(Critical) The pizza was found at 97 - 107 degrees f. Potentially hazardous foods must be maintained at 135 degrees f or above. Make sure unit is set appropriately to hold pizza at 135 degrees f. There is another hot holding box that can be used if this one is not functioning properly. Corrected by pizza being discarded at end of lunch rush
(Critical) Sewer gnat infestation present again. Facility has exterminator contracted for control of sewer gnats. They were just here end of november but cannot spray while school is opened. Scheduled to come in over the weekend. The follow up will ensue after exterminator. If unable to control sewer gnats other options must be considered.
The hot holding box was not adjusted properly to hold pizza t 135 degrees f. Keep unit adjusted so that the air temperature is at least 160 degrees f. Make sure that pizza arrives at 135 degrees f and then is maintained during lunch service at 135 degrees f. Unit must be turned on in advance to maintain proper temperatures of the foods.
(Critical) Upon onset of inspection both food employess observe popping raisins into their mouths while serving and setting up trays with gloved hands without handwashing. This is prohibited. Discontinue this practice immediately. Hands must be washed if eating and put on new gloves. Do not eat while on the service line. Corrected by discussion and washing hands.

(Critical) Mashed potatoes found in the hot holding cabinet at 112 degrees f. Potentially hazardous foods must be maintained at 135 degrees f or above. Most likely this was transported from the commissary at improper temperature. Corrected by rehating in steamer prior to serving to 165 degrees f. Serve time max from receipt of food is 2 hours. . All other foods in the hot holding cabinet were at 135 degrees. Note that the setting on this unit was maxed out. Recommend new hot holding cabinet in addition to proper hot holding transport.

Recommend at new hot holing cabinet and must provide proper transport temps and maintain 135 degress f or above.
