(Critical) Observed packaged cupcakes without the required package labeling. Provide labels that provide the required information

Meat walk in cooler--observed the following cross-contamination concerns: raw beef stored above crab meat and raw oysters (for raw consumption); raw beef next to packages of hard boiled eggs; raw chicken next to deli ham and roast beef, and raw bacon above ham. Corrected by relocating these foods so all ready to eat cooked foods were above any raw animal foods and that the raw oysters were stored below the ready to eat cooked foods but above the raw meats and poultry

Observed condensation droplets on the bakery walk in cooler drain pan and the insulated line off of the drain pan. Also, in the produce cooler the insulated drain line is dripping condensation on to packaged foods. Insulate/re-insulated these areas as needed to prevent the accumulation of condensation

Bakery wic: observed a square plastic container labeled "cheese base" stored on the floor. Corrected by elevating off of the floor onto a shelf. Produce wic: observed a bag of parsley on the floor in the center of the cooler. Corrected by relocating this food to a shelf

The floor in the following locations are in need of cleaning now and more frequently to remove spilled / spoiled foods and/or accumulated debris: bakery--the floor sink under the dish washing station, the walk in cooler; large walk in freezer in main hallway--spilled food on floor; produce wic--excessive spilled food on the floor; the large drain basin next to the compressor room

Numerous wire rack shelves in the meat walk in cooler have rust flaking off of them. Either remove the corrosion and resurface these shelves or provide replacement shelves that area smooth and easily cleanable

Dry storage hallway: observed a couple bags of onions stored on the floor. Corrected by elevating the onions to the provided shelf. Produce walk in cooler: a box with four heads of cabbage and a bag with beets in them were on the floor. Maintain all foods at least 6" off of the floor

Bakery: observed +/- four feet of exposed metal water pipe above a shelf where the counter top mixer is located and above some stored pots that is dripping condensation on these surfaces. Install pipe insulation to prevent this condensation from forming

The interior of the two side by side ice machines are in need of cleaning and sanitizing again to remove the slime build-up

Observed an excessive amount of spilled fruit and vegetables on the walk in produce cooler floor under the shelves/dunnage racks. Ensure the floor in this unit is cleaned regularly to remove spilled food before is spoils

(Critical) Some required information is missing on the cookie labels for the bagged cookies out for sale at the main desk area counter. Update these labels as required and submit copies for review prior to printing/using them

(Critical) The interiors of the ice machines are in need of cleaning to remove accumulated slime (pink and brown). Basement hallway: several of the dessert dishes and a couple of cream pitchers in racks for clean equipment storage were observed to be unclean. Corrected by soaking and re-washing

(Critical) Bakery/banquet walk in cooler: observed a partial case of citrus stored on a short stack of raw, in-shell eggs with three cases of eggs stored immediately adjacent and above the citrus. Staff indicated this is not a normal storage location for the citrus and relocated the citrus to on top of a bakery storage cabinet.

(Critical) The large sink basin in the banquet area hallway is being used as a dump/utility sink as well as a hand washing sink. As a correction install a small hand washing sink to the left of this dump/utility sink and relocate the soap and paper towel to the small hand washing sink area

(Critical) Although the shell stock tags for product already sold are organized in a filing system, the date of the last sale for each product is not written on the individual tags. The chef indicated 6 to 7 employees would be involved with these shell stock tags. Ensure these employees are made aware of the requirement to place the last date of sale (using a permanent marker) on the tags prior to storing them in the filing system in chronological order

Debris has accumulated on and around the low sink basin across from steve's office that receives waste water from the water cooled compressors and the steam kettle floor drain from the main kitchen. Maintain this equipment/floor area clean to prevent pest breeding areas. The floor under the first dunnage rack to the right as you enter the dry storage room has misc. Items accumulated. Clean this floor area to remove debris. Three banana peels were observed in the different locations in the dry store room hallway by the walk in freezer and produce cooler. Ensure staff dispose banana peels in the trash only

1) cases of food service gloves were being stored on the upper shelves of a storage unit across from steve's office. These gloves are located directly under a sewage drain pipe. Relocate these gloves to an area not located under a source of contamination. 2) a rolling cart with 2-3 trays of dessert dishes was stored in the hallway just outside of the bakery. Many of these dishes were stored upright. Cover these trays of dessert dishes with a plastic bag to prevent misc. Contamination. 3) some large black plastic bowls in the dry storage room were not bagged and stored upright. Corrected by inverting these bowls to prevent misc. Contamination

Many of the interior surfaces of the basement keg cooler have yeast/mold growth on them. Thoroughly clean and sanitize this unit

Provide a certified food safety manager who works in this area of the food service business or directly oversees this operation.

