To correct, please reposition drains so the water drops directly into the drain. 2) icicles were observed in the freezer around the cooling unit. Trouble shoot the source of the moisture and repair as needed to protect your stored food. 3) there are several cracked floor tiles by the 3 compartment sink. To correct, replace these tiles
Observed two mop heads in buckets of water. Mops shall be stored in a designated location where they can properly air dry and not contaminate the walls, equipment or supplies. To correct, please store mops in a designated location so they can properly air dry
Observed the interior cavity of the microwave to be soiled with food debris. To correct, please properly clean the interior now and place on a more frequent cleaning schedule

These phfs appear to be at elevated temperatures due to their close proximity to the adjacent hot holding well. Relocate non-phfs to the side closest to the hot holding wells so your phfs maintain 41f or below
(Critical) The upper push block/head for the lettuce chopper is rough from being cut by the cutting blades. Provide a replacement push block
1) the drains for the ice machine in the back kitchen were out of position corrected by re-positioning the drains so the water goes down the floor drain instead of flooding the floor. 2) icicles were observed in the freezer around the cooling unit. Trouble-shoot the source of the moisture and repair as needed to protect your stored food
Observed some employee personal foods stored on the top shelf in the walk in cooler. Relocate employee foods to a container on a bottom shelf to protect your customer food
Outside dumpster area: some trash has accumulated around the dumpster and debris has plugged the drain. Thoroughly clean this area to avoid the attraction of pests
One of the sliding door covers was left open on the dumpster. Corrected by closing the door to prevent the entry of pests
(Critical) Walk in cooler: observed a package of raw hamburgers stored on top of a case of vanilla frosty mix. Corrected by removing the hamburgers and sending them to the cook line burger cooler
(Critical) The drain line for the drive through ice bin has been re-plumbed since the last inspection. The current ice bin drain line is constructed such that the discharge point is below the flood rim of the receiving drain. Repair this drain line to provide the required air gap between the tip of the drain line and the flood rim of the receiving basin (vertical air gap must be at least 2x the discharge pipe diameter)
Observed a couple of the small wiping cloth storage containers to be weak on quat sanitizer. Corrected by providing fresh sanitizer solutions. Note: the dispenser at the three compartment sink is only dispensing quat at a level slightly above 150 ppm (the minimum concentration allowed by the manufacturer. You may wish to have the dispenser adjusted to provide more quat to minimize the frequency the sanitizer needs to be changed)
Closing staff did not remove the garbage from the two small dining room trash cans located in the small island where the ketchup pumps are located. Empty all trash from the facility nightly to eliminate odors and foods that can attract pests
(Critical) Observed multiple employees touching french fries accidentally with their hands during the dispensing and bagging process. Ensure staff handle the french fries only by the fry container or with gloved hands.
(Critical) Several phfs (broccoli, chopped lettuce) on the dining room make line to be at 63f to 52f. Ensure these foods are maintained at 41f or below by changing the storage location of them and/or keeping a cover on the pan
(Critical) No quat sanitizer was found in the three compartment sink, nor in any of the wiping cloth storage containers. The bulk container of quat sanitizer was empty. Corrected by installing a new container of sanitizer and verifying the quat was being dispensed at 300 ppm
(Critical) Apparently staff were not using the provided quat test papers to verify the sanitizer concentration. Remind staff to use the test papers per directions to verify 150-400 ppm quat
Floor tile: many areas of the back kitchen are still in poor repair as far as damaged/missing grout and damaged tile (around floor drains, at the mop sink, along the wall behind the wire shelving unit near the back door, etc. ). Also, the aluminum plate on the floor at the back delivery door is not sealed to the floor. Asap repair these areas to eliminate depressions/areas where moisture and debris can accumulate and become a fly breeding area
An employee had left the small ice scoop in the insulated ice bin at the back counter for lemonade with the handle resting in the ice. Ensure staff properly store the scoop after use to protect the ice and the scoop from contamination
The floors and floor drains in the following locations are in need of cleaning now and more frequently in the future: under the ice machine, the walk in cooler floor drain, the under the waste oil tank (and top surface) and mop sink area
A small, red insulated cooler is being used to store ice for the lemonade station. Provide a self draining apparatus to allow melted ice to drain away (a drain line air gap above the receiving receptacle is required)
Again observed small flies (likely vinegar/fruit flies) present in most areas of the facility. Appropriate steps must be taken to eliminate the fly breeding issue in this facility (repair necessary flooring/drain areas, maintain the facility clean and dry)

