(Critical) At the server station observed three cans of liquid fuel for chafing equipment stored on top of clean plates. Item corrected by relocating the fuel to an approved area. The dished were sent for wash, rinse, and sanitizing. Item corrected
(Critical) Observed the bar blender to not be washed at closing time after the last use. Corrected by properly washing.
Observed a white ice transfer bucket (stored on top of the one door upright cooler at the habitat bar) to have the inner surface starting to separate and peel up. Corrected by taking this bucket out of use.
The bar area ice transfer pail (clear plastic with blue tint) was observed hanging upright on the end of the bar glass drainage rack. Keep this pail inverted to allow moisture to drain out and to prevent the accumulation of misc. Debris
The floors in the following locations are in need of cleaning: cabana--out of the way areas (where the floor meets the walls primarily) and in the habitat bar--the white floor sink under the ice bin
Two of the plastic dividers on the bar citrus slicer have some damage to them. Order a replacement slicer
1) a drain line for the bar tender side spill tray is disconnected at a joint. Re-install as needed to route waste water to the nearby floor sink. 2) a floor sink (possibly a decommissioned one) under the bar tender side of the bar near the right-hand ice bin was completely full of water. Unclog and clean this basin out as necessary. 3) excess water was observed on the floor under the dump sink/left hand end of the glass washer. Either a plumbing leak is present or (more likely) water is dripping down the gap between these two pieces of equipment. Repair any plumbing leaks if necessary or provide an easily removable metal cap to cover this gap and prevent moisture from leaking between this equipment. 4) syrup/liquor has accumulated on the bottom shelf in a base cabinet on the server side of the bar beverage station. There is likely a leak in one of the beverage lines in this cabinet. Clean the area, locate and repair any leaks as needed
(Critical) Observed pulled pork on the buffet table at 116 to 122f. The chef believes this product was placed in a chaffing pan set-up that wasn't pre-heated early enough. At the time the pork temperature was checked, one of the sterno fuel cans was burned out. Corrected by providing another sterno fuel heat source under the water pan and by reheating the pork to 165f in the kitchen
(Critical) Observed a thermometer in the cabana four door cooler to be reading 37f, when the temperature in the unit was 44f. Corrected by removing this thermometer from the unit for adjustment
Observed a small amount of sanitizer solution at the bar to be too weak (<150 ppm). The bar tender will dump this solution and obtain fresh quat sanitizer
Provide a certified food safety manager for this portion of your operation within 90 days
Around the floor sink for the glass washing machine: eroded grout was observed creating depressions where debris and moisture are accumulating. Remove any remaining grout in this area and re-grout as needed to make this area cleanable
The bar ice bin (right hand one from behind the bar) has a hole in the bottom edge. Repair/replace to eliminate the leaking water. If a repair is made, approved materials must be used and the finished work must be smooth and easily cleanable.
Observed a lot of splashed food on the sides and undersides of the shelves at the dirty dish bussing station. This food accumulation will attract pests. Clean this area now and on a daily basis in the future

(Critical) Observed an unapproved insecticide spray located at the habitat bar. Corrected by removing from the facility. Ensure only insecticides approved for use in a food service facility are utilized according to the label directions
1) observed the drain for the water filling station at the habitat bar (located under the perforated drain grate) to have a slime build-up that is a likely fly breeding area. Ensure staff are applying your fly drain treatment to this out of sight drain as well. 2) the side-walls and the undersides of the shelving at the habitat bar dirty dish bussing station have splattered foods on them. Ensure staff are wiping down these surfaces at the end of each night along with their other closing chores to eliminate these food source and/or breeding area
Habitat bar: there are still a few areas of the flooring that are in poor repair (worn/missing grout and tiles). These depressions are allowing food debris and moisture (from melted ice, spilled drinks and mop water) to accumulate and provide a breeding area for flies. Repair the flooring as needed in the bar area to eliminate these depressions
Habitat bar: observed a few small adult flies clinging to the ceiling at the beverage station area. Ensure staff are regularly checking the facility for the presence of pests and taking appropriate action to eliminate maintain the out of the way areas clean and dry
(Critical) Observed a few fruit flies in an open bottle of vermouth. Corrected by discarding. Provide a way to physically prevent access to this food by flies (plastic wrap or liquor bottle caps)
The following areas at the habitat bar are due for cleaning: the floor under the dump sink and glass washer, and the under sides of the wooden shelves for bussed dirty dishes
Cabana service location: the bar blender and the pop corn scoop did not get cleaned after the last time this facility was operated. Corrected by cleaning this equipment. Ensure staff take apart the bar blender prior to cleaning the to ensure all of the surfaces get fully cleaned
Habitat bar glass washer: observed a piece of bare wood on the floor used to maintain a drain pipe in place above the floor sink. Corrected by removing the piece of wood and replacing it with a piece of pvc pipe.
Observed accumulated water on the floor in the basement in the vicinity of the walk in liquor room, the small walk in keg beer cooler and the hallway from the small conference room to the employee only area leading to the ice machine area
(Critical) The habitat bar menu needs some revision to be in compliance with the consumer advisory. A couple of items need asterisks added, and a couple items need to have asterisks deleted (prime rib/prime rib french dip and chefs soup of the day).
The square floor sink serving the bar glass washer and its grate cover are over-due for cleaning. Please clean these areas now and more frequently in the future
Observed several out of the way areas where floor tile has missing or worn tile grout (employee side of the bar under ice bin/beverage dispensing area near the floor sinks, etc. ). Remove any remaining grout in these worn areas, thoroughly clean and dry the floor and then apply a full thickness layer of grout to eliminate the current depressions