The mop sink in the hallway to the dry storage room is damaged. Replace this basin and properly seal it to the floor

An employee in the bakery has very long hair. Although this employee has their hair tied back in a pony tail and is wearing a hat, their hair hangs down over food when bent over. Provide a way to additionally secure this long hair (hair net, braids, tuck it under hat if possible, provide additional hair ties to secure the long loose hair, etc. )

Ensure staff are designated to monitor for pest activity (primarily flies) in the produce storage area and take necessary actions as needed to eliminate food sources and breeding areas

Storage room: 1) observed the "mop sink" type drain area to be overdue for a thorough cleaning; 2) produce storage area--observed old potatoes, onions, bananas, etc. On the floor under the shelving that is starting to deteriorate; 3) walk in cooler--observed excessive produce on the floor under the shelving and dunnage racks, some of it moldy; 4) the floor drain (under the cover) by the walk in cooler and freezer is due for cleaning to remove debris accumulation. Keg cooler: observed misc. Debris accumulation on the floor (some of it moldy). Liquor dispensing room: observed a stagnant puddle of water on the floor. Thoroughly clean these areas now and more frequently in the future to eliminate the food/debris accumulation that can lead to a food source or pest breeding area

Increase the cleaning frequency for the basement ice machines

Catering/banquet equipment storage room: observed a small water bottle that someone has used to collect their chewing tobacco spit in that was placed on the top of one of the shelving units. This bottle was located adjacent to wooden bread boards and above misc. Equipment. Corrected by removing this bottle and discarding it

Bakery: observed a case of individual butter packages to be stored on the walk in cooler floor

Bakery: the fan grates in walk in cooler are in need of cleaning again to remove dust accumulation

1) observed a leaky connection on the right hand side of the right hand softener resin tank. Repair as needed to eliminate the moisture on the floor. 2) provide a replacement atmospheric vacuum breaker on the hose water connection outside the walk in freezer in the storage room

1) liquor dispensing room--observed stagnant water on the floor. A maintenance employee believes this moisture is coming from a leaky floor on the main level (at the habitat bar) when staff perform their daily mopping of the floor. Repair the floor as needed to be water-tight. 2) bakery--observed some small areas of the floor that are missing tiles. Repair to provide an easily cleanable floor surface

(Critical) Observed a wastewater drain line above the basement walk-in meat cooler to be actively leaking from a connection joint. Maintenance staff were made aware of this leak and were taking immediate steps to eliminate the leak. ,

The floor in out of the way areas in the bakery walk in cooler (under shelving and dunnage racks) are in need of cleaning at this time

Observed multiple small areas of the bakery floor to be missing floor tile. Repair as needed to provide a flat, easily cleanable surface

The walk-in meat cooler in the basement: the metal ceiling is corroded/deteriorated and has holes in it. Repair/replace this ceiling to be smooth and easily cleanable

The cooling system in the bakery walk in cooler is dusty and in need of cleaning

Burned out light bulbs were observed in the small keg cooler and the produce coolers in the basement. The bulb in the keg cooler was replaced during the inspection

Observed cardboard being used as a shelf liner in the dry store room for some monin syrup bottles. This cardboard is not easily cleanable and absorbs moisture. Replace this cardboard with a non-porous, multi-use material that can be cleaned when necessary.

The three ice making machines located on the lower level are due for cleaning at this time. Please clean these units now and place on a more frequent cleaning schedule

(Critical) Observed a significant amount (+/-15 to 20%) of the dessert ramekins to still have food residue on them. Corrected by sending these dishes to be re-washed. Ensure dish washing staff are inspecting these items before they are sent to the bakery for use

The banquet two door hallway cooler (traulsen unit) was only maintaining foods at 44 to 48f. Potentially hazardous foods are not being stored in the unit currently. The temperature control dial was at its maximum setting. Repair this unit asap to maintain 41f or below. No phfs may be stored in this unit until it is repaired

1) observed the underside of the wooden work table in the bakery to have a lot of dust accumulation. 2) the fan grates on the bakery walk in cooler have excessive dust accumulation on them. Please clean these areas now and more frequently in the future

The floor and floor-wall junctions in the following areas are in need of cleaning at this time: the bakery walk in cooler, under the dunnage rack for flour and the shelving units along the walls in the dry storage room

Bakery: observed significant amounts of dust on pipes/ducts, etc. At the ceiling. Please clean these areas when foods and equipment can be protected. Banquet: the air handling system located above the hallway near the coffee brewing area is also due for cleaning to remove dust

Observed four square pads under the legs of the counter top mixer to be rough and not easily cleanable. Please replace these pads with something smooth, non-absorptive and easily cleanable