Repair these areas to eliminate depressions that are accumulating water and debris. These conditions can lead to areas that allow flies to breed.
Observed the food service license for this facility to be posted in the manager's office. Relocate this license to the front counter area where it is visible to your customers.
Observed the bottom shelf of the back kitchen prep counter island to be in need of cleaning
The floors / floor sinks in the following areas are in need of cleaning: 1) under the front counter beverage station ice bin; 2) under the drive through beverage station ice bin
(Critical) Observed a "y" splitter with shut-off valves installed on the end of the mop sink faucet. This mop sink faucet contains an atmospheric vacuum breaker. Atmospheric vacuum breakers may not have any down-stream shut-off valves. As a correction install a wasting/bleed-off tee between the faucet and the "y" splitter
(Critical) 1) observed an employee rinsing off burger patties (to be used for chili) in a colander at the back kitchen hand washing sink. Corrected by the employee ceasing this activity and relocating to the two compartment food prep sink. 2) observed two red handled scrubbing pads stored in the front hall hand washing sink (next to the upright salad cooler. Corrected by removing these scrubbing pads from the sink basin
(Critical) Observed an employee out side smoking prior to starting his shift for the day. He entered the kitchen, logged into work at the front counter cash register, then obtained a paper wendy's cup and then obtained a beverage from the drive through drink station with out washing his hands. Corrected by discussing this issue with the employee and by the employee washing his hands at the hand washing sink
(Critical) Dining room sandwich prep cooler: phfs (lettuce, sliced tomato, shredded cheese, pico de gallo, cooked broccoli, guacamole) to be at 64 to 71f. These items were discarded. Drive through sandwich prep cooler: some slower moving phfs (pico de gallo, guacamole, spring mix, cooked broccoli and shredded cheese) in the top of this unit were found to be at 59 to 61f. These items were discarded.
(Critical) No atmospheric vacuum breaker is installed on a hose connection located in the mop sink area just below the rpz back-flow preventer. Install an a. S. S. E. 1011 hose connection back-flow preventer at this location
Observed multiple areas in the back kitchen with cracked and missing pieces of floor tile. Repair as needed to eliminate depressions that collect food scraps and water
Observed both of the sandwich prep coolers to not be maintaining proper temperatures. The evaporator coil in the bottom of the unit was a large block of ice. This unit was turned off to start the defrosting process. Not all of the kitchen a/c units were working so the kitchen was quite hot and humid. This condition greatly impacts the performance of refrigeration equipment. The manager contacted a repair contractor to service the a/s and these refrigerators as needed. Ensure these units maintain foods at 41f or below. Utilize pan covers as needed to maintain foods at proper temperature
Most of the floor sink basins, the mop sink station and the area around the grease trap in the back kitchen were observed with old debris build-up. Thoroughly clean these areas now and maintain them clean
(Critical) Observed phfs (salads and coffee cream) in the low-boy one door cooler under the chili at the drive through station to be at 55 to 56f. Corrected by discarding the phfs.
The one door low-boy refrigerator at the drive through station used for salads wasn't operating at 41f or below. As soon as possible adjust and/or repair as needed to maintain phfs at 41f or below. The gm indicated the maintenance employee will be arriving later to repair an a/c unit and will also look at this refrigerator
The drain lines for the ice machine ice holding bin and the walk in cooler were out of position and not properly discharging into the floor drains. Corrected by staff re-positioning. Also, it appears one of the three soda system carbonator is weeping water from the back-flow preventer. Contact your soda system company to repair/replace this back-flow preventer as needed
The floor under the soda system station at the drive through area has a significant build-up of debris. Thoroughly clean this area now and more frequently in the future
(Critical) Observed the following food contact surfaces to be unclean: over 50% of the yellow plastic hot holding pans for chicken, approximately ten small metal food pans (1/6th pans), and a large hot holding pan for chili. These items were sent to be soaked, scrubbed and re-washed.
The open top, gray rolling trash cart has accumulated food debris in the bottom. Maintain this cart clean to prevent the attraction of pests

Two staff involved in food activities (dispensing ice/drinks, etc. ) were observed with jeweled rings on their hands. Rings must either be removed or a single use glove worn (and changed as required) to prevent jewels from falling into food (choking and/or tooth hazard) or food from building up on these rings.

