(Critical) Caesar salad dressing found in the display cooler with a consume by date of 9/13/2012. Food is past the expiration date and must be discarded. Corrected by discarding food at time of inspection. Due to 2x repeat critical violations of 3-501. 18 a risk control plan template was provided to revise sop on this violation.

(Critical) Frozen raw turkey in tub thawing under cold running water in a mop sink that was not clean. Discontinue this practice immediately. Thaw foods in the 2nd or 3rd compartment of the food prep sink only after proper washing and sanitizing of the sink compartments. Corrected by removing foods from the mop sink and putting into the 3c sink as indicated above.

(Critical) Opened gallon container of skim milk found in the walk in cooler with a best buy date of march 30, 2011 and unmarked with a consume by date. It is unclear as to when the milk was opened, but it was found without a consume by date. Once opened provide a consume by date not to exceed a maximum of 7 days from when the container is opened or food is prepared. In addition watch the dates on containers and do not exceed. Corrected by discarding milk at time of inspection

The condensate line in the walk in cooler is not insulated. Provice insulation on this line to minimize potential for condensate to get onto foods stored under the line.

(Critical) Two sterno containers found on prep table next to togo utensils and two cartons of sterno on shelf bench with paper plates stored on top of them. Store sterno separately away from prep areas and togo utensils and paper plates. Corrected by reorganizing these areas and storing sterno properly

(Critical) Two spray bottles with clear and yellow liquid found hangin under the chemical storage shelf. Properly label spray bottles as to their contents to avoid mix up and mis use. Corrected by labelling these spray bottles as to their contents at time of inspection

At onset of inspection, employees were not wearing proper means of hair restraint. Provide and use ball caps etc to minimize potential for hair to get onto foods. Corrected by using ball caps

Domestic chest freezer in the facility is not approved. Replace with commercial equipment when in need of repair.

The cold water supply line to the handsink is leaking. Therefore they are turning the cold water on and off at the valve below the sink basin. Repair leak as soon as possible so that the cold water knob above the sink can be properly used and have a continuous supply of non leaking cold water.

Corrected during inspection by properly date marking containers of food - day opening or preparation to be counted as day #1 plus 6 days - example open or prepare food on 10-15-2010- consume by 10-21-2010.
