and the shelf under the soda dispensing station (where the soda carbonator is located) are due for cleaning to remove misc. Debris and leaked syrup

Observed the box of single use only food service gloves at the front of the store to be getting wet due to the close storage proximity to the hand washing sink. Relocate the gloves to a safer location where they will stay dry