The floor sink drains in the following areas are unclean: under the dining room beverage station, under the drive through beverage station and at the dish washing station. Clean these areas at least 3 times per week to maintain them clean and prevent the attraction of pests
Observed an employee with a couple jeweled, intricate rings on her hands during food prep. Staff need to either remove their rings (except for plain bands) during their shift or utilize single use gloves (even when working with beverages/ice and non-ready to eat foods). Corrected by the employee putting on single use gloves

This employee then scooped an order of french fries and dumped out a freshly cooked basket of french fries and salted them. Corrected by discussing this matter with the manager and the employee, who subsequently washed his hands. The lack of proper hand washing when required is one of the leading causes of food-borne illness. Please take an opportunity to discuss the hand washing requirements (when and how) with your staff
(Critical) Observed numerous small flies present throughout the facility. Additional measures are needed to eliminate the fly breeding areas. Repair damaged tile/grout, eliminate excessive moisture accumulation and purchase and use several commercial floor drying fans to dry up floors and out of the way areas.
The vertical drain into the floor for the front counter beverage station & ice bin is not continuous. This is allowing liquids to seep under the front counter area. Enlarge the opening in the stainless steel base of the counter (to make this drain area more accessible for the repair and future cleaning), and provide a solid vertical pipe to extend all the way through the bottom of the cabinet to the existing drain flange. An air gap must be maintained between the ice holding bin drain line and the receiving plumbing
Many areas of the kitchen flooring, mop sink and drain areas are rough and have depressions due to damaged/missing tile and grout. Asap repair these tile areas and seal joints with silicone sealant as needed to provide a smooth, easily cleanable floor surface that does not trap food debris and allow moisture to collect. This maintenance item must be made a priority
Some out of the way floor areas are in need of cleaning to eliminate food scraps and standing water. This chore is made more difficult due to damaged floor tiles and missing grout. Also, the area around the removable cover for the front counter beverage station is due for cleaning.
Observed the interior surfaces of the spout for the bulk lemonade dispenser to not be washed the last time it was used. Ensure all staff are made aware of the need to disassemble this spout entirely to thoroughly clean the interior surfaces
(Critical) Observed the following items to be unclean: the bulk lemonade dispenser spout (removed and cleaned), the mounting surfaces for the diffusers and spouts at the front counter soda dispensing station had black build-up, multiple squeeze bottle spouts had mustard/ketchup residue, four metal 1/3 pans with burger and oily residue, and a chili reheating pot had chili residue in the bottom. These items were sent to be soaked and re-washed. Ensure staff are thoroughly cleaning this equipment and verifying it is clean prior to placing them in storage for re-use
Observed missing grout and small tile sections around the floor sink located under the ice bin at the drive through beverage station, the floor to wall juncture under the storage areas for the chili pots and the joint of the wall to the mop sink basin are in need of repair to provide smooth and easily cleanable surfaces that don't trap debris or moisture

Observed spilled garbage in the bottom of the wheeled waste hauling carts. Regularly clean these carts of spilled debris to prevent odors and the attraction of pests

One was labeled for house-hold use only and the other two were identical and indicated they were not for use in a food service facility. Corrected by removing these insecticides from the store. Only purchase and use insecticides that are approved for use in a restaurant (according to label directions), only utilize the services of a professional exterminator, and do your best to maintain the facility clean and dry to prevent pest problems
(Critical) Observed no atmospheric vacuum breaker (hose bib thread-on type (a. S. S. E. 1011 device)) provided between the shut-off valve and the black open ended mop bucket fill hose. Provide vacuum breaker at this location
Observed food/debris build-up in the following locations: under the row of back kitchen area sinks, under and behind the drive through beverage station base cabinet, and under the one door upright freezer located next to the baking oven. Please clean these areas now and on a frequent basin in the future

