(Critical) There is a rather large infestation of drain flies in the kitchen, particularly near where the brooms are hung up. You need to have a licensed pest company out for the next 3 weeks to determine breeding sites and conditions that are creating the infestation of the flies, and the eradication of the flies and breeding areas. Save the work orders from your pest company for my review. I will be back in 3 weeks to verify the flies are out of your facility.

(Critical) The inlet to the dipper well was not air gapped. Corrected by having maintenance secure inlet so it is how air gapped. Obtain a longer standpipe so the water in the well completely submerges the ice cream scoop portion.

(Critical) The hot water temperature at the handsink by the dishmachine was at 63*f. Must be minimum of 100*f. All the other handsinks had water above 100*f. Maintenance adjusted a mixing valve and temp rose to 114*f. This corrected the issue, however, the kitchen needs to be on it's own dedicated hot water tank, and not have a need for any mixing valves. Have a plumber assess your hot water situation and write up a detailed explanation. The kitchen should ideally have hot water at the 140*f range. Fax or e-mail me the plumbers report by june 24, 2013.

(Critical) I observed the cook handle raw ground beef, then change his gloves, but did not wash hands before donning a new pair of gloves. Staff need to always wash hands before putting on a new pair of gloves. Corrected by talking to staff and making sure this procedure is being followed.

Several staff were not wearing hair restraints. All staff that have hair on their heads need to wear a hair restraint such as a hat.

(Critical) I observed several cans of chafer fuel stored next to and above single use food/beverage containers. Store toxic items in safe location away from foods and food contact items. Corrected by storing on chemical storage rack.

(Critical) Raw shell eggs were found stored above ready to eat foods in the line reach in cooler. Raw foods need to be seperated from and always below any ready to eat foods to control the risk of cross contamination. Corrected by doing the above.

I observed staff personal items(keys, sanitizer, pop can) stored on a rack with food contact items. Have a spot for staff "stuff" only that is seperated from foodservice items.

Stagnant water observed in the ice scoop holder. Staff need to remember to wash this once every 24 hours. Corrected. Also, clean the top of the dishmachine more often.

Replace the light shield on the lights over the coffee service area and install end caps on the lights in the hallway to the back loading dock.

You need to clean the exhaust filter over the dishmachine. The exhaust steam is not being captured and it needs to be.

Please clean the top of the dishmachine more often

(Critical) Found a recently made, large covered container of chicken tortilla soup 90°F on shelf in the walk in cooler. Pic corrected by reheating soup to 165°For 15 seconds, and instructed employee to properly cool soup in an ice bath with ice wand. Pic was reminded to cool soup from 135°F to 70°F in 2 hours, and then from 70°F to 41°F in 4 hours. Pic has been instructed to monitor that employees are properly cooling foods.

Found the lids of dumpter in open position. To correct keep lids of dumpster closed.

Found mouse droppings on the bottom storage shelf of the self service cold/hot holding buffet display unit. To correct clean bottom shelf, and continue pest control measures.

Found a wet wiping cloth in a dry sanitizing bucket. To correct store wet wiping cloth completely submerged under sanitizer in sanitizing bucket.

Found a cut out hole on the bottom area of the wall behind the self service hot/cold hold buffet unit. Found a wall panel missing besides the walk in cooler. To correct repair/replace damaged and missing wall panels.

Found dented cans stored on a separate shelf without labeling identifying dented cans, do not use, return for credit. To correct provide a sign identifying dented cans, return to supplier for credit.
