(Critical) Opened milk used for making shakes was not date marked. Neither was the opened whipped cream. Staff need to be date marking these items with open date and use by date(6 days added to open date). Corrected by dating.
(Critical) A carton of raw shell eggs was found stored above ready to eat dairy in one of the display coolers by the front counter. Staff need to be keeping raw animal product stored safely below any ready to eat foods to control the risk of cross contamination. Corrected by storing eggs on bottom shelf.
You need to clean your exhaust hood filters more often. You also need to clean the splash wall behind the fryer more often. Repair the hole in the wall by the fryer so debris can't get behind the walls.
You need to obtain food grade containers to store your foods in. Discontinue using the sterlite/rubbermaid brand containers.
No one got certified as a food safety manager as i reminded you of last inspection. By 5/10/15 you need to get someone that works here full time certified and fax me in a copy of the certificate by that date.

(Critical) I observed chemical cleaners stored with food sprays, thermometer on a shelf in the fry area. Keep toxic items stored in safe location away from foods and food contact items.
Within a month, you need to have a service sink put in that is plumbed directly to waste. Your gray water discharge from the washing machine will need to be directly plumbed to sewer, as well. . . . . All condensation drain lines out of your walk in coolers/freezers need to be provided with a visible air gap before connecting to waste. I need you to have all this completed within a month and i will come back to verify completion.
The hood filters over the fryer needs to be cleaned more often. Also, the room fan gaurd needs to be cleaned more often to preclude the dust build up.

(Critical) The sanitizer strength was too strong in the front 3 comp sink. Need to keep at 50-100 ppm. Corrected by making up new batch.
I observed coats lying on food products. Staff need to be using the coat hooks supplied behind the door to the loading dock area. Corrected.
Please clean the hood filters over the donut fryer more frequently. Also, folks need to wipe out the interior of the microwave after uses.
No chlorine test papers in your store. Always keep stocked and use everyday to measure sanitizer strength.
(Critical) I observed several drain flies on and in the chocolate glaze bucket. You need to have a licensed pest company out to eradicate the flies. You also need to keep foods in tight fitting containers. This glaze was discarded. I will be back in 3 weeks to verify flies have been eliminated from your store.
(Critical) I found cartons of raw eggs stored above beverages in the retail cooler. Also, there was raw bacon stored above ready to eat foods in another retail cooler. Always store raw product where there is not any ready to eat foods/beverages underneath or directly next to the raw product. Corrected by making the above changes.
(Critical) I found several dented cans in the back storage area and one by the donut rack. You need to pull dented cans and put in a labelled location. I will be back in 3 weeks to verify you are in compliance with this.
One of your walk in freezers has a condensation leak inside the unit. Repair this leak asap!!!!
You need to provide new light shields on the lamps in the retail ice cream glass door freezer. Also, need to put shields over lights in upstairs storage area or use shatterproof bulbs.