The following areas are in need of repair: 1) the back delivery door--the latching mechanism is loose and/or the door is damaged; 2) the wall base cove tile behind and along side of the one door back kitchen sink (with the over-head spray nozzle) is coming off of the wall; 3) the floor under this same sink; 4) water is accumulating in a trough located in the rear part of the drive through base cabinet (may be due to a poor seal of the ice bin insert through the counter top--trouble shoot and repair as needed)
Observed three wheeled trash carts located in the corral area behind the back delivery door. The bottoms of all three of these trash carts have worn through and one of them lacks a cover. Provide replacement trash carts that are leak proof and insect/rodent resistant
Observed a couple foods in the top of the front counter area one door upright salad cooler to be slightly elevated. Small metal pans containing salad dressing and coffee cream were filling the entire top shelf area and preventing good air flow to the rest of the unit. Always maintain some open area on each shelf to allow proper air flow throughout the unit
Observed a handful of out of the way floor areas to be in need of more frequent and thorough cleaning: under equipment at the drive through station (including around the floor sink serving the beverage station), the corner of the floor behind the rolling trash cart, under the soda system syrup rack, and under the front counter. Maintaining these areas as clean and dry will eliminate a food source and/or pest breeding area.
Observed the outsides of some bacon trays and some metal and plastic food pans to have an oily residue still on them after going through the washing process. Ensure staff washing dishes are also scrubbing the outsides of these pans/containers to get them clean as well
Observed a hose connected to the mop sink faucet (containing a built-in atmospheric vacuum breaker) and running to the chemical dispenser to be subject to continuous pressure. Provide a bleed-off tee/side kick unit to allow the system to depressurize after the water supply is turned off. Corrected by the maintenance employee providing a side kick unit between the sink faucet and the hose
Observed missing grout / sealant around the floor drain in front of the ice machine and around the perimeter of the floor sink located under the ice machine/bin. Clean and dry these areas and apply grout / sealant as needed to eliminate the current gaps and depressions
Still observed the soda system carbonator on the floor under the ice bin at the drive through station. Have your soda system contractor relocate this carbonator to the rear part of the bottom storage shelf located just to the right of the ice bin
Observed holes worn in the bottom of the blue wheeled trash cart stored inside the back of the facility near the employee bathroom. This condition is allowing any waste liquids that leak out of the garbage bags to drain on to the floor. Repair or replace the garbage cart to provide a leak-proof container

1) observed no tile grout between the floor tile located under the drive through beverage station--provide tile grout in this area after cleaning. 2) mop sink area--observed the sealant to be in need of replacing at the junction of the tile base tile and the frp wall material. Remove old sealant, clean and dry the surfaces and reapply a silicone sealant to prevent the mold/moisture issues at this location
Observed a small number of small flies at the front counter lemonade station area and two locations in the back kitchen with one fly each. Ensure staff are performing frequent inspections for both the presence of flies and possible breeding/feeding areas.
Observed the interior bottom of a trash container in the back hallway to be in need of cleaning to eliminate the food debris and moisture accumulation
Observed some out of the way floor areas with debris accumulation that need more frequent attention: 1) the floor sink serving the front counter drink station (try to provide better access to this area for regular cleaning); 2) under the drive through counter where the drink station and the cash register are located, 3) the along the wall to the left of the drive through salad cooler and chili hot holding unit; and 4) around the holding tank for the used fryer oil.
Observed the metal hamburger grill dividers to be damaged. Some of the dividers are separated from the frame others that have been repaired in the past have poor (not smooth) welds. Either replace or repair these dividers ensuring any new welds are finished to be smooth
Observed a few areas, especially by the deep fryer station to have missing/worn tile grout or the tiles to be damaged or missing. Repair to provide a smooth easily cleanable surface that prevents moisture and debris accumulation

Observed several areas to be in need of cleaning at this time and more frequently in the future: outside--between the outside of the old walk in cooler (used for storage of equipment) and the concrete wall; the walk in cooler drain, the floor area under the clean dish drain board of the dish washing sink, the mop sink area (including the lower section of the mop sink area walls), around and under the solid floor drain cover in the walk way between the dish washing sink and the ice machine, and the floor to the right of the ice machine
Observed the interior of the rolling trash cart located at the back door to have a layer of old food accumulated on the bottom. Maintain this trash cart clean to prevent the attraction of pests

Either: 1) thoroughly clean and dry the underside of the base and the top of the counter and then seal the perimeter of the base to the counter top with a silicone sealant, or 2) ensure this can opener base is removed daily for cleaning. A third option would be to purchase and use a can opener that has a different mounting system that doesn't have this issue
Observed the deep metal chili pans to be not fully air dried prior to stacking them for storage. Allow these pans to fully dry before stacking them or store them separately on a vented shelf

Provide asap. 3) observed some areas throughout the kitchen to be in need of tile/tile grout repairs to eliminate cracked tile and missing/worn grout. 4) the silicone sealant around the drive through ice holding bin and around the hand washing sink basin located adjacent to the upright salad cooler. 5) one half of the walk in freezer evaporator coil is iced up and is not allowing one of the fan blades to turn. Defrost and repair as needed
Back prep kitchen: observed a 1/6 pan with a tomato cutting knife in it stored in the basin of the hand washing sink. No equipment/utensils may be stored in a hand washing sink. Hand washing sinks must remain clear for proper access to wash hands
(Critical) Essentially all of the plastic hot holding pans for the chicken have old, stuck-on oil residue stuck to them (20 total pans). Either thoroughly soak and clean these pans or replace them so staff can work on maintaining new pans in a clean state. Observed a smaller, handle-less yellow plastic pan with old, stuck-on oil residue, and a metal 1/3 pan with some food build-up around some dents in the corners. These two pans were sent to soak and be cleaned. Also, observed the blender for the shake mixer to have shake residue on it from the previous day. Staff immediately cleaned and sanitized this blender
Observed the following areas/items to be unclean: 1) the floor and floor sink under the drive through beverage station (the frosty machine needs to be removed to gain better access to this area); 2) the floor and floor sink under the dining room beverage station; 3) the floor-wall juncture under the three compartment sink; 4) and the dust pan has excessive debris build-up in it; 5) the floor and floor drain at the hot water heater. Clean these areas now and more regularly in the future to maintain the facility clean and dry
Observed a couple small flies located around the base of the hot water heater where the water has accumulated. Ensure someone is inspecting the facility frequently for the presence of pests
1) a water leak was noted on the soda dispensing tower (water is running down the metal panel above the spill tray) at the drive through station. Locate and repair this leak as needed. 2) the condensate drain for the hot water heater vent pipe was detached allowing a moderate amount of water to drain onto the floor. Clean and glue this fitting so it is watertight. 3) observed a leak in the connection of the drain hose to the dining room beverage station spill tray. Provide a hose clamp, etc. As needed to eliminate this leak
(Critical) Observed the following stored equipment to be unclean: the four hot holding trays for the chicken nuggets (oil residue), the lettuce chopper, a couple chili pans, 5 or 6 squeeze bottle tops, a square, clear plastic chili tub, and a frosty machine cleaning brush. These items were sent to be cleaned. Ensure staff are inspecting equipment/utensils after they have been cleaned, sanitized and air dried prior to using or placing them in storage

Observed an air thermometer in the reach-in portion of the dining room make line cooler reading 22f. This unit was maintaining an air temperature of 34f
(Critical) Observed chili (left-over from the previous day that was just reheated and placed into two separate holding units) to be at 126-130f in the front counter hot holding unit and 120f in the drive-through unit. The drive through holding unit was on a setting of 2. 5 on the dial. The gm indicated this setting was too low and the hot holding steam wells may not have been preheated enough prior to the chili being placed in them. Corrected by reheating the chili on the stove top to 165f
Observed an accumulation of food and debris at the floor-wall junction under the following areas: the front counter, the drive-through station ice bin and under the three compartment sink. Additionally, the difficult to access floor sink under the drive-through ice bin and frosty machine is in need of cleaning at this time
1) observed some tile grout to be worn away in the center of the employee bathroom that is allowing the accumulation of standing liquids. Remove the remaining grout in this area an apply a full thickness layer of new grout to eliminate the depression. 2) a gap/depression is located along the wall on the right side of the mop sink that is allowing debris and mop water to accumulate. After thoroughly cleaning and drying this area re-seal this gap with grout or appropriate sealant to eliminate this depression and provide a water-tight seal
The plumbing at the mop sink was changed again such that the "side-kick" bleed off tee fitting was installed in the wrong location with respect to a "y" splitter. Corrected by removing the "y" splitter and re-installing the "side-kick" to the mop sink faucet

The right branch consists of a black rubber hose (this hose has no markings to indicate it is safe for potable water) without a back flow preventer between the potable water supply and the hose leading to a chemical feeder (chemical feeder contains an internal air gap to prevent chemical back-siphoning). As an easy means of correction install a potable water approved hose (nsf standard 61, or equivalent) on the right-hand branch of this "y" splitter. 2) the three compartment sink area: the same black hose mentioned above is used to connect the potable water supply (three compartment sink faucet) to a chemical dispenser without any back flow prevention. As an easy means of correction, replace the existing hose with one approved for potable water (nsf standard 61, or equivalent)
(Critical) Observed all of the yellow plastic pans (12+) on the shelf above the back kitchen prep counter to be unclean. Also, observed the following miscellaneous equipment to be unclean: the two salt shakers for the french fry station, a ladle used for a cheese sauce, and the magnetic frosty machine spinners/agitators used in the top refrigerated hopper. Take necessary steps to ensure these food contact surfaces are thoroughly cleaned. Some of these pans may need soaking in hot soapy water and/or in a degreaser (if plastic safe) to help remove the build-up. After these containers have been cleaned, sanitized and air dried, ensure staff are inspecting the pans for cleanliness prior to using them or stacking them on the storage shelf

Ensure these pans are thoroughly soaked and cleaned with a scrubbing pad approved for use on plastic equipment. After these pans have air dried, staff need to inspect them for cleanliness prior to placing them on the clean, ready to use shelf

Observed shut-off valves down-stream from the built-in atmospheric vacuum breaker installed on the mop sink faucet. The existing vacuum breaker is not compatible with any shut-off valves located down-stream. As a means to correct this issue, install a flow-through type back flow preventer (such as an eco-lab "side-kick" unit, or equivalent) between the metal faucet and the existing "y" splitter.
Observed condensation beaded up on the bottom of the bare refrigerant line for the walk in cooler evaporator coil. Correct by insulating this line to prevent the condensation from forming

(Critical) 1) the water line serving a chemical dispenser at the three compartment sink does not indicate it is an approved potable water hose and there is no back flow preventer just upstream from this hose. This situation can be corrected by providing a food grade hose to connect the potable water supply to the chemical dispenser (the dispenser itself has a back flow preventer to prevent chemical back-siphoning). 2) mop sink: the faucet does not contain a back flow preventer. Attached to the end of the spout is a "y" splitter. One branch is connected to a chemical dispenser via a non-potable water hose. The other branch has an atmospheric vacuum breaker and a short length of open ended hose to fill a mop bucket (no violation for this branch). As a correction install an ecolab "sidekick" flow-through back flow preventer (or equivalent) on the branch of the "y" splitter leading to the chemical dispenser

If possible reorganize the prep line to keep potentially hazardous foods as far away from steamer as possible to maintain proper temperature of 41 degrees f or below. Will recheck modifications to storage containers and reorganization on prep line

The base coving at the floor wall juncture by the mop sink has fallen off the wall due to water damage. It appears that standing water in this area has caused severe water damage to the wall and the area behind the base coving and needs replacement possible of the wall in addition to the base coving and possible leveling of the floor to prevent water from standing in this area and further damage to this wall/base coving section in the future. Repair proble area and replace damaged floor/ wall/ studs as needed
